10/25-26/25 the facility had NO RUNNING WATER. they allowed me to be admitted on 10/28 knowing they had NO HOT WATER!! I have to be taken to while other wing of the facility for a shower. Today is 11/1/25 and there is still no HOT water. It's barely warm. Today I was served curdled milk. Was told by staff that over the weekend the generator had failed. After reading all the reviews I now understand why both the Nurse & Physical Therapist that would come to see my Aunt had the unprofessioinal attitudes that they did. The first appoitment for my aunt to see the Physical Therapist she called and told us she would be on her way she was coming from South Philly which was fine. As soon as she gets here I let her in and told her I would get my aunt ready to come down and went upstairs. When I came back down she immediately has an attitude and tells me that she can work with her upstairs and that I was moving to fast for her to let me know in addition to her needing me to sign some things. So she comes upstairs and check her vitals and see's that her BP was to low. She says because of that she can't do anything with her becuase of chances of her passing out in the process which is understanable but then she's says "I wish I would have know, I wouldnt have wasted my time driving here all the way from S Philly (we were in the NE). Then she goes on a little rant about how she hates driving on the Blvd because of the camera's which slows her down smh. But thats a little about her she came with the same attitude each time now on to the Nurse. He's was so flirty with her each time he came to the point my aunt would ask me not leave the room while he was there. But whats even more shocking is that we got 2 seperate letters from Deer Meadow that had "HOW DID WE DO " Survey's. Not sure if its a new way for them change things around or not but I now completely understand their behaviors. If your employer's don't hold you accountable for your behavior then why change it? OH and one other thing when we tried to get a new Physical Therapist they told us she was all they had in the are (not sure what area if she's driving from S Philly) and they did say they received feedback about how she is all the time. deer meadows needs to shut down that place is a big mess staff are lazy they do not take care of the patients like they should they go there just for a paycheck, residents are left sitting in their own filth they have to do better the state comes and nothing is being done I wouldn't recommend this place if it was the last thing standing. read more

Do not bring you loved one here. The building is in ruin. Elevators broken for more than a year. The food is inedible. My mom has lived here for more than a year, and it's awful. The administrator is disengaged, at best. Avoid this place. read more

They changed poa papers when the patient was not able anymore to make decisions. That is against the law. The floor patient is on smells terrible and they have mouse traps in her room. One day the aide called laughing hysterical while the patient was irritated. Patient was screaming get your hands off of me. It was very rude for the aide to be laughing and just hung up on me. She couldn't even get her name out. She was laughing so much . Not professional at all. read more

My sister was a patient at Deer Meadows Short-term Rehabilitation Center 8301 Roosevelt Boulevard, Philadelphia, PA 19152 between September 1 and September 20 of 2023. Her care there was appalling. When I visited my sister (for whom I act as healthcare proxy) I observed indifference and inattentiveness by the nursing staff. I was not surprised. PA State records show a 50 percent turnover among nurses in the past year. (I would be curious to know more about the credentialling of Deer Meadow "nurses".) An example of inattentiveness was the day my sister was dropped while being transferred from her bed to a wheelchair while being taken to physical therapy. This event led to the social worker at Deer Meadows, Martina, filing what she called a formal grievance. Another example was the consistently wrong meals delivered to my sister's room. Her doctor has her on a soft diet because of her current difficulties swallowing. Instead, my sister was regularly served beef or baked ham. One day she showed another visitor how she was unable to cut a piece of microwaved frozen fish. She picked it up and tapped it against the side of her tray to hear the sound it made. The climax of my sister's stay came on September 20. She was transported that day to Jefferson Hospital for an appointment with her doctor. When she got there the transportation folks wheeled her to her doctor's office and walked out, leaving her. Finally, a Jefferson receptionist noticed her and called the doctor. He was alarmed at how her physical condition had deteriorated while at Deer Meadows. He sent her immediately to Jefferson's Emergency Department. I never heard a word from Deer Meadows about my sister's admission to the hospital. No one called. No one even noticed that her room was empty. A meeting I was scheduled to have with Martina, Deer Meadow's social worker, was still scheduled to happen the next day; the social worker did not know that my sister was gone either. When I arrived at the facility to retrieve my sister's clothing, I asked at the reception desk for her room number. I got it right away because they still listed my sister as present. When I arrived at my sister's room I found that debris from the previous day's breakfast was still there on a tray beside her bed, untouched. I went and found the head nurse, who appeared to know nothing about my sister's disappearance from her care. The nurse walked down to my sister's room to confirm that she was really not there. "I guess someone should have called you," she said. My sister, who died on October 16, told me more than once in her last weeks that "I went backward at Deer Meadows." I'm not sure what my next steps are. What I do know is that I want my sister's experience at Deer Meadows to be a formal complaint to the state of Pennsylvania so that no one else will ever have the experience she had. read more

My MIL is a current resident there and is treated so poorly. We have complained and four staff member went in to her room to questioned her, we feel she was intimidated her to change her story! She has been denied meds because they didn't give her the meds on time. They said she had to wait until the next med administration time. They don't bathe her daily, they treat her terribly. We call and no one returns our call. You can't get anyone on the phone, all we get is answering machines. read more

My father and step mother are current residents there and are under personal care. My step mother is on oxygen and the portable oxygen that the machine is suppose to keep full doe's not work most of the time well that's a joke because it's always empty and nobody doe's anything about it. My father complains about how bad the food is and that they are putting a lot of black pepper in the food, he has asked many times for them not to put black pepper in his food and they keep doing it. When I go and see my father I have to talk a little louder than usual, because his hearing is not so good and because of that,I am no longer aloud in the building. My father is 92 and in a wheel chair. My father cry's to me on the phone that he missis me and loves me. Now the only way to see him is under supervision by there staff which I do not trust. I have called (Sarah) the manager many of times and she never return phone calls. Who do they think they are to take a son away from his father and a father away from his son , that just go's to show you how much they care. This place should be shut down. read more

I wish i can give it below zero stars. The only nice people there are the physical therapy team and one amazing nurse named Nesrine. The rest are awful. They don't answer the phone calls. The staff don't answer the patients call either. They leave patient unintended for long period of times. They give the wrong dose in medication and the wrong time. They have broken wheelchairs that is a safety issue. They miss pt appointment due to lack and miscommunication with transport and won't even apologies for it. They lie in your face. they leave pts in the same diaper for days. If you don't wear a diaper then they dont come to you when you need to go to the bathroom and they let you wet the bed and won't change your sheets. Very understaffed and the staff that are there are not professional I can go on and on. Just don't go there period unless you hate yourself so much. read more

Horrible, disgusting place. The stench of urine is sure to burn your nostrils. Wait 'til you see how the aides fight and curse each other at the nurse's station. Horrible experience didn't treat my family member right never dressed his bed sore got so bad was only red at first but now it's black they dressed him in shorts and a t-shirt to go which is so disrespectful to him he's basically is just skin and bone and it's quite honestly just not right he also fell on his face one day and they didn't even bring him to the hospital to see if he broke any bones he has bruises on his legs and one on his face. They sent him to dialysis dressed in shorts and a t shirt letting everyone see his skin and bone and he was clearly not in the right condition to go to dialysis health wise he couldn't even sit up right in his chair so I don't know why they thought it was okay to send him and why not just send him to the hospital read more

I would have given then a zero as well. My dad went there because he was promised his foot doctor was on the team which was a lie then, He is now on the team 3 1/2 weeks later. He is a diabetic and they don't give him the right dosage of meds. I asked repeated for an endocronoligist for him because he has blood sugars in number of 400 to 500 which he has never had. The social worker is nasty she is trying to pressure me into keeping my dad there . I keep telling her no she then tells me the insurance wouldn't pay much longer. i called the insurance myself they said that wasn't true. She then tells me she can't release my dad until she knows he is gonna be safe. Although I told her I have a back up system . The y leave him in a diaper soaked all day. They keep the phone away from their reach and the nurses station never answer the phone. They had my dad in quarentine for 15 days when he came from the hospital he is vaccinated fully. They didn't bother to tell me I was able to see him until i asked then they told me oh yeah you could have seen him 2 weeks ago. Magically ,the day I had an appointment he was sent to the ER . I ran to see him so I never stepped foot in the Rehab so far.His meals are not for diabetics . he does get therapy on and off. I have told them if I where to place him in a nursing home it would be near my home in the bucks county area. At this point, I have complaint to his insurance which includes medicare . I am seriuosly thinking of getting a lawyer they are violanting his rights as a patient . read more

My dad stayed at deer meadows twice last year for less than a month each time. First time was after being diagnosed with stage 4 cancer and a prolonged hospital stay, he needed therapy. He was there about 3 weeks. Second time was for IV antibiotics for sepsis, that was a 2 week stay. Both stays he had a single room, private bath, flat screen tv, and plenty of room for visitors to sit ( I brought chairs from home to sit and visit). My dad is a smoker so we spent a lot of time out in the garden. My dad would save his bread and give it to the birds. Overall, he had a very pleasant stay both times. Staff was kind, gave meds on time and kept his room clean. My dad was able to do things for himself so he didn't require any assistance from cna's regarding care so I can't comment on that but I did see them doing their work with other patients. It's a nursing home with incontinent people so of course you will smell unpleasant things, and that didn't bother him, it goes away after a few minutes. I (and my dad) would definitely go there again if needed. read more

As a person with insider info I can tell you that there are some really good people that work here however the place is run like a realty company from the top brass your family members will not be cared for depending on where they are at the independent side is good the assisted living is good the mental health side should be shut down and the rehab side is a joke...they pay for false reviews of how great it is but the execs could care less about each resident if they pass away they don't even give 8 hours before they toss the belongings and rent out the bed....in a nut shell there are some good employees and administrators but the owners will Nicole and dime you for some really horrible care in return read more

I am an employee and we had referred one of our patients to Deer Meadows. The patient reported that none of the staff answers the call bell. Nursing has been administering the medication late. Called several times to talk to management and unable to reach anyone. The worst retirement/skilled nursing facility. read more

This place should be shut down . My Grandfather sadly passed away from Stage 4 bed sore dehydration, malnutrition and sepsis all of which stem from an overall lack of quality care and attention. There were several incidents in which he was given the wrong medicine for his heart and the nurse's even overdosed him to keep him sedated so they would not have to take care of him. When the staff could not get him to come around after heavily medicating him, my Grandfather was sent to Nazareth Hospital. While at Nazareth a nurse said to my family most of the patients that come from Deer Meadows have the same bed sore just not as bad and are malnourished. Please if you have a loved one this is not the place for them I wish that someone would have gave my family this information before he was sent there.
