I am beyond grateful for the spirit of excellence that this nursing home has provided for my grandmother and now my mother. My 92 year old grandmother fractured her hip and was able to go home within less than two weeks. My mother is long-term COVID and unfortunately has Alzheimer. Worst food ever! Nurses are nice but you may not see them for a week. Doctors are oblivious of what kind of care is being given. I can't find out when I can leave. I could be sitting in a wet diaper for 14 hours, and no one would come to help me. I've been sitting in a chair for hours without anyone checking in on me. When my husband was diagnosed with Parkinson's Disease, it was a very difficult time for both of us. Aurora Advocate Lutheran General Hospital doctors suggested several skilled nursing facilities in the area. We chose Avantara because it was the closest to this hospital, and the hospital staff would be continuing to monitor his care. We knew the hospital's doctors could be trusted, giving us a great sense of relief. Avantara turned out to be the best choice as the staff displayed true teamwork. The Joint Commission evaluates Avantara, and it passes the inspections because it is a high quality facility. I noticed how clean Avantara was inside and without any smell. The grounds are kept immaculate, and flowers abounded. Receptionists were friendly and offered me a badge for security. I had to sign in and sign out, so I knew that my husband would be safe. My husband's room was clean and pleasant. Staff provided food and care that helped my husband recuperate slowly but surely. I felt a great deal of peace knowing that my husband was treated well. This is why I would highly recommend Avantara as a skilled nursing and rehabilitation facility to anyone who has a loved one in a similar situation. This was my first experience with skilled nursing facilities, and I am grateful that Avantara provided the basis for my husband's physical and mental improvement as well as his continuing to regain his health upon leaving. Avantara Park Ridge was the perfect place for my mother, who suffered from a rare, terminal brain disease. I received a returned call from Krystal, the Director of Admissions, within an hour of my inquiry. That same day, she and her Social Worker, Anntoinette, visited our home for an assessment. Things moved quickly and Mom was approved to move in that same week, which was a harder transition on me than it was for her! The facility is updated, clean, and the memory care units are in a pod-style layout, which is wonderful. Mom had her own room in Faith Place, where there are a few birds housed in the pod that offer a sense of tranquility. I visited several times a week during the 5 months she resided at Avantara. Everyone becomes family and it is a pleasure to be there, where other facilities it feels more dreadful. The other residents in Faith Place seem to be happy there as well and are so kind to visit with. Special shoutout to many individuals who were a huge part of our lives: Krystal for her professionalism, assisting in swift accommodations, and for her checking in on residents; Anntoinette for being my lifeline and helping me navigate so much, especially during those earlier weeks, and also for checking in on residents; Esther, the DON, who helped when I had questions and provided guidance with our hospice team, and also for taking time after my mother's passing to reach out and talk to me; Salami for always being so positive and kind, checking in on residents and being a shining light in the room; the RNs who genuinely care about the residents: Gifty, Joycelyn, Jenny, and others; the CNAs who selflessly care for the residents and go above and beyond: Margaret, Susmita, Teresa, Rena, Suze, Emmanuel, and others; even the Cleaning Crew like Elizabeth (and others) who would make a point to stop and chat with the residents while cleaning the floors & tables. A close family friend was admited in to this rehab center almost 2 weeks ago. I visited during lunch and I was appalled by how bad the food was even after I talked to the nutritionist and let her know my friends needs directly before the meal was even served and it was still wrong. The food was also cold and bland and inedible. The man running the kitchen was also rude and about as helpful as the nutritionist. It's too bad because the nurses and other staff have been wonderful so far. They should do better. How are people going to have energy they need for therapy when they can't even eat because the food service is so awful read more

DO NOT use this facility. The staff is the absolute worst. The room was filthy when my mom arrived and when we asked for Clorox wipes to clean things, we instantly received attitude from the nurse. We did not ask her to clean it. None of my mothers medicine was ready for her, they refused to give her a walker after just having surgery. They had NO safety belts available for people who did need walkers. They brought her food at 6pm that had been sitting out since 4pm and it wasn't even based on her dietary restrictions. Also, they left her personal medical history spread out at the nurses station for anyone to see. This is a HUGE violation of HIPAA. I called the 2 administrators of the facility and they didn't care much either. One (who is a part owner) actually never called me back. I watched another visitor of the facility bring a bug from their patients room outside. This place is disgusting! RUN FAR AWAY from here. We pulled my mom out of this horrid place after only being there for a few hours. read more

I was so scared for my father to do his rehab here after reading reviews. Luckily he had an amazing experience. We had a small hiccup at the beginning but Emilio extended amazing customer service and retention to smooth over the situation. Because my father was vaccinated he did not have to quarantine. I did notice that you might be able to do window visits if your loved one is not vaccinated. My father was so happy with his nurse maria and physical therapist Rich. He hates leaving the house. But he was so happy with the level of service he received and quality of food. Thank you again. read more

I have to commend Nursing Aide Connie; she was excellent in the care she provided, and her cheerful nature made a hard time much easier. I am grateful to have met her. I would give Avantara 3.5 stars, if I could. My dad was comfortable during his stay there which was over the holidays, but it seemed that there wasn't quite enough attention paid to him. Only got up mostly when he was going to PT or OT. Patients need their families there to help bridge the gap. The food, according to my father, was pretty good. Some meals better than others. I wasn't happy about having to being certain things to the attention of the staff, specifically a nurse who should have seen an issue my father was having. I understand all these places make money off how many clients and how often they can be seen by professionals but because manner goes a long way. The BEST part of Avantara was the attention my father and my family received from Veranique. From day one she was always so accommodating. When I needed something I would email her and she would respond back immediately. Even when I knew she was off, and wasn't expecting a quick response, she responded. The attention she paid to my family was beyond comforting. I felt like she was a family member looking out for my father. Veranique does her job, she does it very well, and has the right temperament to work in the industry. read more

Do Not Use -- I can't fully express my disgust for this facility. While we have met 1 or 2 conscientious employees during my mother's stay, for the most part, they abandon their patients. My mother had virtually no mobility when she arrived and wasn't much better when she left 2 weeks later. We tried to make sure one of us were there during the day but it didn't help because you could literally walk the halls at times and not find any workers. It was not unusual for her call button to be ignored for hours. It's not only degrading to leave someone soiled for that length of time, it led to severe skin irritation. At a follow-up with the Orthodepic doctor treating her, his advice when he learned she was at Avantara was " get her out of there, that place is the worst." read more

Our family had a very good experience moving both mom and dad into this facility- on the same day! Both parents have very different needs and the staff was communicative, caring and very helpful during their transition. Managing a nursing facility is a great challenge and the teams, even over the busy holiday season, remained professional and focused on ensuring their comfort, anticipating potential issues, and keeping cleanliness and food standards high. The physical therapy dad gets is great, they have a hair salon, we were able to create nice personal environments in their rooms, the administrators, Nurses, CNA's, and full staff are terrific. Patient care, kindness, patience, cleanliness, patient dignity is their focus and thus far, we are at peace with the decision to make heir home Avantara! read more

Do not send your loved ones here! My mom was there for a short time. An unsafe, mentally ill resident harassed and attacked other residents. The staff did nothing. There was a caregiver that was filming a resident with Alzheimer's. She was confused and upset. They restrained her while the employee was filming. DISGRACEFUL! They will move the residents to different rooms and then charge unbelievable amounts of money for the " upgrade". read more

very inadequate CNA/Nursing care, along with general care, lack professionalism. doctors were fine but the general care and compassion when caring for my husband was horrible. he actually fell out of bed after waiting for more than ten minutes after pushing the call button - he attempted to get out of bed and fell and had to be taken to ER. The food is typical institutional food... our dogs eat better. physical therapists and occupa therapists definitely lack compassion and gentle kindness. the overall care is definitely not a quality facility totally lacking in compassion kindness and tenderness. My mother was there five years ago and we had some issues done as well. Same thing my mother had to be taken to the ER. We thought after five years things would've changed. The appearance in the lobbies looks nice but don't let it fool you. Five years later and two deaths later, I sadly and heartbreakingly later... I learned my horrible lessons. read more

If I could rate Avantara with no stars I would. This place is the absolute worst. On the first day, my father was told by a CNA to sit in his soiled diaper until after he ate his lunch. I let the social worker and nursing supervisor convince me to keep him there which is my mistake. The nursing staff are unresponsive and incompetent. The social workers are morons and gave me misinformation. The discharge process was a disaster and rushed because they didn't even know he was being discharged. I saw a nurse on her cell phone while a resident got up and walked with her alarm sounding. The nurse didn't move at all. The place was dirty and the food was always cold. I recently stayed at Avantara Park Ridge for a significant amount of time, and although I was asked to write a review, I believe that I would have done it no matter what, given the care I received and my experiences as a patient there. In order to be fair and maintain some level of control over the length of this assessment, I am using a minimum of 90 days of work experience for each individual staff member that I am reviewing. I apologize for possibly missing some people. I'm going to do this review utilizing metrics to measure performance, behavior, and skill sets, which was my strong suit throughout my career. I have to split up my evaluation into 2 parts/reviews because of Yelp's maximum character policy. The following review below is the rest of this evaluation and I used another email account (Gio V. also). The intention is for the review to be read in its entirety. Patient Care I felt that the medical personnel was genuinely interested about your care and wellbeing, and your needs were always addressed. I had nurses who went above and beyond the call of duty. When you are a patient in a medical facility, you are vulnerable, therefore it makes all the difference when someone offers a helping hand when you need it or offers words of encouragement. In my instance, they did that and more, and I appreciate that. Permanent Staff CNA's These women make significant contributions and exhibit excellent work ethics. Irene A., Connie, Beata, Franny and Suzy are some of the hardest working ladies I have ever encountered. Even though their work is not always enjoyable, they put in a lot of effort, always smile, and go out of their way to ensure that the patient feels taken care of and that all of their requirements are being met. These women are incredibly skilled at what they do and do it very well. I can't discuss this group of women--who are all exceptional--without mentioning Irene specifically. When I first met her, I was still becoming acclimated to my surroundings and she was crucial in making me feel at ease. It was wonderful to have someone in my corner like her taking care of me because she treated me with such kindness. Irene has a particular place in my heart because of how she assisted me and how compassionate she was. She excels at working closely with patients, so I hope she keeps doing that, and I know she will be successful in everything she undertakes. Irene is and will always be a star. Nurses At Avantara, I worked with a large number of nurses, including agency nurses, but I'm only going to mention the ones that stood out and with whom I worked closely with. I'll start with Nora, a very skilled nurse who also exhibits a great deal of compassion for her patients, always makes time for everyone and she's extremely knowledgeable in her field. I'm grateful to have had the opportunity to work with her, and I have no doubt that she will continue to treat other patients with the same level of consideration and interpersonal skills she always displays. Monique has been working as my night nurse for a long time now, and she is a wonderful nurse and also a pleasure to work with. Last but not least I have to mention Dexter who, despite his personality, is one of the best nurses I have ever met. There aren't many people, in my opinion, who can equal his level of expertise and set of skills. He's definitely a stand-out. Agency/Temporary Staffing (Nurses and CNAs) Since Covid, I believe that the medical industry has undergone some significant changes, and there are things that probably are never going to be the same again. These changes, in addition to the absence of retention strategies, communication problems, and the company's established culture, in my opinion, contribute immensely to the turnover that Avantara is having. Measuring turnover and resolving problems when they arose was one of my key responsibilities throughout my career. I wasn't able to measure turnover numbers at Avantara since I don't have the data needed to do it but I'm going to deduce that their numbers are high and that turnover is a concern. It was incredibly challenging to constantly have a revolving door bringing in and out that many agency people. My issue with the continuing turnover was probably that I had trouble remembering people's faces and names, but mostly building relationships with them. Avantara must find a way to retain more of the agency employees, making them either regular temps or permanent employees. Even though I had a few minor issues, everything seemed to work smoothly even with the "one timers" and I was treated with the utmost respect and professionalism. There's no way that you can go through these numbers and not find some problematic or difficult people but Chris the Staff Coordinator does a very good job at retaining the good while removing the bad. read more

From the very beginning, we knew we picked the right place for our Mother who broke her hip. We originally had her at another facility, which was a nightmare, so we changed places. From the very beginning, the director of admissions Krystal, along with Kathy, picked my mother up from the other facility. We arrived at Avantara, and went straight to her room. We were met by Veranique, the director of guest services right away who went over everything with us. Physical therapists came in that day to evaluate my mom. Everyone put her at ease from the very beginning. I am not going to say her stay for two weeks went without a few hiccups, because we definitely had a few, but the majority of the staff, and the therapists were amazing. She got therapy twice a day, 7 days a week here. You won't find that anywhere else. Her nurses Maria, Jocelyn, Connie, Jorge, Precious, and therapists Matthew, Christina, Theresa, and the girls at the front desk, Mary and Jennifer, were the most wonderful part of this experience for her. They all truly made it better for her. They put our family at ease and we knew she was being taken care of. NOW, the food is another story. It was the worst. But, if you are coming here for the food, you will be very disappointed. We ended up bringing in some food for our mom. The facility was clean, and very nice. Again, everyone's experience is different. But this was ours and we were very happy and satisfied. read more

My dad was there for 3 weeks after a massive stroke. I though it would be good because its next to Lutheran General Hospital and if he got worse the hospital was across the street . We had to constantly request for him to be shaved, bathed , cleaned up or have his diaper changed . He could not eat on his own and they would put the food in a tray in front of him and come over to feed him after the food was cold. They were ok if you are constantly on the staff to get him cleaned up . however once there was nobody there he was very neglected. ( this was during the 2Nd year of Covid and visitors were limited to 1 a day) After 1 week he was afraid when one particular staff member walked in the room we do not know why because he could not talk. The management was nice to talk to you and they paint the place like its great . Its not !! don't go there if you need long term rehab because as soon as they get you in they are asking when are you planning to move your loved one out . I believe they will accept a patients that have more needs than they can provide to fill in a occupancy. I would not recommend this place . We moved my father out to another skilled nursing facility that was much better . read more

Do not go here. The "care" is horrible. The staff refuses to help, and have a nasty attitude. My dad calls here constantly crying because they won't help him. They make him wait over an hour when he urgently needs to have a BM. His "physical rehab" is a joke. They walk him around his room, and don't even bring him to the gym. The sessions last 4-10 minutes, yet they bill Medicare for a full session. The food is not edible, so we need to bring in food so he doesn't starve. The dishwasher has been broken for a week, so the residents are being served overcooked cold food on soggy paper plates. I'm an RN and I'm horrified that Lutheran General Hospital even recommends this hellhole. read more

My dad and I are thankful for the team at Avantara during my elderly mom's 3 weeks there to help her recover from her back fractures and various health challenges encountered unexpectedly. - Dominka welcomed my questions even before my mom transitioned from the hospital to Avantara. - Antoinette at the front desk has been consistently friendly and warm which makes a difference as people visit their loved ones and feel distressed. - George the PAN Coordinator and Supervisor/RN John Dexter were both very patient centric and quick to assess and respond to my mom's health situation when it dipped on two separate occasions. - RN(s) Salami, Maria, & June had a loving touch when trying to get my mom to take her medication and use the washroom. Overnight shift RN Tiffany was attentive as she identified, took action quickly, and involved the family as necessary when my mom's oxygen unexpectedly dipped in the middle of the night. - CNA(s) Lamesha and Susy have an extremely tender and dedicated touch in their care to help my mom eat and drink (which she struggles with) and spend time asking my mom questions in getting to know her. This touches my heart greatly. - PT's Rich, Melissa, and Dennis were able to get my mom to progress quicker in her strength and recovery than my dad and I could have imagined during her short time there. And a very special thank you to Rich who with patience and expertise, guided my mom in being able to do stairs (up & down) in the short time she was there. - Stephanie (Social Worker) and Kathy (Nurse Practitioner) responded warmly and attentively when we raised concerns with my mom's readiness for discharge and helped to ensure my dad would be equipped to help my mom acclimate back home. We have unfortunately returned to Avantara (due to fractures that have worsened due to my mom's condition) and hope that we'll come across these model professionals again where they can help build my mom's health back so she can return home again. read more

Do not go there. My Mother was there for about a year and a half and it was extremely stressful They had a reported sexual assault in 2021. You can look that up online with local news or the Park Ridge police department. It is PUBLIC information. The nursing director and Facility Director were difficult for us to deal with. In the face of Covid-19, and the conseisolation of residents, social service and IT (if Avantara has tech services) should have long since set in place procedures for residents to have face-time (or the equivalent) contact with their loved ones. I worked for a hospice, and we had such services in effect 20 years ago. I know that "window visits" are available for local family members. I received a response to my rating that virtual visits are available. I'm wondering where on Avantara's website this information is available, or how this service can be accessed. read more

A family member came here and could not understand the staff due to their thick accents. The nurses didn't do their jobs efficiently, and my grandmother had to care for him. the food was horrible, and the nurse didn't check in with us once. My mother was moved there for rehab on a Friday evening after a lovely week at Lutheran General Hospital and by Saturday at lunchtime she had declined 100% they did not call me, they did not call the doctor on call, nor did they call an ambulance to get her back to the hospital STAT. They did NOTHING, I feel so sad that I subjected my beautiful mother to 18 hours there. I said call an ambulance now I'm not waiting for " orders " from a doctor that has never met her. The only good thing I can say is there was a very nice charge nurse upon intake named Marsha. Although I walked into her room and she was eating chicken on toast and canned fruit without her dentures! And some sort of koolaide In a cup with a straw and no lid then she started to choke on the chicken seriously? I spent Saturday morning washing her clothes and blankets to bring to her all the while I was unaware of this decline when I arrived she was wearing some spare shirt and bottoms from some poor deceased persons donated clothing and unresponsive Keep your loved ones far away from this place! My father was transferred to Avantara after being a patient at Lutheran General Hospital. With high praises from the hospital we thought we were in good hands. My father was in that rehab facility less than a week, he went in with spinal stenosis and congestive heart failure. My father called crying begging us to get him out of there after being left in his wheelchair for over 12 hours overnight into the next morning. The male aide assigned to him for the evening hours left him there perhaps thinking he was a stroke patient that was unaware of his situation. After that incident my father developed such horrible PTSD he could not be left alone without a bell to call for someone if he was alone. My father was taken out of there quickly and we had a meeting with the supervisor who in return sent flowers after reviewing the video tape of the incident. Until my fathers death he could not be alone, I would sleep next to his bed every night because he suffered horrible panic attacks until the day he passed away. Beware of Avantara, it may look nice and bright but that's where it ends. They are for profit and not for senior patients or anyone who is disabled and cannot speak for themselves. read more

A cold, bleak indifferent environment this place is not. The nursing staff made this emotionally draining experience better. No request was ignored or question unanswered. The staff made time for everyone to speak their mind and express their concerns about how our dad was being treated. The outcome was as expected. Avantara's staffing was rated as above average in the rating from US News and World Report. We have found this to be untrue. Their staffing is subpar almost every evening shift. The poor patients can hardly be attended to, and the staff is running around like crazy. The staffing is at half of what it has always been to save money. Avantara rehabilitation center does not look like the sleek photos on the website. The reception has a lovely, hip look, but the rooms are very basic. Meals are not tasty. Dinner can just be beans and rice. The staff need some communication training. They aren't aware of the difference a friendly, respectful listener can make when a family has a member with a medical need. Also, it's not pleasant to see a stack of dinner trays left sitting in the dining room area. Promptness is not their forte. It's not a horrible place, but the staff sure needs more training on bedside manner. As one patient told me, "the service here sure makes me want to get well as fast as I can." read more

This place looks nothing like pictures posted. The rehabilitation is weak and very distracting as stuff talk loud not paying attention to patient. CNA's and nurses are often not qualified. They asked my husband who was continent if he wears diapers multiple times whenever he would ask for a bedpan, which was absolutely humiliating. Food is absolutely awful and often I had to buy it outside and bring it in for my family member to eat. Each time i needed anything to be done for him it would take them a very long time and if I wouldn't wash him I doubt anyone would. Please pick different place. read more

Avantara was recommended by a social work at Loyola hospital. I later found out from a Loyola nurse administrator that Avantara is not considered a preferred provider by Loyola. Avantara is extremely short staffed, and it is very hard to reach anyone by phone with questions or concerns. A much needed home evaluation was not performed upon patient discharge with no explanation. Lack of communication lead the follow up nurse from Loyola to understand that the patient's family would perform round the clock care which was not the case. Family members explained to the social worker that they are employed full time. Family members were quickly instructed on providing feeding tube feedings to the patient post discharge, but several important factors, such as wearing gloves and keeping the patient elevated to a least 30 degrees during and after a feeding were not address. There wasn't adequate help provided post discharge. If the social worker knew the right questions to ask the patient, she could have persuaded the patient to go into a nursing home facility. The only positive note, the place smells clean. read more

My grandmother has been living at Avantara for almost 2 years. She initially came for rehab a few times and then moved in for long term care after the decision was made she couldn't live independently. This is the best decision we all have ever made! My grandmother "Loves it here" and couldn't be happier with the care she recieves, the activities she engages in and the friends she's made. The nursing, social service and activity staff are genuinely caring, kind and understand my grandmother's needs. They are truly the best! I have recommended this facility numerous times to friends looking for rehab and long term care and will continue to do so. read more

My mother has been a long-term care resident here for about a year and a half. She is almost 95 and needs a lot of help. Mom loves living here. Lots of caring staff who never make her feel that she is bothering them. A lot of good activities which she participates in. The activities people are especially creative in the projects they develop for the residents. Something going on all the time which keeps Mom occupied, socialized, happy and content. read more

The ONLY reason I gave two stars is because the rehab units are beautiful and because I do have to recognize how amazing a FEW of the CNA's were., but They are overworked! Avantara is understaffed to meet the needs of the units. The food is awful... yes, the place looks nice, but that is it. Except for those few CNA's--everyone else --just going through the motions barely doing their job or NOT doing their jobs. Most of the RN's are crabby and useless... when my father needed to use the bathroom, the male RN literally said " I do not do that, let me get a CNA". WHAT? Are you kidding me.... scary! The scariest part of the whole place is the miserable admissions person Carrie... she is unwelcoming, unfriendly, moody, unprofessional, and has a total LACK of empathy for people with sick family members. Avantara Higher Ups should be aware that in most cases she is the first person perspective customers come in contact with, customers who are worried, stressed,and emotional -- and she is nasty. If her position is too much for her and she is overwhelmed and miserable... maybe it is not the job for her! Seriously, she needs to go! Owners need to take notice of what is happening at this place!! read more

Do not place your loved one in this nursing home period. It is a corporate run facility and the residents are treated poorly. My Mother was a resident and we had to be there daily. Call lights did not get answered on several occasions. Residents with special feeding needs often didn't get fed. Residents with bed sores. Staff not trained. The Regional Director is horrible. She addresses issues by quoting their US World & News rating. The reality is that the State of Illinois has low standards and thresholds for staffing to begin with so it isn't hard to get a high rating. The constant follow up on chronic issues was very stressful. They also started to run it like a prison ward sealing off public rooms from family and residents. A male resident was allowed to bully other residents, staff and even family. This is truly one of those places that looks good from the outside but once inside, it actually is very much like the movie One Flew Over The Cuckoos Nest. The nurses are LPNs not RNs. Only a few that seemed knowledgeable. Go to Lee Manor or Hillcrest or other privately held or Christian run homes. Get referrals and try to talk to family of existing residents when you visit homes. We were able to take my Mother out of there and I had to wait to write this review for fear of retaliation. Stay away from Avantara and Legacy who owns them. My mother has had two 3 weeklong rehab stints in Avantara over the last year after falls in her home. On both ocassions I felt that she was well taken care of by everyone involved there, from nursing to physical and occupational therapy to housekeeping. I felt everyone was attentive to her needs and spent plenty of time with me in answering all the questions I had which were plentiful. I would feel very comfortable recommending Avantara to anyone in need of a short rehab for a loved one. The facility is immaculately clean, the staff very professional and caring. read more
