Our family was told there would be active interaction with the residence, music therapy and other therapeutic methods that would be used to achieve the proposed individualized/focused care. The actual result is that NOTHING could be FARTHER FROM THE TRUTH! The REALITY is that immediately after moving in, while our loved one was getting adjusted to a new place, people and surroundings in the first few days, the immediate course of action from the Gianna's staff was to medicate him to the point of extreme sedation, so that he did not disturb the staff! This sedation was to the point of him being asleep in the chair all day every day and not moving, not eating and inability to be mobile when he had just walked into the place days earlier. At the direction of the Gianna's nurse, the staff issued PRN meds (as needed med's) automatically when they were not needed. He was in the chair asleep in the chair, and they continued to issue sedating medications. This was so severe, that one time I walked at dinner time and our loved one was laying on the floor with the staff walking around him, like nothing was wrong. Not helping him up or attempting to help him. The staff did not exhibit any compassion or attempt to find alternative solutions. This continued. Simultaneously NONE of the therapeutic methods promised happening. I am not saying this lightly as someone who was hearing this from afar. I witness this firsthand as I was there 22 of 28 days he was in Gianna's. This persisted and he did not come out of nor was able to recover from the extreme sedated state the Gianna's nurse and team put him. He paid the ultimate price died 28 days after entering Gianna's. Save your loved one! DO NOT believe what they tell you! DO NOT PUT YOUR LOVED ONE IN GIANNA'S. They too may pay the ultimate price like my loved one did! PS: As someone who was there nearly daily, I observed that of the following: Of the 14 residents at Gianna's ONLY 1 person was mobile on her feet and verbal. ALL others were confined to a wheelchair, nonverbal and appear to be sedated with no movement, verbal or physical interaction with staff. This was observed throughout many different shifts. My dad lived at Gianna for only 28 days: from the first day when he walked in on his own 2 feet…read more(able to ambulate, eat, and interact) to the day he passed away. We moved to Gianna because we were promised low staff to resident ratio, more individualized care, a "medication last" approach, experienced staff and a warm environment that treated residents like family. While the ratio was as promised, I felt the care, staff competency and medication approaches were not. Often, I found my dad sitting alone, in the corner, unable to move or open his eyes from sedation. With drool down his cheek and leftover food on his face. He would often have globs of liquid medicine in his mouth from staff trying to give medication while he is unresponsive to verbal commands and touch. I would often find him in this state with no interaction from staff leaving me to advocate for his needs. We felt like outsiders and an inconvenience. This was not the care I was promised. We had multiple instances of their staff not following hospice medication orders. We witnessed staff administer anxiety medication labeled for as-needed use when my dad was already so sedated that he was unresponsive. I was able to intervene while present, but who knows how much extra, unneeded medication was given when I wasn't. It was also administered incorrectly. Instead of administering sublingually (as stated on the medical order), they were dissolving the meds and shooting them down his throat with a syringe. This is incorrect and can be very dangerous especially if the person cannot swallow as medication can be forced into the lungs. I believe this was occurring as my dad would often cough after being given medicine this way. There was an instance where the doctor ordered a stop to a medication, yet staff continued to administer it for four days! For four days my dad was given discontinued medication IN ADDITION to the medication that replaced it. The head RN at Gianna locked the medication in a cabinet in her office to prevent staff from continuing to administer it. Apparently, she was not confident enough in her staff to be able to verbally tell them not to give this medication. When my dad was bedridden at end-of-life stages and NEEDED this medication, it couldn't be accessed. This caused a 2.5-hour delay while we waited for the medication to arrive. If that wasn't bad enough, once the medicine arrived, staff administered the medicine incorrectly. My dad began vomiting and choking after a few doses. We immediately alerted staff. There was no urgency by them to help which is VERY alarming in this type of situation. I would hate to see what would have happened if he was alone during this episode. After alerting the hospice nurse of what occurred, she had to call Gianna's staff to educate them on how to give medication sublingually as written in the medication orders. After the medication was given correctly, relief was seen almost immediately. I believe that none of the prior doses were effective due to incorrect administration. All this to say there was so much prolonged and unnecessary suffering that took place because of mistakes that should never have happened at a facility of this caliber. Sublingual medication at end-of-life stages is extremely common. It was shocking to me that their "experienced" staff were not familiar with how to give medication this way. These events support my experience of lack of competence and compassion within the staff and lack of proper RN oversight/direction. Medication mistakes are unacceptable and can be life threatening. This was far from the end-of-life experience I wanted for my dad and family. It certainly was not the experience I expected given what was advertised to me and the price that came along with it. While I should have been focused on enjoying what little time I had left with my dad, I felt forced to monitor staff to ensure they were performing care properly, giving the right medications, and administering medications correctly. I feel robbed of this time. The only silver lining is that my dad and family didn't have to suffer through the poor care and experience of this facility for long. Although our stay was brief, the damage caused by this facility was large. If you care for your loved one and the end-of-life experience you have with them, I recommend you steer clear of this facility.

We visited a family member who lives in the nursing home section of Trillium Woods. The complex is huge and we have appreciated that he has been able to stay at Trillium Woods as he has aged and his health care needs have increased. We used to eat with him in the restaurant when he was in assisted living. It was like going to an upscale restaurant. It was nothing like "nursing home food." The menu would change and provided the residents with a wonderful selection of options from which to choose. The activities are many and varied. We have appreciated that when the residents go on outings that the staff photographs them and shares them on Facebook. We have enjoyed seeing our loved one at different places, and seeing how happy and engaged he is with other residents and in the community. There is a library for residents and I was happy to see that he uses it regularly. At our visit, he shared that he had just read a book about Robert Frost, the poet, and had a new book about the he sinking of the ocean liner RMS Lusitania that occurred on May 7, 1915, during the First World War. Keeping the residents' minds active and encouraging them to read and continue to learn is impressive and needed! The nursing home section is well-maintained, elegant, and welcoming. There are public seating areas, water with lemon slices in it for guests and residents, a puzzle table, t.v. area, and dining area. When we arrived, the cloth napkins were beautifully folded on the table that had a white linen table cloth on it. This is normal...not the exception. The staff are friendly, helpful, and professional. They treat everyone with dignity and respect. I could only wish that when the time comes that I need nursing care that I could live in such a beautiful, well-maintained facility with top-notch staff and programming in the arts, music, and outings like Trillium Woods offers.

My Mother was at Amira for 4 years in the memory care unit. It was bad, and then it got worse…read more Every single time I went to visit, she was wet. Every single time! We even bought flow-through pads to use in addition to the depends, in case she was a "gusher". There were times she was wearing the depends and two flow through pads and was still soaking. FYI, that means she was sitting in 8 cups (1/2 gallon) of urine. I am just a dumb landscaper, but perhaps that explains the multiple UTI's that she had every year. She also had multiple falls (a result of dizziness from the UTI's) and a broken hip and later a broken femur. The best thing Almira ever did for us was kick Mom out, because it forced us to find another place for Mom. She spent the last 4 months at Breck Homes in Bloomington, where they made her feel useful and had so many outings that it was hard to visit her. Pretty is as pretty does, and Amira doesn't! If you are looking at memory care, visit here at meal time. Do these people look like they are enjoying themselves? Does the food look appetizing? Can they even hear each other, if they wanted to talk? Is there room to comfortably move around the tables with a wheelchair or walker? Yes, the fireplace is pretty. Is the room functional? Comfortable? Does the staff chit-chat with the residents, or do they just boss them around? Ask the employees you run into how long they have worked there, and ask if anyone has been there since the opening which is less than 6 years ago. There is only 1 who has been there the whole time, and she is the only one who knows how to get things done. Hint: it is Linda at the front desk. Raona

We would come visit her and she would smell like urine, she would have a dirty depends on for hours. She was barely washed with either a cloth or actual shower. She had 2 falls that didn't make sense, so we bought granny camera's, which were frequently turned off by staff until we told them it was illegal. Some of the staff did the bare minimum to get her ready for the day. She had 2 more falls which the staff was no where to be seen. She layed for 2 hours on the floor until they found her. Just when you think things couldn't get worse, it did. I sent flowers and a teddy bear to her one day and the teddy bear is no where to be found. My grandma contracted the Covid-19 virus here. She has since passed away. Unfortunately her death was not peaceful in any way. This company is absolutely horrible. The management is cowardly and quite literally pathetic. This company should be shut down. Our parents, grandparents, sisters, brothers and all other family deserve so much better. Please think twice before sending your loved ones here. I guarantee you, you will regret it, I do EVERYDAY. I wish she was still alive but at least she is no longer in pain .

I will be forever grateful to Spirit Care for the role they played in the final months of my father's life. This is a top notch facility that I fully and wholeheartedly recommend and endorse both as a physician and a family member.
