This astounded me since the same guy that lead the job and came back to fix it the first time, came back for the second time, to only tell me this. I hired another company to fix it again. When they were fixing it, they showed me how throughout the edges of the patio, short cuts were taken that allowed the patio's base to wash away. They fixed it and for the last two years the patio has held steady.
