I would not recommend this facility…read more When a facility is unable to have sufficient staffing levels, it is impossible to provide care in a timely manner. (On one weekend all the aides called out for the overnight shift, so an aide was pulled from another unit to help. With 40 beds in Mom's rehab area, it was tough.) Aides should not have to hunt for supplies when preparing to change a patient. Large sized briefs, barrier cream and garbage bags should not be in short supply in a nursing facility. 95% of the employees here are good souls. Their heart are in the right place, they care about their patients and attempt to do the impossible. The kitchen team is incredible. They offer so many options to patients, produce delicious food and are incredibly responsive to requests. As someone who stayed with their loved one - leaving only to shower and briefly nap - I have the following recommendation to caregivers. - Keep a careful eye on prescribed medications - Visit frequently - Check skin integrity frequently - Engage with the dietician and ensure personnel delivering/recovering food are aware of current meal plan protocols. Dietician does not reach out to patient/family to inform them how things are supposed to work. You will need to ask. - The PT program is not a rigid structured time setup. It is effectively first come first serve. Which may result in the therapist who is familiar case being unavailable when the patient is available. - lack of visibility into prescribed medication as well as report of medications actually dispensed, needs significant improvements. Key personnel administering medication have experienced significant unexplained turnover, which is somewhat in step with facilities personnel charges at all positions. When Mom was last released from a hospital, our family opted to bring her directly home instead of risking another experience like the one at Island Nursing.

After many many attempts to get medicine on time it becomes an impossibility to find out why they don't respond. One nurse tried giving my family member the wrong medicine. Thank God they would ask about what they were being given. Their own nurse attended to wounds once every 3-4 days when their wound was to be taken care of 2x a day. She was eating with no teeth in her mouth. wh changing her diaper, stool was left in her diaper and vaginal area where I had to take off diaper and clean after aid. This went on for weeks as well. There is no such thing as low sodium diet and mom has been not 1 working kidney and heart failure which makes it dangerous for her to have too much sodium. The food is also really bad and cheap. Chicken consists of a chicken patty, and lots of pork being served without choice but a sandwich. The physical therapist deprives my mother of the time allowed, so she hasn't made almost no difference since she came in 4 weeks ago. Mom isn't Mobil and can't teach anything by herself and after I leave everything is removed in front of her so she can't get a drink of water, a snack, her tissues, her books to keep busy or anything else. When changing mom's room they left everything of hers, including what will keep her from falling out of bed, in the hallway. I came in at 7pm and had to put her room together to keep her safe. If thinking about coming her I would say don't if you care about your loved ones!

The staff always kept me well-informed about my mother's health. This is key for families of dementia patients. For this kind of care, it's the staff that matters most. The key players are the unit coordinators, and we had great ones: Kelly and Jenna. Both were very patient and attentive. Jenna eventually felt like a friend of the family. She took a personal interest, and went above and beyond to resolve issues in my mother's care along the way. I was there constantly during one period of time, and really saw how things work. I was amazed at the quality and number of people involved in my mother's care. Besides the coordinators, I was very impressed by Supervisor Marilyn; NP Liz; Night nurse Christen; Med nurses Garcelle, Angie, and Danielle; Aides Carline and Christia; OT/PT's Gary, Pierre, and Jennifer; Speech therapist Ann-Marie (regarding chewing/swallowing); Social Worker Donna; Recreation assistant Jennifer (who helped me Facetime with my mother when I couldn't visit); and Violetta (who *tirelessly* kept the whole unit clean). Pretty much everyone really knew my mother and the details of her care! My mother REALLY did not like taking medicines, etc., but the team displayed some amazing skills in gently getting her to cooperate. The comfort care period here went much better than home hospice care would have. The team took care of all medications, cleaning, laundry, and other tasks so I could just be there for my mother emotionally. In all, Smithtown Center is staffed by a large team of very nice people who were very helpful to my mother and me. Thanks, Smithtown Center! One last thing is, that in comparing notes with others, I learned that Medicaid patients got the same, high quality level of care as the self-pay patients.

The staff became our family as we all spent alot of time there during mom's illness especially. The staff in nursing, housekeeping, nutrition and rehab were so gracious and loving to our mom and to us. It was difficult to adjust after mom's passing and Jen the social worker offered her suggestions regarding dealing with our grief. Several days before the Corona Virus became a topic of any sort, I moved my mother to Ridge Crest…read moreat Meadow Ridge due to declining health. This was a very difficult decision as it was going to separate my mother from her husband. However, as I would soon learn, it became the very best choice I could have made. From my initial conversations with the Admissions Nurse Daneen, my mind was put at ease by her expertise, her overwhelming compassion and complete sense of caring about all aspects of the move. Soon after the move, I met Danielle, the head nurse of the wing where my mother was placed, and found the exact same wonderful and comforting set of values in her (I am able to talk with Danielle daily to get updates on my mother's condition). This was followed by the rest of the staff, who, again, continue to impress me in every way. Then the word of the Corona Virus made the news. Ridge Crest under the direction of Kim Held, the Director of Nursing, took decisive and immediate action and locked down the facility to all visitors. This was far in advance of other nursing facilities,and has played a pivotal role in keeping the facility safe. This meant my mother's husband and I would no longer be able to visit her. However, once again, the Ridge Crest team stepped up. They understood the social and emotional toll this would take on their residents and their loved ones. Through the staff's loving and compassionate ways they put the residents at ease, as well as their family and loved ones. I am able to be in regular contact with the nursing staff by phone, my mother via FaceTime (set up by the staff). I cannot imagine a better, safer, more professional and loving place to have placed my mother. Ridge Crest is simply an outstanding facility.

I live here, and I need to warn others: this place is not safe. Most days there are only one or two…read moresupervisors for the whole building, and staff don't respond when you need them. I've been left waiting for help for long stretches, and when they do come, they are rough and impatient, like I'm a burden. The building is dirty, the bathrooms often smell, and the environment feels depressing. I've seen residents yelled at and treated with no kindness. Once, I even witnessed someone being restrained for hours without anyone checking in, which was frightening. Supervisors don't take responsibility--they push their jobs onto others who shouldn't be doing them. When inspectors are around, they make everything look fine, but as soon as they leave, we're ignored again. I don't feel respected, safe, or cared for here. If you want your loved one treated with dignity, please find another facility.

The bad: truly short staffed for the type of care we needed (needed more 1:1 care for physical neurological needs), often fell out of bed/chair with no one to help, got dehydrated, got UTI, got Covid. Not a good place for neurological patients. Need to be able to speak for yourself. You might be lucky and have a roommate that can speak for you when you've fallen out of bed. Maybe you are lucky to have family to make a stink to get the care you need. If not, you are highly likely to languish. Sorry Hamlet. We had high hopes but it did not work out.
