I was recently a patient in Signature HealthCARE for physical rehabilitation. At first, I was…read morepretty impressed with the care I received there, but something happened over the course of time to cause that care to deteriorate. There were definitely some bright spots, but unfortunately the bad experiences color my perspective. There are some great people working there. The care I received from the occupational therapists and the physical therapists was top-notch. I can't say enough nice things about them. Most of the nurses and nursing techs were awesome, but there was no continuity with them. Apparently the hiring shortage has caused the facility to draw on an agency to fill in. Those agency personnel cost the facility three times what Signature pays their regular staff, and the agency folks don't seem to be nearly as well trained nor caring as the regular staff. The cleaning crew are terrific people, personable and friendly as well as thorough in keeping the rooms and adjoining bathrooms clean. I had some really interesting conversations with them while they were doing their work. The kitchen put out some very good meals. To be honest, there were a few hiccups where the food was concerned, but the head of the kitchen visited me on a couple of occasions to ask about my food preferences and what suggestions I might have. She went out of her way to pick up some condiments that aren't generally on the food trays, ie. crushed pepper flakes and hot sauce. A couple of items that I really need to mention. The facility physician was practically non-existent during my stay there. He didn't lay eyes on me the entire time I was there. He made all his medical decisions based on the nursing notes; many of the nurses didn't chart things correctly or at all, so it was a hassle getting some of the meds that I needed dispensed properly. For instance, I'm on a daily psyllium supplement and it was regularly missed. I developed an infection and the wound care nurse never bothered to show up at all. That care was left to the floor nurses, and even they needed me to remind them that I had a bandage that needed changed. My biggest complaint was not getting a shower when I was scheduled. There were days when the techs were working short-staffed and just couldn't get around to make sure I received a shower. Some days I was promised, "We'll get you tomorrow," and sure enough, tomorrow came and went with no shower. This happened on multiple occasions. I witnessed agency staff arguing with patients with dementia, treating them more like objects rather than people. I heard them yelling at other patients; granted some of the dementia patients are abusive to the staff, but must be given some slack due to their mental condition. However, a patient getting rude with a staff member is to be expected sometimes, but having the staff yell back at the patient is uncalled for and very unprofessional. I can't say that there was any physical abuse of patients, but I did hear some very unprofessional verbal exchanges going on. I've tried to be honest and fair in this review of Signature HealthCARE of Clarksville. I'm aware of what the pandemic has done to cause staffing shortages. But Signature needs to be aware that word gets around about how little the staff makes, yet they'll spend twice to three times as much for temporary help. If the regular staff were paid what the agency personnel were paid, perhaps Signature's employee absentee rate would go down. Folks would be much happier coming to work, than complaining within earshot of patients (and sometimes complaining to patients themselves) that their payrate is low, yet the agency rated personnel get so much more for doing so much less. I wish I could name all the wonderful staff who went out of their way to make my stay there comfortable and easy, but they know who they are. I would brag on them and praise them for the quality care they were giving me amidst the mediocre care I received from some of their co-workers. So overall, there is much good I can say about Signature, but there are a LOT of things that need improvement.

worst facility ever experienced. The manager does not respond to…read morerepeated requests for weeks to even return the first phone call. Best communication occurs through corporate office which repeatedly tells me that they apologize and don't understand Clarksville facility's business practices.
