We placed mom in this nursing home from late February til the end of May 2025. The first night…read morethere mom was talking to family on her cell phone when night shift supervisor came in disrespectfully yelling at her. It was reported. Mom repeatedly said how mean and rough the workers were. Many times I came in and she was laying soiled with no clothes on just laying in an adult diaper. It was reported. No change so I cut every night gown up the back and put signs up in her room. Mom developed a decubitus ulcer on her foot. Nobody noticed it or dressed it for two days after I repeatedly told them it was there and bleeding. On the third day a nurse came in and dressed it. Aides brought her tray in and placed it just out of reach. Many times. Mom couldn't cut up her food much less get to it. It was reported and I put up signs. The floor was NEVER cleaned in the three months she was there. The bathroom had bm all over the toilet and sink was beyond filthy. Daily. I went every morning. My brother went every evening. We saw personally. The food was mostly inedible. Blackened tough hamburger patties served without any condiments. We asked for mayo or ketchup or mustard and were given ranch dressing to put on a burger. Oatmeal served without sugar. None available we were told. We brought in ketchup, mustard and brown sugar. Finally my brother just brought in dinner every night to her and I brought her oatmeal every morning from home. She had the same (empty) ice water cup for 2-4 days in a row. We brought in our own ice water for her. We kept her hydrated. A very few nursing staff members are outstanding and I truly wish their great care made a difference. But the majority are not there for the elderly. PT was a joke. Mom never had activities dept reach out to her in three months. Every department from nursing to cleaning to the horrible food caused our decision to move her. Thank God the new facility, while having same #stars, was overwhelmingly better in every way. I wish mom had never been placed in this substandard facility. In three months time she lost 60#. SIXTY pounds!!! (Without wanting to lose it) If we left her there and didn't provide our own food (mom is not a picky eater but they occasionally even "forgot" to send a tray for her!)mom would have starved. When I questioned the weight loss after two months I was told she lost 12 pounds. Yet when we moved her 30 days later and saw her weight it was 60 pounds from her admission weight at Nugents. (Also documented at UPMC upon her 24 hour admission there prior to Nugents). 48 pounds in 30 days? Not likely. I highly recommend NOT placing anyone in this facility. We have documented EVERYTHING because some of mom's family live out of state and we sent daily texts. And pictures. Nothing reported was ever dealt with. When they are short staffed, those unable to ask for help get left alone. With a puddle of urine under mom's chair, we had to TELL (insist actually) mom be changed when she was put in bed. They were going to leave her there soiled! On and on. I wish I'd never heard of this place. As a retired nurse, I can truthfully say this facility needs zero stars, in my opinion. We gave every opportunity to rectify unbelievable awful scenarios yet something equal or worse took its place.

My dad just called me to let me know that my grandmother passed in DECEMBER! My dad has called there multiple times prior to and after her passing to talk to her on the phone (no transportation to physically go see her) and every time he called he was informed that "she was busy at an activity". Can anyone explain to me how the hell a dead person can be busy at an activity?!?!?!?

After two referrals from a doctor's office and six messages left with them, still no return phone…read morecall! We currently use them for some therapy. Our doctors office sent two referrals regarding palliative care. My wife left three messages to get information and scheduled and I also left three messages to get scheduled. After weeks, we finally contacted the doctors office to let them know, we had to find someone else because patriot will not call us back. The doctors office contacted them again and they were informed they no longer offer for palliative care. Why couldn't they take one minute to return our phone call to let us know? They are working with vulnerable people who need help and they don't have the courtesy to call back.

I had a knee replaced and after 2 weeks of in home therapy with Patriot I was back to 100% knee…read moremovement. Now I just have to do outpatient therapy to strengthen my knee with physical equipment. Their team of nurses & therapists are very professional, encouraging, conscientious, compassionate, and always on time. They were also very flexible about my time schedule, as mornings were next to impossible for me. They would call to check up on me and make sure that I was doing okay, as well as having a 24 hour phone access if I ever had any problems. I can't say enough about Randy Williams, who was my therapist the entire time. Not having a different therapist each visit made a huge difference. He was so encouraging and patient while pushing me to improve and reach my goal, but also making sure that I wasn't in pain while doing so. He never left my home until I had an ice pack on my knee to reduce swelling. I was sad to see him leave when his job was done. Thank you Patriot for making a very daunting time in my life bearable. I wish I could give you more than 5*, your team deserves it.
