The search was terrifying...every single place had at least one horrible review. A family member who works at a law firm with lawyers who deal with nursing homes highly recommend this place. I can see why! Within 12 hours of being there my dad had a meeting with an occupational therapist and a few hours after that he had a session with a physical therapist named Daniel. He had my dad up, walking 10 to 15 ft with a walker...something he hadn't done in almost 2 weeks. We were very impressed. They were focused on getting him better. As with any situation, I would recommend asking as many questions as possible, double checking with them, and keeping up with ur loved one's progress, but after the first night I was confident that my dad was in the right place for his recovery. Unfortunately, my dad was only there for 1.5 days before his kidneys got so bad that he had to return to the hospital....all due to problems that we knew could end his life soon. He passed away a few days later. All in all, ManorCare West was highly recommended to us, and we would recommend it to anyone else in our situation.

My care there was adequate but with confusion about diet and medication often from the nursing staff. My doctor verified multiple times that I was to have a diet increase as I had lost roughly 100lbs and was denied adequate food. I was denied physical therapy of any kind and told that my insurance did not approve it. I was left there to die.....but I did not. On March 6th I was notified that there would be an additional patient sharing my room. This patient was Derk Ball and was brought in with his sister. The staff installed a bed but I do not recall a phone nor call button and the room was not ready when Mr. Ball arrived. I overhead and was told that he was brought in from Palos Hospital having undergone a biopsy of his lower back and suffering from Pneumonia. There was a minimal amount of care done to ensure that he was made comfortable or had adequate oxygen. He was not receiving fluids or nutrition and after his sister left was basically ignored by all staff other than to check vitals. Several times I heard the nurses comment that he was removing his oxygen tube from his nose and had it in his mouth. Food was brought to him, it went untouched and his tray was removed. On Friday night I commented to the nurse that he had not had food all day and nothing to drink and offered her to give him my milk. To my knowledge he had not had any nourishment all day. On Saturday morning Mr. Ball was awake partially and had a male visitor and by Saturday afternoon and into the evening he seemed to be asleep. Food was brought but removed shortly thereafter and other than a bed check or to check for vitals, Mr. Ball was ignored throughout the rest of the day and evening. Late into that evening the attending nurse seemed rather unprofessional in making comments to the CNA about subject matter that I found to be offensive. This escalated to the point that when they were changing Mr. Ball she began to make comments about his genitals and very off-colored remarks directly at him. The CNA was laughing histerically which only encouraged her cruel behavior. She then commented that he was sucking on his oxygen tube and that it goes into his nose and not his mouth. Mr. Ball was not vocal and did not seem to be lucid. At this point I was able to get myself out of bed and into a wheelchair and after she left the room I went into the hallway and notified the nurse that he had not had anything to eat nor drink since he was there. Her response was vague and basically that if he doesn't eat they remove his tray. Sunday morning I was awake from most of the night and the nurse checking in that morning was told by me that again he had not had anything to eat. Our breakfast came and I told the nurse to give him my milk which she brought to his tray. Mr. Ball did not seem awake for the rest of the day. After lunch came and went I again told the nurse that he has not had anything to eat. Late into Sunday evening the attending CNA did a bed check and said hi to me and was very pleasant. She went over to check on Mr. Ball and left his side of the room after less than a minute. She said goodnight to me and left. Within about an hour the nurse coming onto her shift was doing her bed check and we exchanged a few words and she proceeded to Mr. Ball's side of the room. After only a few seconds she ran out of the room and I knew something was wrong. The scene of this became very frantic, very quickly with multiple alarms and multiple people entering the room. A cart with life saving equipment was wheeled into the room as staff attempted life-saving procedures. The CNA who had last checked on Mr. Ball prior to shift change was present but admitted several times that she did not know what to do. She said that she did not know how to perform chest compressions. I was in my bed and because of multiple people in the room I was unable to leave. I heard mention that paramedics were on the way and one of the nurses got me out of bed and ushered me down the all to the lunch room area. It was very cold so I stayed in the hallway. By this time some of the patients that were mobile came out of their rooms out of curiosity. The paramedics arrived and assisted in the life-saving measures of Mr. Ball. The head first responder kept asking who was in charge and what was happening. After life-saving measures for Mr. Ball failed this paramedic started to become loud and was yelling that nobody seemed to be in charge and that there were needles left on the window sill and he tried to put them in the sharps box and it was full. He demanded multiple times to know who was in charge and what was going on at this place. He blatantly stated that they were not removing the decedent and to call the medical examiner's office immediately. He left in utter disgust over what he had witnessed.

Little effort is made to communicate with residents who are hearing-impaired or who have dementia. Insufficient time is given to these ailing residents to process information and to respond. Avantara doesn't grasp that many residents, especially those with dementia, need help with ordinary tasks. My loved one could not remember how to use a knife to cut up his food, and he could not remember to press an inconspicuous call button for help. He was not bathed at all during his 5-day stay. If you care for your family DO NOT send them here find ANY other place that will not just take your money and mistreat your family. Do your research people! This place has changed names multiple times just search the address (I won't get into it here). My grandfather has dementia and the night staff tried asking him for cash up front after he was checked in, he fell out of his bed and his bandages were not changed as they should've been. He ended up being sent to a nearby hospital without notifying the family. This place is disappointing and should be shut down.

They were absolutely awful. They refused to clean his bedding because he could not get up out of bed. It was excruciatingly painful to put weight on his FRESHLY RODDED LEG. They left him in soiled bedding covered in pus and blood that oozed out of his surgical wound. At some point he told one of the employees that they could be kinder to him because he has cancer. She replied that she knows other people who have cancer and that those people "didn't lay down to die." HE JUST HAD MAJOR ORTHOPEDIC SURGERY, YOU PSYCHOPATH! Other things: The physical therapist had no idea how to work with his condition and help him regain mobility. They wheeled him to physical therapy in a wheelchair that was missing the footrest on his bad leg, so that was excruciating for him, then they wanted him to do an exercise involving pulling weights toward him that were mounted on some kind of a wall or machine (not 100% certain) but it was a rowing motion. It caused the chair to move forward and he had to use his bad, freshly operated on leg to brace himself. Also, there are no phones in the rooms so if your loved one does not have a cell phone, as mine didn't, it will be insanely difficult for them to get in touch with you. To make matters even worse, they would not help him to get up to use the bathroom and they recommended an adult diaper... and they did not like to change people. The entire place reeked of urine. There are better options. Don't send anyone here, especially if they have issues with mobility.

A couple of staff members stand out as exemplary, but then there are others that don't care at all. Have you EVER heard of CNA's delivering food. That should not be their job - they should have food service workers do that. And they talke about the aqua therapy and pool. I was one of the very few people to use it. I was told it gets used about 2 or 3 times a year. So, they should stop promoting that feature. May main complaint is the FOOD. It would have to work hard to get up to the level of terrible. A health care facility where you cannot get a piece of fruit, if your life depended on it. One time my stomach was queasy and a nurses aid looked all over the building for a couple of crackers. They should be at each station. If my husband did not bring me food every day i would have starved. One time i had a little of the fish???..shortly thereafter i became violently ill - and so did about 50 others. They told everyone it was the stomach flu. I think not - more like food poisoning. They have to get their kitchen and staffing levels fixed. Until they do, i would also say - stay away.

When my brother was discharged from the hospital, facilities were carefully screened because of the severity of his injuries and the level of care he required. Bria was fully aware of his condition and assured us they could meet his needs. That could not have been further from the truth. The two days he spent there were the worst of his life. Upon admission, they did not have the nutritional formula he was prescribed. They did not have his pain medication available, and instead of delaying admission or providing a safe alternative, they borrowed oxycodone from another patient while waiting to stock his medication. This alone is extremely concerning. My brother had a hole in his esophagus and a history of aspiration from vomiting. Despite this, he was overfed, which caused him to vomit repeatedly throughout the night. When he asked the CNA for anti-nausea medication, he was told the doctor was not in the building and they could not bother him overnight. When my brother pressed the call button for help, a CNA entered the room and told him to "shut the f* up" and stop pressing the button**. His wound care was completely inadequate. Staff only changed the top dressing and ignored deeper wound care instructions. Two days later, he was sent back to the hospital with an infection. The wound care physician there was visibly upset and stated the wounds were not cared for according to the medical orders. Medication administration was also unsafe. His pain medications were not given on schedule and were administered whenever staff felt like it, despite his severe wounds and documented pain needs. When I called for assistance, a CNA responded holding an energy drink, smelling of cigarettes, and asked if we could wait because she was on a smoke break. This experience occurred in November 2025, and even now, just thinking about what my brother went through causes me anxiety. No patient or family should ever endure this level of neglect. If this is how they treated my 38-year-old brother, I can only imagine how they would treat your elderly loved ones.

This isn't just a place where you go to get treatment it's a family of men and women whose hearts and passion shine through everything they do. Their goal isn't just to help you heal physically; it's to help you function better in everyday life and return to the things you love with strength and confidence. Every therapist here pushes you but in the right way. They never push you to the point where you dread coming back. Instead, they challenge you in ways that remind you of what you're capable of, and they encourage you every step of the way. They don't just treat the body, they uplift the spirit. From the moment you walk through the doors, you can feel the warmth. There's no attitude, no negativity just kindness, professionalism, and genuine care. The front desk welcomes you like family, and the therapists work together as a close-knit team (family) who clearly care for one another and for every patient who comes in. Today was my last day of therapy, and honestly, leaving felt bittersweet. I'm grateful for the progress I've made, but I'll miss the support, encouragement, and positive energy I felt every time I walked in. I've done plenty of reviews for restaurants and customer service before, but this review is different because this place has made a real impact on my health and my life. If you are in the Oak Lawn area and you need physical therapy whether it's recovering from an injury, surgery, or just needing extra help to get your body right come here. I promise you will not be disappointed. Unless you're someone who can't be pleased no matter what, you will feel the difference here. This is truly a home away from home for healing. My loved one had complaints that they didn't have enough ice packs, that the packs they had weren't great, and other things of that nature, but I do think they are a tad underfunded and I don't hold that against them because the staff really does the absolute best that they can with the resources that they have. It's not the most engaging or exciting environment for long-term care, but it is peaceful and everyone is doing the best they can with the resources that they have.
