    Their memory care staff did not inform me that my mother had stopped eating and drinking until 3…read moredays had passed. She had also had 3 more falls I was not informed about. My mother was on hospice care and I should have been allowed to visit. By the time I was allowed to see my mother, she was incoherent and writhing in pain. My father required further assistance that he frequently did not receive. When he started hospice care, the director tried to deny my visits, and spoke to him about moving to memory care behind my back. It took an intervention from hospice care for me to be allowed to see him. The rest of the staff seems caring and pleasant, but I wouldn't trust the director to care for my plants.

They provide delicious meals with special needs taken into consideration. The home is always clean and bright. My father is always clean, bathed, and dressed in clean clothes. The grounds are beautiful and enjoyed by the residents when weather allows. The owner, Nucharee Yokdang, is very knowledgeable about running an assisted living/group home, including being able to help with all the necessary paperwork for long term care reimbursement and hospice. She knows just what the elderly need from a care perspective and is always looking for ways to improve their quality of life. The Holiday parties and other activities they do are wonderful for residents and family members. The whole staff treat their residents with such love, care, and respect, you can tell they view them as part of their own extended family. The greatest compliment that I can give though is the fact that my father calls it HOME! I feel so blessed there is such a wonderful place with amazing people for my father to spend his last years. The level of incompetence, unprofessionalism, disorganization, horrid communication and lack of ethics is staggering and dangerous, which ultimately puts your loved ones at risk. In point of fact, within the first twelve hours of arriving at Skyline, my mother was not properly supervised and was allowed to fall and catastrophically break her ankle. Her orthopedic surgeon said she will never walk again. I initially accepted that sometimes these things happen, but given all that I witnessed over time, I realized that this may have been easily avoided. Of note, this happened under the more competent previous administration. As a retired clinical social worker with over 40 years of experience in human services with federal, state and local facilities, this is by far the worst I have witnessed. Please note that everything stated here pertaining to the issues and communications is objective, and documented with dates, emails, photos and video. Based on their responses to other horrible reviews I expect management to try and discredit this review as well. In dealing with issues like this, I try not to make it personal, but both Kristy Lavey, the Executive Director (ED) and her completely unqualified "Wellness" Director, Elysia Angala (WD) on many occasions were so arrogant about their uncaring and incompetence that they chose to make it personal, with myself and other family members. Repeatedly! Whenever I politely brought up anything from the fact that Mom was usually lying in her own excrement (frequently noted by multiple Renown medical staff), to the never-cleaned black moldy toilet and piles of dirty laundry on the floor, I was met with DENIALS, LIES AND THREATS of retaliation: "perhaps your mother is no longer appropriate for this facility". I noticed these things in my visits, but they call into question what's going on when you're not there to see it. Skyline unilaterally decided that Mom needed to leave (after being taken OFF of hospice care). They DID NOT communicate this to me. Shortly after the director told me "this is your mother's home. We will do everything we can to keep her here!", I got a call from a nursing home salesman, who had been given a considerable amount of protected personal and medical information about my mother. CLEARLY unprofessional and very unethical. WD later indicated that a call from a third party was supposed to be notice that my mom had to move out. According to the Long-term Care Ombudsman who got involved with the case, Skyline was legally required to notify me in writing, with a rationale. This is one of several State violations she noted. What we got was an email from the WD on a Friday, notifying me that Mom had to be out on Monday, and that if she wasn't they would discharge her to a local hospital - a perfect example, among many, of Skyline's complete lack of caring. RUN AWAY! Current open investigations HCQC: Nevada State Bureau of Healthcare Quality Compliance Federal government for HIPPA violations Carson City Sherrif's Department for multiple theft issues including cash and personal items Ombudsman stated that Skyline had violated several state statutes and should be reported Stolen property (ED and WD took absolutely no responsibility) from cash to an entire new bed, which disappeared when a hospital bed was brought in. When we reported this to the Sheriffs deputy taking our statement she told us "the exact same thing happened to my grandfather when he was at Skyline!" Ball dropped multiple times on prescribed medications The worst quality food you can imagine. Think of a ten-cent hotdog as the main course Front desk person eliminated. Impossible for family or medical providers to reach a human Other family members not allowed to attend meetings with ED and WD Although there were a few really great line staff, they could in no way make up for the truly blatantly NASTY, uncaring management

    I went here again on Thursday, April 4, 2019 for the grand opening. It was a blast!…read more The were celebrities everywhere, from the Mayor of Carson City, to Miss Carson City with her crown and sash, to an awesome signer and violinist. There was an ice sculpture. There were fancy hors d'oeuvres and desserts. There was beer, wine, and water. The head honcho from Prestige Care, which has locations throughout the western US, came here all the way from Washington, I believe it was.. The were so many people I had trouble maneuvering to the front to see the speakers and even get water. The center went all the way to impress people. This event had been advertised for months in the Nevada Appeal. And it was packed. Good thing I came early, as I got the tour, which was quite extensive, out of the way, just in time for the ribbon cutting and speeches. I was exhausted at the end from so much walking. I guess I would compare this to a Yelp Elite event, except I have never been to one since I am (still) not a member of the Yelp Elite Squad. Boo hoo I guess. :-( The unit is a combination of skilled nursing facility (temporary rehabilitation with the goal to go home) with 80 beds (Medicare and Medicaid do cover that), and memory care assisted living with 54 beds (Medicare and Medicaid do not cover that). The memory care unit is a locked unit, with codes required to access the stairs, exits, or elevators. They take Medicare right now, but have not yet been approved for Nevada Medicaid. Approval is anticipated in a few months. The rooms are either semi-private, with an actual wall separating the two beds in a very large room, "jack-and-jill" rooms, with two rooms connected by a shared bathroom, or private. Some residents rent both jack-and-jill rooms, and use one as a sitting room. Most of the unit is not occupied, but all or almost all private rooms in the memory care assisted living part have been reserved. People come from as far away as Reno to live here, since the facility and staff are top notch. The units features wifi (which does not work yet according to the receptionist, so she would not give me the password--kind of snippy to me); cable TV; a hair salon; music therapy with bluetooth wireless headphones for each resident; a large monitor which is controlled by a tablet which can play videos, music, therapy games, etc.; a cornhole game; lounges, checkers; and a multitude of daily activities. There is a gym for supervised rehabilitation. There are also multiple kitchen and laundry units, but I am not sure if the patients use them or the staff does. All restrooms are gender-neutral. There is also a rooftop covered balcony area with a movable tile floor on wheels, so the individual tiles can expand in the summer without breaking. There are private dining areas should the family wish to dine with the resident. I was a little bothered by the fact there was no regular gym, chapel, library, or computer room. And the rooms did not feature an area for laptops like hotel rooms do. I know, many of the residents are older, but many are computer literate and will not be happy when this amenity is not available, especially in the future as Baby Boomers get older. The exits required a password to access in the memory care unit, which is a fire hazard. Another aspect which I was not thrilled about was the staff appeared mostly white. Not much diversity, but hey, this is northern Nevada. Parking was ample. There are actually two entrances: one on Washington Street by LabCorp, and one on Mountain Street. Last time I went, I did not know of the main entrance off Washington, so I went to the side entrance near Mountain, where no one was, lol. They make everyone exit onto Mountain though, which I don't know why, as there appears to be enough room to exit onto Washington Street. They staff, other than the receptionist at the end, appeared professional and knowledgeable. They have two doctors, which seems like too few for 134 beds.
