Went back to my insurance company and they confirmed that it was not only received but also approved. Insurance told me to have the provider call them with any questions. Called provider back and another person told me it was received, I had to get the documentation for approval, and refused to talk to my insurance company. By then I was done. I got my insurance company to refer me elsewhere. You don't want my business, fine.

I'm not big on medications and I steer clear of anyone who even mentions surgery. I had chronic pain in my back, hips, knees, and legs for a number of years. I really didn't think there was any help available to me. I don't remember the names of any of the techniques that relieved my pain but I remember that it worked like a miracle. Keith Moyer and Malachi are the Batman and Robin of the pain relief world as far as I can see. So, if you smoke, do not even see this arrogant person. I received a copy of my notes from the visits and the form I signed. In the doctor's notes stated he talked to me about treatments to quit smoking and I have to say that it is FAR from the truth. I feel sorry for the office staff. If your a smoker think twice for you might suffer the same humility. where does he get off treating a patient like this.
