My mom fell and ended up in Sunharbor manor she was there for month or so. Michael the concierge was extremely helpful. He was always so pleasant in getting anything you need very quickly. Alisa social worker was very helpful and knowledgeable explaining the plan to help my mom get better. Tenisha is brilliant knew everything you need to know about insurance Medicare Medicaid. She was Extremely helpful we would be a loss without her guidance. My moms nurse aide Julet was so kind and loving. My mom adored her she always made sure my mom was comfortable and gave her hugs. Many of the patients of what I observed felt at home by the care received. It was very clean , organized lots of activities. If and when mom gets the point of being in nursing home hands down it will my first and only choice. read more

I was transferred from LIJ hospital to Sunharbor Manor for rehab. I was unable to stand without severe pain in my right leg. The rehab staff especially Diego, Rowena, and Jenna were amazing and I was able to go home after 48 days with a rollater and some other tools to help me with dressing, picking up things, etc.. He was always clean, his room was well maintained, meals arrived warm, and the staff followed a consistent regimen to meet his needs. The nurses, CNAs, wound care team, and social worker Kim Geisler were all responsive, compassionate, and professional. I can only speak about what I have personally witnessed and experienced. My father came here because it was recommended by another doctor. The food is terrible and close to not eatable. The rooms are pretty gross, and I'm trying to be nice. Im surprised they don't have bettercleaning procedures. My father's roommate had asked a woman walking by who was wearing scrubs to please push him into his room, the woman replied " no I'm busy, you have to wait". So they were going to leave him in the hallway until someone had 30 secs just to push him into his room? So I did it. He fell out of bed and though my father pressed the call button nobody came. My father had to go out and find someone. Then his roommate got sick and had an awful cough, my dad asked them if they tested for the flu, COVID, RSV they said yes and my dad asked if they swabbed his nose they said no they took his temperature. SMH they said they needed the supervisor to give orders. Seriously, you need a supervisor to do a nasal swab?! The man's wife had to threaten them to have him transported to the hospital immediately. Every assistant we have had has been pleasant and as helpful as what is allowed of them. I believe the issues at this place stem from the top. The nursing "supervisor" wanted to move my father to another room after he was already in bed at 9:30pm on a Sunday. And as we were talking to her she walked out of the room and completely ignored us. So unprofessional and this is supposed to be a supervisor?! You have to check the room out first and see who you are going to be paired with. Some tvs don't work and my father was there for 3 days without one. You have to stay on top of the PT or they won't do it. Also they didn't have my father's medicine he takes at night and said they were waiting on the pharmacy?! Makes no sense and is basically BS. Thank goodness my dad is only here temporarily. I wouldn't leave him here for long term care. read more

That when a patient is sick they don't wait 3 weeks so that then the patient gets three infections then a seizure from the infections then dies!! I would put no stars!! DO NOT BRING YOUR ELDERLY PARENTS to SUN HARBOR. They break their spirits and leave them in Diapers all day. Even when they come home and can poop in the bathroom they do not!! Physical therapy ("PT") asks if you want it today and when the patient says" No" they do not do anything! Leave them in bed rather than PT- easier for them. My father came home after spending $15,000 and did little to no PT for the month of October 2025. Patients need encouragement to complete PT!! The food is not suitable for a dog! Discharge at Sun Harbor failed to coordinate Physical therapy and he has not received NONE!! Sun Harbor needs to approve the first 2 Physical therapy sessions to start! My father was punched by another resident 4th of July week 2025. No one told the proxy. The following week that SAME resident attacked my father at 6am in the morning pulling him out of bed, dragging him on the floor. When I visited my father that morning at 630am (just missed the incident) my father told me. I was in SHOCK!!! We called the police, and a police report was filed. The resident was taken away and returned ~2 weeks later. Sun Harbor's solution was to put this dangerous resident on the third floor and my father on the first floor putting other people in DANGER! My mother hated this place so much that she left before she should have due to lack of care and being left in her own decors for hours on multiple occasions. I was at Sun Harbor Short Term Stay. Diego PT & Monica OT were the right fit for me. Very professional and caring. The Nurses were great and the Aids were always there for me and attentive. The decor is very old and needs updating, but very clean which is important. Of Rehabs I've been to the food is quite good. Administration was always there to answer questions and Alyssa was perfect. Cruz in recreation was awesome. A fellow by the name of Harry made me welcomed with coffee each morning and a smile. Lizzie & Karen are Fabulous aids as so many others to many to name. There's a gentleman by the name of Michael the concierge who did everything. He is the heart of Sun Harbor. There isn't anything that's nots his job. I don't think he ever sits down or stops. He is a blessing. If I'm ever in need of rehab again I'm coming back to Sun Harbor. read more

If you have someone you love do not send them here! Food is inedible , lowest quality allowed for human consumption! Lost 15 lbs in 21 days. Then where is the staff when you need them!? From my room. I heard an elderly person screaming for help and nobody came. It turns out she fell out of bed. I went in to see what was up after a few minutes. There was not one nurse or aide in the entire corridor, nor in the second corridor I went to. Finally halfway past the third corridor I was able to find an aide halfway up the third corridor, who went to find a nurse. Inexcusable!! read more

My Grandfather was ignored more times than I care to think about when calling for help..including One afternoon where he was left alone in the shower ringing for help for over 30 minutes to no avail. I Actually had to drive 40 minutes to San Harbor to help my grandfather out myself one night he was in tears about Having a urination accident and being left in his own urine for 90 minutes. It's Close to impossible to reach anybody in nursing and if you do reach someone in nursing be prepared to get blown off or not taken seriously. The administration and case managers are no better I called every day for 5 days to several different departments to try to get some help from admin to absolutely no avail not one call back. My grandfather wound up having to go to the emergency room from there because he caught c-diff ( google it) And became septic from a wound that he was not getting proper care for. My mother was admitted here after being in Northwell Hospital, Mineola. She had fallen on her head and received stitches. Sunharbor was recommended, first night there she fell out of bed on her head and was taken back to the hospital. I called many times and either I could not reach a nurse, an administrator, or a janitor. The phone rings, no one answers or it hangs up. The receptionist has a heavy accent that you can't understand him, plus he has a rotten attitude. Should be replaced or fired. I have been trying to get some information about my mothers procedures for weeks, leave messages, no communication. I know dog kennels that treat dogs better than this place treats humans. read more

When I wheeled my Mom back to her room yesterday afternoon after our visit, I once again found her call button out of reach, wrapped around the opposite side of her bed, and her telephone laying on the night table behind where she sits, also out of reach. My Mom is a "choke risk" and since her room sits at the far end of the hallway, if she needs any kind of assistance, she is on her own. read more

It's extremely unconscionable how understaffed this place is. I'm writing this review because I don't know how else to reach the owners or management and let them know that their residents, including my mother, are suffering as a result of an extreme lack of staff availability. There's a complete lack of coordination. Most of the staff seems to be doing their best in difficult circumstances, except social work, who doesn't seem to feel it's their job to ever pick up the phone or return a phone call until their manager's manager is called twice. And the nurses are too busy to answer the phone, whether I call the unit, the unit manager, the nursing manager, OR page them from the front desk. My mother had an x-ray and infection test weeks ago and no one ever told me what the results were. The staff is being asked to implement a complex visitation system without adequate time or support on top of their normal responsibilities, resulting in a complete mess. When I have finally been allowed to visit, my mom was unable to see me for the entirety of the 2.5 visiting hours because staff didn't get her transferred to a wheelchair despite being asked repeatedly. Their online visitation portal is now down and no one from social work answers the phone to get a visit scheduled. I have been trying to hire a private aide for two weeks and I hope we're finally about to make it happen, but my mother would've had that support WEEKS ago if social work had returned my calls from the beginning. I also spent weeks trying to get my mom a bipap, because even once I brought the machine itself from home, nurses and social work kept telling me (when I was able to get them on the phone at all) that they didn't have a prescription for it, and no one felt it was their job to coordinate and contact the doctor or follow up to see where the prescription had gone. I have a day job that is suffering from the amount of time I need to spend calling this facility and every various specialist to get anything done, down to reminding the nurses to please administer an as-needed prescription that they keep telling me they can't without a doctor's order (which they have). My mom has even been repeatedly left to sit in her own feces or without water to drink for an hour at a time. She gets drymouth from a medication; how hard would it be to stock her up with additional water when they come around? I shouldn't have to hire and pay for a private aide to come into a facility where she's supposedly receiving 24/7 care. Please hire additional staff immediately! read more

Why Northwell Health suggested this nursing home is beyond me!! This place is awful. First impression: Unpleasant daytime front desk! My best friend was admitted with leg paresis following a stroke. On the same day of his admission from the hospital, they had a COVID case on his floor, so I could not visit him for 2 weeks. They require you make a reservation online on a very limited visitation schedule and limited number of visitors at any given time! If you work 9-5, you can only visit on Thursday evening and Saturday, no Sunday visits permitted! Once 2 weeks had passed, I tried to see him. I was told I could, but 1) Friday and Saturday were already filled and 2) Monday was promptly canceled for some unknown reason. When I tried to find out, the front desk had no clue that the Covid issue had been resolved. My friend needed items from home which I brought to the front desk. When I asked to talk to the nurse on his floor, the floor manager answered instead. My friend had received a pacemaker at the hospital just before he went to SunHarbor. I wanted to know if the wound on his chest was healing properly and if they had changed the dressing. The answer from the floor manager was: "We change his dressing daily and he is angry and demented and doesn't remember anything... and promptly hung up." Well, she clearly had the wrong patient, since my friend is neither angry nor demented! They cannot even keep their patients records straight! Forget to call the social worker or a nurse. They never pick up or call you back. Actually, no department knows what the other is doing, a complete lack of coordination. The day of my friend's discharge, I was informed by phone that he was scheduled for doctors appointments 2 weeks later! This person had no idea, that he was being discharged from SunHarbor. read more

I don't understand how this faculty can get away with their lack of communication with family. No answered phones!! The FaceTime we were offered was once a week and 10 min max. I understand this was during covid however, it's unethical treatment of patients...especially those with dementia who are they for rehab purposes. This is the first night, and my review may change once this explained tomorrow, but I am sort of horrified by my friend's first experience with this. My housemate had a stroke and lost fine motor control of the right side of his body. Tonight he left the hospital and was transported to Sunharbor Manor for rehabilitation therapy before coming home. He was put into a room with stains on the wall and a gooey-disgusting spoon in the nightstand drawer. Then they served him a dinner with a moldy sandwich. He sent us pictures. The mold is obvious in the pictures. (The spoon can be seen, but it isn't clear from the pictures how gooey it is .. but the whole idea is disgusting and especially in the age of Covid.) I have tried calling the facility. Since I don't know anyone there, yet, I chose to speak with the operator. After several rings I get a recording that says "thank you for calling, goodbye" and hangs up. On a another annoying, but slightly less offensive note: earlier today, before he arrived at the facility, when I found out he was going there I looked up the visiting criteria on their website. It said that you could visit, up to two people at a time on the patio for 1/2 each, once per week, but only with reservations. The reservation link on their website wasn't working and so I called to make reservations to visit him this week. The person who answered seemed to think visitation wasn't allowed, but recommended I call in the morning. Either they should update their website OR train their staff on what is allowed. Update on 1/14/2021: I just spoke with someone at the main desk of Sunharbor Manor named Karen. She was really responsive and said she would talk to housekeeping and get someone there right away and speak with the kitchen. She didn't sound like she was just trying to brush me off, but, of course, I don't think that justifies it. My friend is going to let me know how it goes today. I am not going to call the health dept or anything until I hear back from him later today. A friend of mine said her mother was there for years until she died in 2014 and they were really happy with the nursing home. Maybe this was a one time mess-up? As for visiting: new patients are basically quarantined for two weeks. If he is still there after two weeks, then we can start visiting once per week. (Based on my interaction with Karen this morning, I am changing it to 2 stars from 1 star. I will come back to update after my friend gets back to me today.) Update 2 on 1/14/2021: Someone else just followed up with me from Sunharbor Manor. They seemed to be taking my complaint quite seriously and really responded apologetically to my friend who is there. I am more inclined to believe now, that last night might have been a fluke. I am upping my rating to 4 stars based on my interactions today (cannot give 5 stars after last night). I will update again if more changes. Update 3 on 1/19/21: My friend has had a number of issues over the weekend. No one ever cleaned the wall (or even noticed it when they apologized and came to clean up) and the messed up his medication, missed his room when giving covid vaccines, etc. I need to downgrade the rating again. read more

This nursing home is no good with a few exceptions. Pros: the small dialysis concession run by NYU has a solid staff. Cons: The nurses are generally kind but woefully understaffed. The food is fair at best. The furniture is shabby and chipped and the floors aren't very clean. At night, there is one nurse for 18 patients. My disabled father waited 45 minutes to be taken to the bathroom while he lay in bed. This isn't a real rehab center....it's a nursing home. As one of the very kind staff social workers confided in me: "this used to be a good place." Folks, i dont know what else is out there, but stay away.... read more

I wish I could leave 0 stars. I am trying to get my dad out of this facility as they have no one to help him. There is no staff he was forced to get up without a walker at night after a full hip replacement. He waited 90 minutes for a nurse who never came. He was put in a room with a cognitively impaired nursing room patient (my dad was there for a short rehab stay) and did not sleep for 3 days. Its disgusting how this facility does not care about their patients, safety, or quality care. I'm making a complaint to the state and medicare. No one is safe here. Even during the day you have patients screaming for water. My dad is on 2 drugs and they tried to give his meds to someone else in another room. He's supposed to be under the care of a doctor who is off duty his entire stay. There are poop stains on the couches. The covid procedures are a joke. You are left prisoner because the facility wants to bill you/your insurance company. Your loved one will die here. read more

PLEASE DO NOT PUT SOMEONE YOU LOVE HERE. My aunt spent 17 days there until I went there and wheeled her right out. 1. Her medications were wrong daily. Despite speaking with the head nurse and nurse practitioner on her last night they almost overdosed her on muscle relaxers and Xanax. Luckily my aunt knew it was the incorrect dose and didn't take it. 2. In 17 days not a phone call from anyone! No PT plan, no treatment or post discharge plan. Was called on day 15 to tell me she was being discharged on the day my aunt was told that we were having a family meeting. 3. My aunt developed a severe rash that became open and raw and when I spoke to the nurse she was nothing but nice, however she told my aunt IT'S NOT THAT BAD and did nothing!!! 4. I took daily photos of the garbage under her bed after she asked daily for it to be cleaned. 5. NO ONE EVER IDENTIFIED THEMSELVES WHEM THEY ENTER THE ROOM. We never knew who her nurse was. 6. NO ONE ANSWERS THE PHONE. 7. They refer to her as GIRL as they are extremely racist. 8. The nurses say I'll be right back with your medication and 4hrs later still no pain medication!!! 9. At 2am she had to walk with her broken back to the nurses station to get her pain medicine and they made her wait 3 more hours! 10. She was scheduled to be bathed twice a week and had 2 showers the entire time she was there! I could go on and on! She needed to use the bathroom and hit the buzzer and by the time they came.....almost 40 minutes later she was screamed at for going in her diaper! She was left sitting in filth! They are so rude to the patients! Had I known all of this as it was happening when it was she would've never been there for 17 days!!! AVOID THIS PLACE AT ALL COSTS! read more

My father-in-law was being taken care of at Sunharbor for several years. Listening to conversations between my sister-in-law and my wife, the staff took very good care of him and he was treated very well and respected and loved by many. Unfortunately my father-in-law passed away last week; we were all there spending his last moments with him, then set our goodbyes and left that night, getting ready to start preparing the next morning for the upcoming wake and funeral. The day of his wake, we arrived an hour early to discuss plans with the funeral director and noticed that his wedding band was missing. My father-in-law has not taken off his wedding band in over 50 years. We asked the funeral director if he knew anything about it, he's a trustworthy person and knows the family very well and said he would never do anything like that nor do they have any need to. They just simply transport the person from the nursing home to the funeral home. I called and spoke to the staff at Sun Harbor, they knew nothing of what had happened. I told them the only place where it could have disappeared was when he was there. They asked such a stupid question wanting to know if any of us removed it. If any of us did remove it, would we be calling asking why it's missing??? They were going to call the staff that night and then call me back. I did receive a call back later, they explained that the social worker and possibly a nurse had removed his wedding band after we left and placed it in a box and then placed it on the sink, and that's the last thing they know about it. They asked if they want me for them to open an investigation, I told them absolutely and to call me back. That was this past Monday, it's now Sunday almost a week later and I never heard back from Sun Harbor. It is absolutely ridiculous, detestable, unacceptable and unprofessional that this happened! This nursing home needs to hire the right people, do background checks more thoroughly, have better security and better methods of working with the families. They have my wife's and sister-in-law's numbers on speed dial, they couldn't have called and said to them we are removing his jewelry and what do you want us to do with it? Not a phone call was made, they removed his ring without telling us anything, misplaced it and now they can't find it. Someone took my father-in-law's wedding band of 50 years and now is selling it on eBay or melting it down. This is ABSOLUTELY DISGUSTING. My father-in-law had to be buried with a fake wedding band because there was no way that I was going to let him be buried without one. Personally, I would rather die on the street than ever be a resident of Sunharbor after this happened. read more

Avoid this place if your loved one is seriously ill. They've got covid on 2nd floor, so no visitation except for end of life cases. My Mother is on the 3rd floor in hospice, dying alone because we've been denied visitation two nights in a row. She's in a private room + vaccinated + we're all vaccinated. But the evening security guard is breaking NYS End of Life Covid guidelines by not allowing us to visit. May karma come around for her soon. Oh, and no one answers the phone there because I guess you can only have a problem up to 5pm, even though a nursing home is a 24/7 operation. Hopefully my Mom passes 9 to 5 to make it convenient for Sun Harbor. read more

My mother-in-law arrived at 2pm and by 3:30 we were working to get her transferred to another facility. Just read the other reviews and you will understand why we were moving her. I could not locate anyone in admission to get the transfer started. I talked to the nursing staff and they said they would follow up. Nothing happened.. I found a administrator around 4:45. She spoke to the facility that she was to be transferred to and they required a PRI form. Well get this. Sunharbor told me that it would take a week to complete the form that any Registered Nurse can do in 20 minutes. We did get her out the next afternoon after a terrible night. Unfortunately I had to give a star to post a review. I have experienced the facility for over a year. THIS FACILITY IS EXTREMELY SHORT STAFFED. As a result there is a very long wait time to have adult diapers changed, hence residents uncomfortably sit in feces/urine for long periods of time. Some of the residents do not receive their dinner until 6:30 pm. Most residents are up at 5 a.m. sitting in wheelchairs all day. The call bells go unanswered for up to an hour. I have witnessed aides walk past a room with a patient calling out for assistance, with no response. When questioned why not help - the response is "that is not my patient" . I have witnessed aides in the day room on their cell phones texting or watching youtube. Many of the aides have attitudes and seem quite bothered when they are asked a question. The fault lies with the OWNER/ADMINISTRATORS/MANAGERS - this culture stems from leadership. Staff is overworked and underpaid as a result the patients/residents suffer the consequences. I do not justify these behaviors, however if leadership does not care about providing quality care in a compassionate dignified manor why should they. The reviews/complaints fall on deaf ears. My mother was in for a little over a month For Rehabilitation, they got her up and walking again. She was so happy with this place and called it a Spa to me. Every other rehab she was ever placed, did she begged to get her out immediately. Here she ask me if she could have an extra week. They did an amazing job from every angle. My grandfather went here. Not only did they do a great job on his rehabilitation but tons of activities and things going on to keep him occupied during his stay. Staff was friendly. Food was pretty good as well. Would def recommend to anyone looking for a good center located centrally on long island. We have family on long island, nyc, and Jersey who wanted to visit regularly so location was very convenient.. the facility is really nice as well and clean, nice outside area too during warmer weather. Been calling to check on my father since 1pm it's 5pm no one picks up the phone the operator is useless, the nurse manger VM says you can't leave a message but you can page her, you ask to get her pager & the smart ass receptionist puts you back in her VM again!
