I would estimate this happened about once a quarter. I would start off each call with them explaining what had happened the time before and would let them know that they were consistently mixing up the accounts. This all ended when they closed my business debit card and cut me off from my money while on a business trip because I had requested a new personal debit card. I finally had to close both accounts.
