DO NOT... I repeat DO NOT EVER send your loved one to this HORRENDOUS establishment. This place should be shut down. From the moment I walked in I knew my loved one would NEVER get the care needed. Unfortunately, we didn't have any choice at the time when he was placed there. I frantically searched for another place for him to stay. He walked in as a patient. 5 weeks later we took him home. 4 HOURS LATER he passed away. I will NEVER FORGIVE Bridgeview. I BEG whoever has a loved one there to get them out!!! read more

Very unorganized, under staffed, not at all concerned about the residents upkeep in apprences, or being keep clean!! smells like Urine ,and Fecal matter!! Employees of Housekeeping are walking around in Hoodies, wearing pants around there rear ends! They Hang around medicine carts! Its just utter chaos whole meds are being passed around like a candy machine, to others that aren't allowed nor qualified to handle such medications!! Its a free for all!!! Very , very unhealthy environment , unclean, staff are not helpful nor are they polite, they are rude, unhelpful for information about residents!! State needs to step in! read more

My sister was transferred to Bridgeview. She was there for 10 days. She was alert and talkative. The following day- she was "sleeping" and mildly responsive. We were told she was up all night. The next day - same thing- only this time - harder to rouse. Third day - essentially vegetative. I am extremely alarmed and acting so because I know she isn't sleeping for 72 hours. Additionally her body is very cold. I'm trying to get answers from staff and from supposed doc on site. I'm asking - how long will you let her stay like this? A week? A month before you do something!? The head of the unit tells me I am "worked up" and she's cold because of the air conditioner. I am making it harder for the staff, and not trusting their medical expertise. The staff medical person, not a doc, or a PA, finally orders bloodwork. They send her to the ER for low potassium. Her body temperature was below 90 degrees. She was dead 3 days later. This is my advice to Bridgeview whose director mysteriously cannot be reached: DO NOT DISMISS FAMILY REACTION. WE KNOW OUR LOVED ONES BETTER THAN YOU DO. LISTEN. DON'T SHUT US DOWN. The ER said my sister suffered an "event" that stopped oxygen from going to her brain. It could have happened internally or externally. In my mind, something externally happening at Bridgeview is very possible. The images on their website are fictional. The food inedible. Just go there to see for yourself. The government pays $10-12,000 a month per person as long as they're breathing. That is what Bridgeview cares about and nothing else. Do not bring your loved one to this facility. They may not make it out alive. read more

We drove by the location, and thought it looked very nice, this was our big mistake, that we did not tour the location, the next day patient was to be admitted for rehab. Patient was finally transported around 6pm from hospital, by a very unprofessional person. This location is deplorable. Once we entered the side entrance the strong smell just hits you, and when you walk down the hallway it gets more and more obvious that this is no place for a patient who has just had open heart surgery and also an open wound from the surgery on his leg. Needless to say, refused admission and requested to be transported to a new facility. With no answers, no communication and no help, family requested a wheelchair chair, 4 hours later this was the first time we actually saw a nurse who cleaned the filth from a wheelchair and gave it to me to use. So without any help, or any other communication we physically loaded the patient into the car and took the patient back to the Hospital ER that the patient was released from. read more

This place needs to be closed down, they have no business taking care of anything that's breathing. These people are negligent in their jobs and ignorant to family members of the residents when questioned about their loved one's. I don't know why the state hasn't shut this rat hole down, I'll definitely be calling to find out though. Then they have the audacity to send a bill for 4 thousand dollars when my father was there for 11 days before my sister and I had to call 911 and have my father rushed to Halifax ER after we found him alone in a hallway by an exit door unresponsive with food literally crusted to his face, lips, and tongue. The nurses ( I use the term sarcastically ) had to be tracked down then none of them could call 911 because " I ain't got my phone on me, but you can call if you want". They had no answers for how long my dad had been that way or how he hurt his arm then tried to blame the place he was transferred from, my father was transferred there on December 28, the bandage on his elbow had 1/6 on it, me and my sister were there on 1/9 for a care plan meeting. Which no one had checked or cared for his wound for 3 days. When the staff was asked (by my sister) when was the last time someone checked on him she was told " I think it was around 10 am. Mind you it's now 1 pm My sister asked is it practice that the resident are not checked on for a few hours ? The " Nurse" replied quite sarcastically May I add " well, we do have 60 other patients and can't check on all of them all the time ". I believe in my heart that the negligence in care is directly responsible for my dad's current situation. I wouldn't stick my worst enemy in that hole. Nothing deserves to be treated the way the patients in the memory ward are treated. But it amazes me that all of them cared enough about themselves to make sure their nails and fake eyelashes were perfect. Maybe they should put that much care in taking care of people's loved one's. It's not like the staff was forced to work there, they chose to work with the elderly and chose to work in a nursing home. read more

Don't put your family member in here. There are dedicated staff but they are overworked. My family member went in for a short stay rehab to gain strength to walk again after having a minor stroke. She went in with a bed sore acquired at the hospital. Bridgeview did inform us that she had a bedsore and they were actively treating it. Meals were below par; serving size was inadequate. Covid struck and communication to the center was sporadic. Many times the phone went unanswered. They were technical and staffing issues. My family member caught Covid and her stay had to be extended. The center repeatedly tried to coax us into extending the short stay into a long term rehab. We definitely weren't going to do that. When she got home, we scheduled for nursing care and discovered that the bed sore was at a Stage 4. What should've been an uneventful short stay turned into a major setback. My family member has been bedridden since February, 2023 after the initial hospitalization in January, 2023. The hospital is also responsible for not rotating my family member regularly. The hospital is Advent Health -Palm Coast. I'm definitely disgusted. I found out about Bridgeview Center through a non affiliated reviewer. From many reviews of other centers, this reviewer gave Bridgeview Center high reviews. SMH. read more

If you want your loved one to live out their last days in comfort and joy do NOT place them here! The staff is incredibly neglectful to those in memory care. All the patients always look like they haven't been bathed in weeks. They smell like soiled diapers. The staff acts very inconvenienced if you come and offer help with your loved ones. The buildings are always dirty and smelly. The patients rooms look like they are falling apart. Closet doors are barely attached to the walls. If ask to put in a work order to have things fixed the staff acts like it will take an act of congress to do minor repairs. They don't let the patients have toilet paper or paper towels because they don't want to have extra to clean up. So what are these people supposed to do when they use the bathroom? If you bring your loved one any personal belongings even with their name on it and it logged at the front desk it will be stolen. They allow other patients to sleep in other people's beds and then wonder why the facility ends up running rampant with illnesses and rashes. I'm completely disappointed and disgusted in this facility. Before Ashton purchased them this was a decent place. I feel like every patient here has been let down not only by staff but by the families that allow this kind of neglectful behavior. read more

Our Hospice Nurse sent my Lewy Body Dementia/Parkinson's Husband to Bridgeview Center for a 5 Day Respite for me (Caregiver) as I was totally stressed out from the constant cleanup of feces/urine. He recently took a fall and had hip surgery and was non-ambulatory (bed-ridden). I called to check on him the first night and the Nurse asked a lot of good health questions, which made me believe he was in good hands. The next day, it took several calls (phone rang & rang) to speak with a Nurse to ask how he was doing. She told me that she just passed him walking the halls and he said "I'm getting out of here" and she redirected him back to his room. I said "WHAT"....He can not safely walk on his own. Is he using a walker? She said NO. I said he just had hip surgery 2 months ago and cannot safely walk on his own....AND IS A SEVERE FALL RISK. How did he get over the bed rails? She said there are no bed rails. I said ...Did the bed alarm go off? She said there are no bed alarms. I said....can you see that he gets a walker, so that he doesn't fall again? She said there was a walker in the room. I called our Hospice and had them bring him home ASAP. So much for my 5 day Respite! The next day he told me they had left him in wet diaper for 4 hours....came back later & took off wet diaper & left him naked (he had to pee in coffee mug, since no diaper)...then 4 hours later brought a diaper and gave to him for him to diaper himself. They only gave 1/2 dixie cup of water and when he requested more water...THEY REFUSED WATER. I asked him if he used the Call Bell to get them to bring the diaper and he said THERE IS NO CALL BELL. THIS FACILITY SHOULD BE SHUT DOWN....THIS IS GRAPHIC NEGLECT. My Recommendation....Never accept a Respite from your Hospice (since they send you to the WORST NURSING HOME AVAILABLE). ALL THE NEGATIVE REVIEWS ARE TRUE....PROTECT & KEEP YOUR LOVED ONE SAFE BY NEVER ALLOWING THEM TO BE NEGLECTED AT BRIDGEVIEW CENTER in Ormond Beach, FL. read more

If this facility was an animal shelter, it would have been closed down by now. My dad would have been better off at the humane society. He gets a shower 2-3 times a month. Last week for breakfast he got two pieces of bread and some syrup. He currently has bedsores which do not heal because he is either left in his wheelchair, or in bed, all day. He was told not to drink water so he doesn't need to be changed. Although they never respond to the call light (minimum 3-4 hour wait), he was told if he used it then "they would leave him to die on the bathroom floor". There are about 3 excellent CNAs, RNs, and administrators here. These rest are cruel, lazy and racist. Of any of the poor decisions or bad choices I have made in my life, putting him in this facility was the worst. I will regret it and feel guilty about it long after I get him out of this depressing hellhole. If you love your parents - or even if you just don't hate them - don't let anyone fool you in to thinking this place in even vaguely adequate. read more

I rarely write reviews, but felt that justified praise was certainly warranted for all the personnel of Bridgeview Center. Unfortunately, due to my father's health, I had to locate a nursing home in Florida for him and would venture to say that this is one of the best nursing home facilities in this region. The administration, nurses, aides and other members of your staff were all professional and courteous. Everyone made an extra effort regarding his care and ensured he had a good quality of life during his residency. From his meals, to his personal needs, to coordinating his medical appointments, to their kind and compassionate manner, my father was treated with dignity and always greeted with a smile. I was kept abreast of any and all medical updates or changes and I called and spoke with him often and even saw him on Skype, which he made him very happy since I live so far away. The highlight of his days was his enjoyment of the beautiful outdoor area provided. Since his condition improved, he was well enough to relocate to an assisted living at this point in time. In the event that his health becomes such that he will require a higher level of care, I know that I would be able to rely on Bridgeview Center to provide that special care for him. read more
