You have to have a SilverFlume account online to do anything online. If you don't have an account, create one, it's pretty easy. BTW, To find the checklist, was a bit tricky, but it took a few searches on the website to find it. You have to get a bunch of other things done before you place it in your cart. I guess, this is going to be hard to explain, and one might not know what I am talking about, unless you've been through the online process.
I'll do my best to explain the steps. I went through it. I sent in the checks first, without completing the checklist, so my request was denied, via e-mail. TIP: have everything in place before applying. So, I completed the course (I think that it expires in a month, from when you start the course) and printed out the completion certificate, Yay. I had it in my cart. When it came time to check out, I realized, that this whole process would've been easier if I had selected paying by credit card (but I requested checks from the accounting dept). I didn't know what to do, so I e-mailed them to see what my options would be. So, I did that, gave all the details, and I got a response. I had to e-mail all of the supporting documents that were needed. I was able to get it approved so it went through. Oh my gosh, you can understand my relief, that I had done it.
The problem was I received my certificate in the mail in September, however, it was incorrect. A bright colored paper is enclosed, to make sure you check for any errors. So, I responded via fax. Well, it came again in October, and it was wrong, again. I was surprised, because both of the receipts had my correct name spelling. So, I responded, via fax, this time including my drivers license, stating that this was wrong again. I didn't get a reply, so I sent another e-mail explaining my situation. I didn't get a response.
I called this morning and spoke to a clerk who had me on the line, asking me to resend the e-mail so she could locate it. She was able to transfer the e-mail to her e-mail. She said that the system was down, but when she was able to, she would work on it. She sent me a proof to make sure that my name was spelled correctly. Hallelujah!
