I came here for PHP after my time in Rogers residential #2 in 2022, and that Rogers residential stay #2 was so scary (though looking back on it I'm so grateful for that experience, my first Rogers residential experience, and this PHP experience in Sheboygan which I'm getting to right now), but this clinic helped me wind down so well. I was so impressed by the care of every staff member in both the clinic and supportive living; especially my phenomenal treatment team at the clinic (Scotti the Behavioral Specialist, Ashley the Therapist, and Erin the Nurse Practitioner). I tell that they all truly wanted to be there out of a deep and genuine desire to help (not to mention that they were all super fun!). It's very likely still the case there, but at the clinic they challenge you with exposures but make sure they don't go to an unprofessional degree, and they make exposures with you based on what you would like to focus on and what you think is important. Of course they offer advice (which is normal and always appreciated), but they always said that at the end of the day it's the patient treatment and that they will support the patient with the fears that the patient would like to address. And every staff member at the Supportive Living was so kind (and fun!). The rules are a lot more relaxed than they are at residential (which makes sense). I won't lie - being at the Supportive Living was almost like being at a college party 24/7. If you're looking for PHP at Rogers you HAVE to go to this location. I also want to give a huge shoutout to Scotti, Ashley, and Erin. You three really helped me feel safe and more like myself after residential and supported me when I needed it the most, and for that - as well as so much more - I will be eternally grateful. Thank you all for everything (and please don't leave the clinic in case I ever need to go back - I miss y'all so much as it is)!!

The front desk is a complete circus, and apparently, it doesn't matter what I actually say--they just write things however they want. Make sure to get a copy of every report from your visits, or you're the one who's going to lose out.

I'm extremely disappointed with my recent experience at Prevea. I went in for what should have been…read morea routine physical, and at no point did the provider clearly communicate that the discussion had shifted into billable diagnostic care. Because of this lack of transparency, I was later hit with unexpected charges. When I reached out to billing for clarification, I received repeated canned responses stating that "the coding was correct" and that nothing could be adjusted. Even after explaining that my insurance had changed since my last visit--and that the real issue was lack of communication during the appointment--Prevea made it clear they were unwilling to revisit or escalate the concern. The biggest disappointment is how little emphasis was placed on patient understanding or satisfaction. Clear communication during visits should be a basic standard, and it's disheartening to see that Prevea doesn't prioritize this. I'm sharing this review so others are aware of how easily a simple routine visit can turn into unexpected charges without proper explanation. I will be seeking care elsewhere going forward.

I came in for what I thought would be a routine new patient physical to establish care, but ended…read moreup receiving a surprise bill for $348 due to how the office coded the visit. Despite not bringing up any new concerns and only discussing existing conditions already in my chart, they added a diagnostic office visit charge (CPT 99204) on top of the preventive physical -- something I've never experienced at any other clinic when establishing care. When I asked for clarification and requested a review, I was told that simply refilling a medication for a long-standing condition was enough to justify the extra charge. I respectfully disagreed, as there was no new evaluation or medical decision-making. Still, they refused to make any adjustments after multiple attempts to resolve the issue. I'm disappointed in how this was handled and feel that patients should be clearly informed about how visits may be billed, especially when it differs from typical industry standards. I've since decided to seek care elsewhere for myself and my family.
