Terrible facility. Not concerned for their patients. Rooms are unkempt. Short staffed. Would not recommend this facility. Expressing serious concern about the level of care my relative received under facilities supervision. It is extremely distressing that they developed bed sores--a preventable condition that typically results from insufficient attention to basic patient needs. I am surprised at the negative reviews. My husband was expecting to go to the Bella Terra in Lagrange the sister facility, but there were no beds available at that time. The liaison stated he could go to Elmhurst and transfer to Lagrange if we wanted when there was an open bed but most people chose to stay there. My husband has only one complaint the food has no salt, and that was after he'd been there for weeks. [[HIGHLIGHT]]The staff was attentive, therapy was great.[[ENDHIGHLIGHT]] He had an acute illness which left him bedridden. He was bathed, turned, repositioned and medicated routinely and as needed. His call light was answered within a reasonable time. And he received therapy at least 3x week with him stating he got good work outs on his core and legs. The head Nurse practitioner, told a patient that their Pseudogout was exactly the same as fibromyalgia. That was the excuse she gave for not correctly treating the very painful pseudogout attack when going to this rehab after the hospital stay. They had several different temp nurses daily, so no continuity of care. read more

I can't believe the negative reviews I just read before entering my review. My wife Teresa was here twice, once over Thanksgiving 2024 & again New Years Eve 2024/2025. Her Therapists Kasha (aka Katie) & Shannon were WONDERFUL!! [[HIGHLIGHT]]In fact, the entire therapy department staff was so friendly & welcoming![[ENDHIGHLIGHT]] [[HIGHLIGHT]]The nursing staff was very friendly & caring & the CNA's were awesome too.[[ENDHIGHLIGHT]] Only a couple incidents that were so minute compared to all the positive things, not worth removing a star for. [[HIGHLIGHT]]We've gone back for 2 social visits to the therapy dept. & took them some treats to brighten their day.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]There were some good friendships created during her stays here.[[ENDHIGHLIGHT]] Last but certainly not least Jonathan & Demus are are the men to go to if you have any issues or need information. She was supposed get antibiotics thru a pic line and when it came time to administer them,,,,,they didn't have them. They should have never accepted the patient unless they had everything. But they accepted the patient because it is all about money money money not quality of care anymore. My mother was transferred here from Lexington Square Elmhurst just as COVID lockdown began. We were very limited in being able to visit with her, & communication with staff/Mom was difficult via phone. Chronic understaffing led to her being mishandled (repeated leg trauma during transfer, progressing to cellulitis). Her condition further progressed to GANGRENE, of which we were not notified until too late. Hospice care was a joke, and she suffered needlessly. [[HIGHLIGHT]]Our visitation status was not upgraded until her last days, when we discovered the extent of her condition.[[ENDHIGHLIGHT]] This facility should be closed down! read more

My father was admitted for rehab. He was not assisted with even going to restroom or with a urinal. He did more at Elmhurst hospital. They let him lye there to get bed sores and weaker. He is a heart patient. He was begging them to call 911. He was sick. They refused all night and all morning. They lied and said a doctor seen him and they ran tests. They lied and said they called attending doctor at Elmhurst. The attending demanded after we contacted him they call 911. And still they gave my fathers hard time and were telling my mom why would they ? They finally called. [[HIGHLIGHT]]Of course DON in a meeting.[[ENDHIGHLIGHT]] I'm reporting to IDPH. Nursing homes have a bad reputation as it is. Why make it worse. Why risk someone"s life. Everyone has a right to medical care if needed. [[HIGHLIGHT]]Disgusting people. Immoral.[[ENDHIGHLIGHT]]then they say they lost his stuff. [[HIGHLIGHT]]Please check on your family members. If you have a family member that you don't want to ever see you again send them to Bella Terra in Elmhurst the food is horrible. I have people who should not be working with food or people. The nurses and CNA's were wonderful. Don't get me wrong but the doctors and others on a different page. They don't talk to each other about your care they won't talk to you about your care. I lost 30 pounds after being there for three weeks I went from 195 to 166. I demanded to be like let go and and told the doctor I want to go home I said I don't recommend this place to anyone unless you don't want to receive them again read more

After speaking to Jonathan and Tremaine and the team about my mothers care at this facility during a meeting, they addressed every concern brought to their attention. I greatly appreciated the level of care they attended to my concerns and my mothers and that she generally seems appreciated here at this facility. Some of my review was a misunderstanding and other parts are generally mixed up with staff way below them and never relayed to them. My father just spent time at Lexington Skilled Nursing/Rehab. The staff was great, PT excellent, OT, etc. His CNA made his stay work so well. Her name was Gina and she couldn't have been nicer or more helpful to my father & all the family. He went in there angry that he couldn't go home & she made his stay pleasant!! Lexington Elmhurst Health Care Center Rehab Facility Recently my mother was in the hospital for a pulmonary embolism, a serious life- threatening event. After being in the hospital for 10 days, she was discharged to Lexington Elmhurst Health Care Center Rehab Facility. Unsure on what to expect, our family was concerned with this transition. However this concern immediately disappeared when we arrived at the facility. When we arrived we requested a private room, and our request was honored no questions asked. We were greeted by Walter, the director of admissions and Jonathon whose sole job is to advocate for the needs of the patient. They both visited my mother everyday to check in with her. The nurses and nurse assistants were all hard working and kind. They often greeted my mother with a smile and hug. The nurse manager, Vallyn, was also was amazing. She addressed any concerns I had and followed up with the physicians for answers to any questions. The nurse practitioner, Jim, was extremely knowledgeable and patient with my mother. He toke the time to explain everything to her. When he learned I was a pharmacist, he was sure to include me in the medication decision-making and address any and all concerns I might of have about my mother's care. Above all the physical and occupational therapist were outstanding. They patiently worked with my mother to regain strength and confidence. They provided her the motivation to work hard. Of special note was Renata who was certified in lymphadema care. She took special care to apply the pressure wraps to my mother's legs even coming from another facility (once she was transferred) to finish her care. The swelling in my mother's legs were improving for the first time in many years. When we ready for discharge, Kaitlyn from social service arranged a meeting with the family and many of the healthcare providers to address any questions or concerns. Kaitlyn followed up with any questions we had and even investigated the price of some of the equipment that would not be covered by insurance. Kaitlyn also worked hard to get approval from the insurance to extend my mother's stay so that she can receive more necessary physical therapy. The excellent care extended well beyond the healthcare providers. The maintenance people always greeted my mother warmly, and were happy to assist with a blanket if my mother was cold. The food services employees were always happy to see her and had no problem providing an extra drink when requested. My mother also enjoyed the food. Overall I would highly recommend this facility. read more

My mother was elderly and had a stroke. We sent her there after she was released from the hospital because it had a good reputation put the care she received was extremely poor. She was not supposed to have anything by mouth (NPO) but some assistant mistakenly gave her a glass of water and she started choking!! When she was sick from pneumonia, they said that they were unfortunately out of the antibiotic that she needed (levaquin)!! That's ridiculous! They should get in a car and go over to Walgreens or something and get some!!! They hardly ever came in the room and she had to lie in a dirty diaper until we finally had to call someone. This happened many times!!! I was very disappointed and would never go there again! read more

Do not send your loved ones to this place . Walked into the dining room to visit him . Only to find him sitting face down at a table all by himself with a pillow in his face. With two employees sitting behind him just chatting away. If he was that bad off he should have been in bed. I got them to bring him to bed. After the nurse checked him they had to 911 him to the hospital with a low oxygen level. read more

My uncle was in the facility after a knee replacement and I cannot speak more highly about this building. The staff and aids were all so helpful every step of the way. The therapists were so friendly and he loved his specifically that he worked with daily. We have even gone back to visit since his stay in November. Highly recommend read more

This place is the worst. they seem nice but, they don't care about the clients and they only want the money. they let my mother unattended for hours at a time. food was not good at all. I would not bring a dog to this place. read more

My mother lives in the Lexington retirement facility which I would give 5 stars to. However she recently had surgery and is currently right across the street at the Lexington rehab facility. She is often left unattended. She needs assistance to get out of bed to the toilet and back again. She often has to wait over an hour to get to the toilet and then is often just left on the toilet for over 35 minutes. This is disgraceful. It does not even deserve one single star. We can look past the old 'beat up' (as my sister puts it) rooms and all the other things that make rehab hospitals really uncomfortable and unpleasant. But assistance to and from the toilet is where we draw the line. read more

Be warned if you choose Lexington of Elmhurst! My mom was there and was belittled- her care was off the charts.. HORRIBLE. My mom was deteriorating by the day - only to have the Nurse practitioner there tell me there was nothing they can do for her and. They were ready to release her after she had severe cellulitis and gained 22 pounds- my husband and I willingly took her back to the emergency room where in 24 hours is 100% better. Shame on you Lexington- you did not show any concerns for my mom even after I requested her to go back to Emergency Room! Their staff there made fun of her- laughed at her because she was high maintenance- heard it for myself! Absolutely disgusted and disappointed! Be very careful to choose this location- they clearly do not know what they are doing to have my mom suffer for weeks and almost died! Thank god we as a family took over and made the decision to take her back to E/R! read more

Never ... never again. They treated my aunt poorly - BED SORES!!! They are money hungry. The Administrator never returned our family's phone calls for a request to meet and discuss patient care. It was hard to find a nurse and or care giver to answer family questions about my aunts care. We finally had to remove her from the facility - they promised a wheel chair, ambulance to take her home and bed to be ordered for the home - they didn't do a thing. We had to wait hours for the ambulance to show up. They tried to get us to keep her there extra days - they said "for free" they would not make us pay the out of pocket, but we got out as quick as we could. read more

My mother was at this facility for a few weeks until she passed away. She was paid up for an additional nine days beyond the date of her death. The financial coordinator told me that they would send me a prorated refund. More than a month later - no refund. I called and left a message and received no reply. I called the next day and was told to expect a refund in one or two weeks. It sounded like they had taken no action until I asked for the refund. Mom received good care there, but the business practices are lacking. read more

first off i only gave it one star because i had to. i put my mom in here after her first stroke and she was in fear for her life which subsequently caused her second stroke from lack of sleep and exhaustion. everyone there is so rude, they don't care about people. they let my mom sit in her own filth for many hrs at a time and even in front of my own eyes. i would try to look for someone to help her but no one was ever found. i am horrified that because i made one simple call about my mom experiencing more stroke symptoms they ganged up on her instead of checked. she was ignored that whole night because of that. when she was also in the middle of having her second stroke she only experienced signs of slurred speech so they felt it wasn't priority to call 911. i as her daughter had to call 911 myself because they refused. i am now known as my moms hero but i don't like to be seen that way because i just did what was right and these nurses don't even know common sense. i would NEVER send anyone here. they would be better off in jail & i will forever be living with the guilt of putting my mom through such a horrible experience. read more

If I could rate this place a zero I would! My mom was in here 3 weeks when we decided that she couldn't be at this facility any longer and called an ambulance to bring her to the hospital. They are severly under-staffed, my family could never get a hold of anyone to get updated information on my mom when we called and when we did speak with a nurse or MA they were not very knowledgable on my mom's condition. I walked into my mom's room once to visit and she was covered in her own feces and screaming for someone to help her. Lexington looks nice and upscale and they put on a good show when you tour but this place is a nightmare! I wouldn't send my worst enemy here let alone a loved one. NEVER AGAIN. read more
