I left this review on Google as well. Why does this site only have bad reviews? If you look on Google it is more varied. I have NEVER seen a poor treatment of a patient. In fact the complete opposite. Call lights are answered within minutes, the patients I see laugh and joke with the staff even though they are busy, they spend the brief time they have making sure needs are met as best as possible. I do what I can to help communicate and I follow up. So far all of the requests and doctors orders have been met or exceeded! My mother is in this facility. It is clean (I always see someone cleaning!) and though an older building it has all that my mother needs. She is a long term patient currently living here and has since Sept 2025. She's had many appointments and starting with my rockstar Brooke (Admin Assistant) that makes rides possible and any other communication details, to Jen the Queen of activities and things interactive, to ALL of the CNA's, nurses, social workers, doctors and therapists (who am I missing??) that look after her. She has really thrived here. She loves to sleep and would all day, but they get her up and interacting to keep her mind working. She has come out of her shell more. They change her often because she is bed bound. She has NO bed sores. They arranged for a more functional wheelchair and do everything they can to make her comfortable and get her up. If she stops eating or doesn't eat very much, the staff lets me know right away. I will say, if you don't advocate for your loved one, or yourself you may not have as good of an experience. I show up several times a week even if it's brief to check on her and communicate her needs. Everyone is so busy doing their best and it's hard work. If you show appreciation, BE KIND (even if you're hurting). Be curious, not rude, about the care you (your loved one) are getting, ask questions and communicate your needs. You will be thoughtfully cared for. Have patience you're not the only person in the building! Anyway, I am so thankful to God for finding my mom this facility. It's so much better than where she was! My dad was there 12 days -- In those 12 days: He fell twice. 5 minutes after we left first day (they contacted us after) - then fell on 9/16 - and did not notify us and told us it wasn't our business because we didn't have POA - which we do and they have known since day one we had - they just didn't want to be yelled at. When I asked for the incident reports for those - the nurse said she hadn't finished them still - but supposedly would email them to me. ( An incident report for a patient or resident fall must be completed immediately after ensuring the person's safety and finishing the immediate clinical assessment, typically before the end of the current work shift - I still have no received them) During care conference they stated that my dad said not to call me - he said that was not true and his roommate corroborated that my dad had asked the nurses to notify me. His blood sugar was out of control and now it's stable after I forced them to do their actual job. And proved they had been falsifying documentation around taking his CBG. They continued to give him regular diet which caused his sugars to be between 250-380. His pain meds were withheld from him. - they would turn off his call light and tell him to have patience and walk out. I brought up my concerns to the director who was condescending and stated "my family ran nursing homes in Idaho I've done this work a lot" - yet when you look at his linkedin he was a caregiver less than 8 months. Upon talking to him - he said "if you want me to do something for you concerning your dad you need to do something for me" then said he wanted privacy and proceeded to try and close the door to my dads room while I was in there alone - then seemed annoyed when my dad stopped him because he had came in from smoking right at the second Josh attempted to close me in the room alone with him. He had no occupational therapy and has only had group physical therapy instead of one on one - and has not focused on any items he needs - like walking, transferring, using restroom alone ect. Yet his discharge paperwork says he was given occupational therapy, physical therapy, and speech therapy. Which he only had PT -and that was far and few between. Upon signing discharge papers and getting the medication - I looked in the bag and they didn't include his insulin - thankfully we hadn't left yet. I had to go remind the nurse to give me his insulin- as she "forgot" he was a diabetic. I noticed his left leg and ankle was swollen - notified PA. When I lifted his foot I saw this huge sore about the size of a quarter- which was not there before. The bottom of foot is red and hot. I asked what he did to his foot and had no idea what happened. He wears socks at all times and I've washed his clothes - none of which had holes in them if he had dragged his foot to open it. He had a callus there prior - and a few days ago the feet people (should have been a nurse since he needs diabetic foot care - but with this place god knows who touched his feet) had messed with his feet. This caused a major infection in which he had to be placed on high dose antibiotics at new facility. -- You will constantly hear people yelling for help - Cnas and nurses ignore call lights - they will go in the rooms and turn them off and walk right back out. They left my dad in briefs because they didn't want to assist him in using the toilet and then would leave him in soiled briefs over 2.5 hours so he ended up with a fungal infection in his private areas. The first day we were there both the nurse and cna said he was NOT ALLOWED to use the restroom. They refused to help him eat when he was a 1 on 1 feeder when he got there due to paralysis of right arm and weakness in left. They don't check who comes in or out - and people can come in at all hours of the day and night without anyone asking who they are or asking them to sign in placed vulnerable people in possible harms way. This place is a disgrace and absolutely atrocious- if you care about your loved one - do NOT send them here! read more

I was sent to Bridge crest for rehab after a fall. I was in the hospital 12 days. This place is old and not maintained & is dirty. I was put in a room offered NO Water and it took hours to see a nurse. Who had me sign some papers and give me Tylenol. I couldn't get my pain med my doctor prescribed. I kept being told they are waiting for the pharmacy to send them. After 8 hrs I had my family pick me up. I couldn't tolerate the pain and couldn't get any help going to the bathroom and the hospital bed had no railings. My injury was from falling out of bed and getting a bleed requiring a blood transfusion. The bathroom wasn't safe either. No railings around the toilet and my wheelchair didn't fit either. Has I was signing out the nurse say's to bad your leaving because we now have your pain pills. She refused to give me a dose to help relieve the severe pain. I was taking pain meds in the hospital every 4 hrs. After the lack of care, pain management, disgusting food and finding dried poop on the floor. Was all I need to go home. At home I can get help, I now have a bed railing and have proper railings in the bathroom. I feel so bad for the people in this place that have no choice. I had concerns about this place but a case worker at PeaceHealth refused to contacted more places. read more

If I could give it a zero, I would. My client went there late last year. She is about 5 foot seven and when she went in, she was about 135 pounds. She was just just discharged against medical advice, came home weighing just 94 pounds - 40 pound weight loss in 7 months for someone that was a healthy weight. She looks like a holocaust survivor. She is a quadriplegic and cannot feed herself. She was basically left to die there. The food is terrible, as they have a vegan chef. They are understaffed, so patients wait for hours. There is a resident that screams, "Help!" Pretty much around the clock. Her first doctors appointment after leaving the facility, led to her immediate admission to a hospital for malnutrition. Her hair is coming out in clumps. I would only ever go here if I were suicidal. read more

Worst place EVER! Watching love ones die here and they had zero cares. Would not apply medication on time and had to fight with them for comfort of our dying dad. Hospice had to raise hell to make them do their jobs! read more

Food is not proper healing, nutritious food. Items are repeated every week. Spaghetti, fried fish, ham sandwich, taco nacho (kind of). We get no fresh fruit except maybe an orange if you ask for it. CNA'S and RN's are consistently overworked (sometimes it's 18-20 beds per cna, and the nurses are stretched as well.) agency staff are used as fill ins but For what we are paying a month (literally 95% of our social security check) we should be getting a higher quality of service than we do. People get very sick but are not given covid, rsv, pneumonia, tests. One resident was septic from an upper respiratory that was not treated for at least 7 days. She hadn't eaten or drank in 3 days. she was not offered a nebulizer treatment. Her blood pressure had fallen, and she was Not oriented to time or place. she was finally transported, and was put on comfort care and then she died. She died because she did not get proper care in a medical facility. Bridgecrest is mismanaged, drugs are mishandled, and frankly a death trap. I cannot wait to break out of here before I die too. I gave them a 1 only because there is not a lower number. 1 is exceedingly generous. read more

This was formerly Discovery Nursing. Things haven't changed much. It's a nightmare here. Neglect and abuse. Most shocking, they let a gentleman stay in bed while he ate and drank - RECLINED. He had a gurgled cough, staff ignored him. Another CNA happened to hear the coughing, yelled "he's aspirating!" and jumped over to help him. He had pneumonia. What did they do? Called the family and had a meeting about planning for his death - then walked out. What the?!! Thankfully this man had other people watching him who helped him recover. You are placed there to die, not to recover - that's how they roll. Don't answer the call lights for two hours. I hear a woman scream "help" for over five minutes before I went to see what happened - I do not work there. No one answered the call light so she tried to go to the bathroom on her own, but fell and damaged her already bad arm (she was placed there for rehab of this same arm from a prior fall). I went to get the nurse. This happened to many residents. They don't follow doctor's orders on meds, "I have four hours to drop meds," regardless of when the patient needs them. Pills dropped on the floor - no one picks up. Lose track of labs, don't care. Lots of agency nurses who are fill-in - don't care. They retaliate against you if you ask for better care. They tell you you can leave and/or document false info against you. It's very unprofessional. Discovery was purchased by a large investment fund group. Staff suspected the care would go down because they were already at 18-20 patients per nurse. Food is pre-packaged, not fresh. High sodium. Don't follow specific diets for residents. I could go on. But it is a death trap here. read more

worst place to send a love one for rehab! cnas don't do work are not interes in the care for your love one especially if they are bed bound from a accident! my grandmother only got therapy 3 times in the 2 weeks at this facilit! mind you she acquired a pressure ulcer in those 2 weeks due to not bei changed timel! and iwou go in 2 x a day to change her and feed her! they will over medicate your loved one even if you have medical power of attorney and want a medication that is being used as behavioral restraint! The nurses are rude and do not care many times I caught them giving the medication I told to be discontinued to my grandmotheronly! if only I could tell the entire nightmare here I would! I have worked in many nursing homes myself and this is far the worst! your love one is more than likely to decline drastically at this facilit! read more
