This review is for the Brookdale jones maltsberger…read more Before deciding to place a loved one at this facility, please read this review and look up "lawsuits against Brookdale Senior Living." This is a Type B facility, meaning it should be able to provide 24/7 care for residents with limited mobility. My mom stayed here only a little over a month and experienced what we believe was neglect along with verbal and financial abuse. Shortly after moving in, she suffered an injury that left her barely able to walk due to severe pain. We quickly learned this facility could not provide the compassionate and attentive care they claimed they could. During this time, Brookdale leadership suggested hospice and convinced me, while I was emotionally distraught, to add medication dispensing and grooming services. I left that meeting emotionally drained with an additional $1,000 added to my mother's monthly bill believing she would receive the care she needed. I was wrong and looking back, the experience felt predatory. Most second shifts (2-10 p.m.) had only two caregivers for the facility and no supervisor present. Even during first shift, caregivers appeared overwhelmed and rushed. If my mother was asleep, meals were sometimes left out of her reach. I once found Desitin in her toothbrush, and one weekend, she was left in the same nightgown from Friday through Sunday. She often waited 1-2 hours for call requests to be answered, and one night her call went unanswered for six hours. If she had fallen or experienced a medical emergency, help may not have arrived in time. The final turning point came when a caregiver verbally intimidated my mother for using her call button. While my mother was in pain in bed, the caregiver leaned over her and told her to stop pressing the button because they had many other residents to care for and "everyone pays for extra services." After this, my mother became afraid to ask for help during second shift. Her hospice nurse later told me the staff could not care for her appropriately and recommended a personal care home instead. I moved my mother out as quickly as possible. She is now thriving in a personal care home where she is treated with dignity and respect, something that was severely lacking during our experience at Brookdale on Jones Maltsberger. I did not immediately report the verbal abuse because my mother feared retaliation while she still lived there. Instead, I focused on getting her somewhere safe. The fact that she feared retaliation at all says a great deal about the environment there. I have since submitted detailed reports to authorities and included contact information for family, friends, and clinicians who witnessed these issues. Because Brookdale could not adequately care for her, I obtained a support letter from the hospice physician requesting a 14-day notice instead of the full 30 days. The Executive Director rejected this request, insisting the facility could meet her needs despite the serious issues occurring during second shift. I also asked them to consider refunding at least part of the $2,000 community fee my mother paid given she was barely there a month and left under these circumstances, but this was also rejected. Additionally, despite revoking authorization for automatic withdrawals, Brookdale still attempted to withdraw a large disputed amount that would have placed my mom at a deficit if I hadn't already placed a stop payment on Brookdale. I was present at this facility almost daily and my mother was still verbally abused and neglected. I worry deeply about residents who are even more vulnerable because not everyone has someone to advocate for them the way I could for my mother. No family should ever have to wonder whether their loved one is being ignored, humiliated, or left to suffer behind a very costly closed door.

Unfortunately there just aren't enough aids to take proper care of the residents they are assigned way to much work to do so they don't have time. The management of this facility is pittifull. The charge nurse is totally unprofessional in her attire and her attitude. This facility is constantly changing management. When the facility was Emeritus the management was totally professional and caring. Recently the rent is going up by 50% with no added care. Just be careful when considering this facility for your loved one. Any assistance to the residents is an additional charge (ie) showing them to their room, that is considered Redirecting. Please be aware and ask many many questions about levels of care. Look around at MANY other facility's before you make a decision.

During the Covid-19 situation, they have followed a strict hygiene protocol and have locked down the facility to protect the residents. We are grateful for their prompt attention to this issue and we know that their actions are the reason that none of the residents have contracted the virus. Whenever we have visited previously, we are always cheerfully greeted by staff.

They're dedicated to dementia-related residents and the facility design is conducive to that: the two buildings with up to 16 and 20 private rooms and homey dining and living rooms. Both have huge, peaceful fenced backyards. CG is older than many facilities in town, but generally well cared for. However, the caregivers make the difference at CG. Despite the fairly frequent turnover of some staff and directors over the years, our favorites remain constant and genuinely caring (Executive Director Georgina Merrill and her team, to include Laura Maharich, are great). We can never thank them enough for their dedication to be mom's loving extended family. Remarkably, she's been at CG for almost 6 years with little illness except the continuing decline from the Disease, which is a testament to the great care she receives there. (As I write this review, we're in the midst of The Pandemic and they're selfless in protecting our loved ones.)

My mother has lived at The Forum since 2010; first in Independent Living, then Assisted Living and now Skilled Nursing. The facility, food and activities have been consistently excellent and the staff has always been professional, attentive and friendly. The staff's actions the evening of February 20, 2017 however deserve special recognition and are the most recent example of their dedication to serving residents. A tornado struck that area of San Antonio that Sunday evening at about 10:30 pm. The night staff responded quickly and efficiently, as the entire staff was recalled, to calmly and effectively evacuate resident, many to safe areas within the facility. This task was especially difficult given the large number of older and infirmed residents and the time of night, especially on a weekend. The facility received moderate damage, the weather threat passed, and no residents were harmed. Clean-up and repairs began the next morning. I visited The Forum on March 6, 2017. Repairs continue but operations have returned to normal. It surprised me to learn that many residents, my mother included, still did not realize the extent of the danger or damage. I conveyed my thanks to Kristi Allen, Director of Community Relations, but words can not adequately express the gratitude my family and I have for the staff. This was, quite simply, another example of the excellence we've come to expect from the staff at The Forum at Lincoln Heights.
