From the moment he was admitted, it was clear that cleanliness and basic standards of care were not a priority. The hallways and patient rooms were consistently dirty, creating an environment that felt unsafe and unfit for recovery. Nurses and CNAs were rarely available, and when we raised legitimate concerns about my grandfather's well-being, we were met with dismissive attitudes and hostility rather than compassion. Our family visited daily--not out of comfort, but out of necessity--to make sure my grandfather was alive and receiving even the most basic attention. No family should have to live with that kind of fear when entrusting a facility with their loved one's care. And if you scroll through the reviews, you'll notice a flood of suspicious 5-star ratings clearly coming from their own employees. That says it all. Instead of fixing the systemic issues in patient care, this facility seems more interested in propping up its reputation online than actually improving the treatment of vulnerable residents. Salem Health & Rehab should be held accountable for the neglect, lack of professionalism, and complete disregard for both patients and their families. I only went 6 times, after that, I could not go anymore due to personal reasons and because I could have done these exercises by myself instead of going there. After a couple of months, they sent me a bill for $120 dollars blaming the co-pay of the insurance. I called the billing office and they told me that they could not do anything about it, even if they specifically told me that the insurance would not have kicked in until the 7th treatment (which I did not do). I had a painful ulcer under my tongue that made it extremely painful to speak, drink, chew, and swallow and was told by a provider at an urgent care that it should resolve in a week or so. That was 10/28/2018. It only got worse and when I called Roanoke ENT about a week and a half later to get an appointment, they got me right in. That was 11/9/18. Dr Lenkowski saw me that day and was concerned as it was not just an ulcer, but a 7mm hole in the underside of my tongue. He put me on antibiotics for a week and when I came back in for my recheck he had me come back in at the end of the day for a biopsy as it was not getting any better. That was 11/16/18. He received the results 11/21/18 and had me come in that day to deliver them personally. It was cancer and he said he would be more than happy to perform the surgery but he would prefer I had it done by an Oncology ENT-head and neck specialist. He had already spoken to that office to get me in for a consult. I saw that Doctor 11/28/18 and he scheduled me for surgery 12/10/18. They had to remove a larger than golf ball size area of my tongue and they had to do a neck dissection to remove all of the lymph nodes on the left as the tumor had increased in size in just the 10 days from my consult. I am now cancer free and under surveillance for the rest of my life, but I can still speak and drink and chew and swallow my food. I am fairly certain, had it not been for my visit with Dr Lenkowski, that my story might not have the ending I have today. Thank you Dr Lenkowski

My dissatisfaction stems from the fact that during my check-up I was asked if I had any concerns, at which point I mentioned some mild issues I had been having and the doctor simply confirmed that the steps I had already been taking or was planning to take were appropriate. I was then surprised to find that this interaction led to me being charged an extra $153 because speaking about an issue reclassified my visit from a check-up (covered by my insurance) to a level 3 office visit (not covered by my insurance). In my opinion, a doctor should not ask a question which if answered will incur an extra cost without warning the patient. When I brought this up to the staff, I was told most people have better insurance than I do and that is why they do not warn patients that answering the question could incur unexpected costs, which I did not feel was a satisfactory answer.
