I call this place The Hell Hole. They are not proactive and not even reactive to health care. I was there almost 2 weeks and had to ask for my blood oxygen level saturation to be check, which should be routine. The first 2 finger monitors had dead batteries! Mine was so low I had to be rushed to ER. At the ER a blood clot in my lung was found and clots in my legs. This developed at The Hell Hole. At night, I had to listen to people screaming and vomiting. It was quite unsettling. The food was TERRIBLE. My dogs eat better! I am not sure how the owner sleeps at night. He needs to be forced to eat that food for a week. The "thought" of it still makes me nauseous. If I could post 0 stars I would. read more

I was a patient at this facility back in 2015 after having a mild stroke and multiple seizures that turned into comas. I had a speech deficiency after my health issues. I transferred to this facility after being in Kaiser hospital for 4 months and nearly dying. I was only at this facility for 2 weeks and I left due to getting a stage 4 pressure ulcer from not getting changed when needed. I was constantly ignored due to my speech deficiency, my aunt would visit when she would get off work and change me herself and talk to the nurses about the neglect but the administrator didn't know anything about me and I think they were short staff back then. I left that place feeling so depressed and emotional. I've never been treated like that in my life! I left and was in the hospital and from there was transferred to kindred and later had to get a colostomy for the wound to heal on my bottom! read more

My husband stayed at this facility for over a month and received the best of the best care possible during his stay. The staff was very caring and made every effort to help improve his health, his mobility, his speech and his overall physical status. I am very thankful for the great care they gave my husband especially Sarah (our Angel) and the rest of the team. Shout out to Pauline for her sense of urgency and without her I would not have been able to survive the difficult journey my family and I went through. Most people cringe when thinking about nursing facilities mostly because of the uncertainties involving their loved ones when they have to check them in. Recovery is a long process and can be frustrating. For a nursing center to operate efficiently, it needs to break down their "caring" into tasks to be performed by different staff members. Thus, it may appear that the facility is task-oriented without a lot of human elements. Nursing facilities take on a very challenging job because they are only 1 of the 2 factors (in my personal opinion) for a successful recovery but are often held fully responsible for the outcome. My 93-year-old father was transferred to Pasadena Meadows to "recover" when there was nothing else the hospital could do medically. He was as feeble as a child when he was checked in. The first few days of his stay I sit by his bed watching different staff come and go every hour or so if not more frequent: to change, to take medicine, to eat, and repeat... it was exhausting just watching. Progress was slow at the beginning that I did not think these small tasks would amount to anything. But little by little over a two-month period he has miraculously recovered enough for us to contemplate a discharge plan. From the bottom of my heart I thank you: Jennifer, Johanna, and Delores at the front desk (I bug them the most), Eric the social worker for his guidance navigating the system, Tina the occupational therapist who made my father exercise 5 times a week, Alyssa the nurse who skillfully sent a lot of pills all at at once down my father's throat (I need to learn that), Francis and Rossana the nursing assistants.... There are many other people on different shifts for different tasks that I did not get to interact with or fail to catch their names. The other factor for a successful recovery I think is family. A nursing center has a job to do which is to provide medical and rehabilitation support. But we the family members need to be an active participant in the recovery process to make it work. read more

My mom has been two different nursing facilities and hated both of them. She has been at Pasadena Meadows now over two years and loves the staff and they love her too. I guess you can say the staff has become an extended family for us now. She doesn't want to go anywhere else and neither do I. I wish I could list everyone's name here that has helped her but that would be a huge list and I'd hate to leave anyone off by accident. Very happy I found this location for her! read more

There is both good and "needs improvement" at this facility. It is clean and the patient care staff is very nice. Residents often complain about the food. I have heard this from multiple sources. I am allowed to bring in food for my mom, but the staff looks at you blankly when you do. Make sure anything that needs refrigeration is sent directly to the kitchen or it will sit on a night stand for over a week. (Today is day 7.....I am waiting to see how long it will sit there. ) My mom is in hospice. Much oversight is needed. The nursing is great. I would give them a solid 4 stars. The administration and social worker is the loss of the other two stars. One would think a social worker would assist the family moving a patient from rehabilitation to hospice. I have no words. I would recommend Pasadena Meadows for rehabilitation but if your loved one is at end of life, I would not. read more

My grandpa stayed at this facility for a period of time and received the best care possible during his stay. The staff was very attentive and accommodating. I also loved how clean and bright they kept their facility as other nursing homes I've been to have been dirty and dreary. The facility looks good and the CNAs and LVNs try hard but the administration is a mess. The director (David J. Weaver) is barely attentive and the social worker and other administrative staff are too young and inexperienced. It seems as if they are focused on getting patients for the reimbursement but not concerned once admission is finalized. I think the nursing staff is good but the pace is too busy and they are overwhelmed. My grandmother's clothes went missing a lot and ended up on other patients even though they are labeled. Don't be fooled by the administrators that make all sorts of promises about care. You will end up frustrated. My grandmother was moved around a lot to different rooms without notice and when I would call to speak with social workers and administrators, I would get the run around. Her hospice care team was also denied access to her, so I moved her to another facility. This place could be good with a different administration in charge. read more

It's been a rough past year because of COVID, and the restrictions it incurred with being able to visit my family member at this facility. When I had a question or needed someone to check in on my father because he wasn't answering his phone, a staff member was always so kind and helpful in getting my dad his phone or relaying a message to him for me. Overall, the staff and its administration have always been friendly and tended to my dad's needs, even before COVID came around. So, for anyone looking for a place to send their family or friends that need rehabilitation and this type of extended nursing care, then inquire with Pasadena Meadows. We found ourselves in need of a nursing home for our mother who experienced some life-altering changes and had to leave her independent living facility, and it was very difficult to finding a reasonable priced place with the amount of oversight that she required. This place has been a god-send. Agreed that the food needs some improvement - my mom complains a lot about it - but the care and the place itself can't be matched. It's brightly lit and very clean. Despite being relatively small (we actually preferred this to a huge institutional facility), there are various places for a family visit. Also, the staff is just wonderful. The nurses are very caring and treat my mom with kindness and respect (and she can be a tough patient!). The front office staff is very nice and helpful when we arrive or call. The administration is good about staying in contact and addressing concerns. We would like to see more activities for my mom to engage in, but granted the recent Omicron surge has made things difficult. Hopefully once things subside, the residents will have more options. Nursing homes are difficult places to process given the various stages of life its residents find themselves facing, but we have felt we made a good choice and were fortunate to find this location. read more

terrible, I was abused there from the first day. came from hospital after major back surgery and after being there over five hours no one came to see me. when I finally got meds they gave me two excedrin and one of the cnas shoved me from behind because I couldn't turn over fast enough. night time nurse Glenda refused me meds on my list because she said nifedipine is only for blood pressure. the next day she was brought to me and corrected her mistake but that didn't help the pain of the night before. nighttime Josh was extremely rude the whole time and several of the workers raised their voices at me. I would Never send a relative there. appallingly bad care, I finally left early so I could at least get my meds on time. they should not be in the business read more

My grandma is currently a resident here and it's been one of the worst experiences we've had. Making this decision was one of the hardest things we had to decide as a family because she needs 24/7 care but every time I leave this place after visiting her I leave with tears in my eyes. I went to see my grandma and she has COPD I found her with her head down and her oxygen was low. After turning on the call light a nurse walked in and when I told her my grandma needed to be repositioned and changed she told me she was the treatment nurse and that a CNA needed to do that it took someone 45 minutes to get there and the entire time I saw that same treatment nurse talking with someone else. Finally after the CNA changed my grandma she went to tell the nurse that her oxygen was low and finally someone came to see my grandma. It breaks my heart to know that my grandma is being treated this way. After explaining this to the DON and staff nothing was done and now my grandma is fighting for her life in CCU because of the negligence she received from this place. Please do your research before having a love one be in this place. A simple apology is not going to make this go away we trusted this place with my grandmas life and now we don't even know if she'll make it. read more

LIFE-THREATENING CONDITION DEVELOPED AND ESCALATED Sept. 15, 2018. After spending weeks at Pasadena Meadows Nursing Center (PM), a family member developed sepsis and PM did not take appropriate action in a timely manner. After a couple days of violent pain and suffering, the patient was finally rushed to the ER, very sick, and was transferred to CCU. The patient's condition had developed and increased until it was immediately life-threatening due to inattention and delayed response on the part of the nursing facility. PM has some staff members that are quite nice and trying to take good care of people, but the facility is understaffed and they are apparently operating under policies that hamper appropriate care in a timely manner, even with sepsis. The building is overcrowded and has too many patients for the staff and space. After I had reported some concerns six times, they were still not addressed. Insurance is also a problem. The insurance company does not appear to cover facilities that are local with good track records. I had tried to get them to cover a different place. This happened during Sept. 2018. read more

My Mom had her nicer clothes STOLEN our of the laundry here, many times! I want to get her an iPad but I know for certain it will be STOLEN by the staff!!! This really irks me. The aesthetic was deceptively inviting.Upon my first visit to the location I thought it was beautiful inside and out. If their patient care was anything like their maintenance care of the facility perhaps there would be no complaints. My aunt has issues that were communicated to the staff upon her arriving at the location and several times during her stay. She was sent for an evaluation and was placed on medication to prevent her episodes from occurring. The staff often called my mom that my aunt was having episodes and when my mother would question if they were giving my aunt her medication on a consistent basis, they would transfer her around and leave her on the phone to hold until someone ultimately hung up on her. Since my mother is disabled herself and doesn't have transportation she wasn't able to visit the location personally to see to my aunt's care. In respect to my aunts privacy, I won't go into further details on her treatment but suffice it to say they "lost" my aunt. After the hospice nurse informed my mother that when she went to attend to my aunt, she was told she wasn't at Pasadena Meadows any longer, my mother called them and they told her she had been sent to the hospital. Upon contacting the hospital, they advised my aunt had been sent back to Pasadena Meadows after being seen in urgent care. It took my mother several calls and being told they didn't know where my aunt was, that she ultimately contacted Pasadena Police Department to request a welfare check. After the Pasadena Police deputy contacted Pasadena Meadows, they were provided with the whereabouts of my aunt and provided them to my mom. It's ridiculous that law enforcement had to get involved just to get simple information to the whereabouts of my aunt. They treat patients who aren't entirely capable of caring for themselves as an inconvenience and dismiss concerned family's inquiries. The way this location treats patients who are mentally incapable to take care of themselves is atrocious and I would NEVER recommend placing a loved one in their care if they can't make decisions for themselves or if you have a disability or distance issue where you cannot visit regularly to monitor the care of your loved one personally. read more

From the moment my mom arrived in this facility, the staff worked to make my mom feel comfortable and at ease. The staff being at her eye level when communicating with her. Every person I met, no matter what time of day went the extra mile and had time for my mom. The staff are very professional, and so caring and compassionate to my mom. Questions were answered thoroughly and everything was approached with a sensitivity and attitude of a loving family. We highly recommend Pasadena Meadows and their staff to provide a quality of care we had only hoped for. I've been to two rehabilitation or care services. The first one had decent staff but we were not allowed to leaved the building unescorted. I felt like a prisoner. I won't mention the name. I'll stick with Pasadena Meadows. I was there in June of 2018. In my two experiences of care facilities I realized that there are employees who are great and some who are horrible. I experienced both ends of the spectrum. Overall I was fortunate to have nurses who understood some of the mental issues I faced after dealing with my surgery. The ones who didn't heard from me big time. I have no problem telling anyone regardless of their position when I think they are incompetent. I liked the fact that I could outside anytime unescorted to walk around. I so appreciated the staff that took care of me when I was having emotional meltdowns. I was fortunate to have two wonderful roommates because that makes a big difference when you are stuck in a room with people you don't know. I was on a special diet and the director of nutrition was always checking to be sure that my dietary needs were being met. I was also on some serious IV antibiotics and one of the sweet nurses would always make sure to bring me in some cereal and milk before my treatment because it helped make the side effects less severe. I do know that the facility was going through management changes however that is no excuse for what some of you posted regarding your experiences and I'm sorry you had to go through that. I hope I never have to go to another care facility but if I did, based on my personal experience, I would have no problem going back to Pasadena Meadows. I guess that if you or someone you know has to go to a care facility, the best thing to do is to go for a visit, walk around and see if residents look happy or distressed and talk to management. read more

Wow. This place seems amazingly disorganized. I made a simple call to talk to a patient there who might wife was having difficulty getting ahold of. In fact, they had already hung up on her. In any case I talked to one person named Charles and asked what visiting hours were. He told me they closed at 8 PM. When I ask what was the earliest someone could come, he said he didn't know because he only got there at 3 o'clock in the afternoon. When I asked if the hours were posted anywhere he said 'why would you ask'. I told him that if this was his first day, I could understand that he didn't know the hours for visitation. In any case, I asked for the supervisor and the administrator and he told me that the supervisor's name was Princess and the administrator was Charles. He then got irritated and hung up on me. I called back and asked for Princess and was transferred to a woman who started asking me questions. I then asked her what her name was and she said that it was Dawn. After a few minutes of talking with Dawn, I asked if I could talk to Princess. She then said her name was Princess! I ask her why she didn't just tell me that to begin with and she was evasive. Anyway, I finally found out they have a cell phone that can be called and they walk it over to the patient. BTW - Charles on the first call told me that that it was not possible to talk to the patient. When I told Princess that this was a bizarre conversation she interjected that it was not. In any case, if you have any choice in the matter you might want to seriously avoid sending anyone that you know or are close to to this facility. read more

The CNA's are so over-taxed! At night, the patient to nurse ratio is 19+ to one. You push the call light, and 1/2 hour goes by with no assistance. By the way; the call light is general--could be for A bed,B bed or C bed. This establishment is changing rapidly (for " cost efficiency")! Rumor has it that it will be an only short stay tracheotomy post-acute center. It is the worst SNF I've experienced. I would not recommend it at all. read more

Once again I found myself in need of medical observation and care after discharge from Methodist Hospital. There was no question in my mind that I had to "come home" to the Meadows. I can only re-iterate that I don't believe there is any "perfect" nursing home but as skilled nursing facilities go, I wouldn't go any place else when I need care. It's good to feel comfortable in a place when you can't be in your own home. The administration & customer service have once again gone out of their way to make my stay as pleasant as possible. a new, improvement is free wifi throughout the facility. TV being what it is, its a real bonus to be able to stream Netflix on my laptop. I noticed that someone complained in another review that they thought there were "homeless" people here. As a retired medical professional, I can only say thank God they take in the homeless who need medical care. When those folks are ready to be discharged, social services works with other agencies to be sure they don't have togo back on the streets unless they choose to do so. So again, no SNF is perfect and none of us "want" to be in one but given the many alternatives, I'm for Pasadena Meadows every time. read more

My sister was placed here after cancer moved to her spine and she couldn't walk. She needed 24 hour care that I couldn't provide. She was placed in a room with a patient that couldn't speak & who either screamed or made loud sounds almost 24/7. As she was dying and having a hard time talking, I couldn't hear her speak with the loud woman and asked if they could move the woman to another room since my sister was in her last 24-48 hours. Instead they moved my sister. When I came back a few hours later I found her in her new room in the dark, in terrible pain. Clawing at her head, tears rolling down from her eyes, unable to speak. I started asking when she had gotten her last meds. They said about 4 hours ago. I started asking how someone in that much pain, on their death bed, could only be on meds every 4 hours. They said that was what was ordered. Then I called Hospice to ask what the hell they were thinking giving her so little meds. They said they had changed the med orders 3 times since the every 4 hours order and she was supposed to be on morphine every 30 minutes since the night before. The staff weren't doing it and didn't even know. Truly horrific. The shift that picked up her care once we cleared up the orders was wonderful and empathic and I've met a few good people here but they are understaffed and many seem too overworked to deal correctly with many of the patients. Not knowing the medication orders for a person dying of a painful cancer? Unacceptable. read more

Months ago my my dad was admitted here and I saw the yelp reviews, mostly positive. I overlooked the bad reviews because of the glowing reviews. The facility looks brand new, looks on top of their game. Especially the first month! They try so hard to make a good impression. It was as if I had hit the jackpot for my dad & I had found a great facility! Then the cracks started to appear. My dad was constantly complaining of needing his diaper changed and the staff saying they had just done it. Of course it hadn't. I watched other patients call out for help and spend 45 minutes to over an hour calling for a nurse, even with the call buttons being lit up. Or I'd see nurses come in, turn off the call button and say they'd be right back. It would be an hour or not at all. They're short staffed, so a few nurses are really watching over 20 patient clusters at a time. The nursing staff are all relatively nice but they're overburdened. Then there's the social workers. One of them, Patricia, she's very curt and doesn't care to talk much with families. I had met with her once and she kept deferring me to other people. Always hiding in the nurses station. Then there's Jasmine. Useless. Misplaced important request forms, didn't schedule appointments even when I had confirmed multiple times, had no idea about any of the programs my dad would qualify for, had to reminder her over and over again about things I wanted her to look into. I don't think wants to be a social worker. She's going to school for something else & just wants a new job. Neither social worker has any idea how to hunt for social programs to help your patient. I was constantly told to talk to billing for these programs. Why the heck would billing know? On that topic, the business office are the most knowledgable there. There were questions I had the rest of the straff should have known. It was their business to know, and I'd get a "uhhh I don't know." I asked a discharge nurse a question and she said it would take 5 minutes. I walked out for a minute and came back to her on facebook. She didn't know I was behind her, so I watched her for those 5 minutes message just endlessly scrolling. One of the other staff had to tap her and she promptly hid it like I had never seen it, and said, oh, well, honestly, I don't know the answer to your question. Go to business office, took 2 minutes. Even printed out what I needed. She's a badass. That's the best thing I have to say about Pasadena Meadows. The 2 discharge nurses aren't registered for the job. Admin are making them do the work anyway. Let's say, once they decide they want you gone, they'll try to get you anywhere else quickly. The staff doesn't have ANY connections, so they outsource to recruiters working on 100% commission. They have "customer service" workers who weren't certified in anything far as I could tell. Nothing but helper monkeys moving patients & sending messages to the staff. They were relatively attractive women who would smooth over bad news to families. I watched my dad's stuff get misplaced under their watch & mix it up with other patients. Constant issues included the lack of communication between staff. The nurses & nurses station had ZERO idea my dad was being discharged the day he was discharged. Despite me telling both the business admin and the discharge 'nurse'. If you don't have planning meetings with every one of the staff that works with your patient (PT, dietary, activities, social worker, etc) none of them will know what's going on. Their doctor is offsite and only comes once a week. Honestly, never saw Dr Jacarvi. Food was inedible. I tried to see why my dad was so hungry when I visited him. Everything was like moldy cardboard. Oh, and you need to go in person to do any serious talks. Trying to get ahold of any of the staff over the phone means 10-30 minute wait times, or a constant "I'm sorry" from the receptionist (they've lost a few receptionists in the last few months, that's telling). Let me also add that Physical therapy will tell you patients are doing better than they are...until the insurance runs out. I was told my dad was being seen by PT every day. I can tell you that wasn't the case. I'd wait hours to see if they'd come. Nope, no PT. Their PT admin will make the actual therapists cover that up. What's worse, after they discharged him, I found out my dad had a bunch of serious health complications (UTI in blader, broken molars, eroded teeth from irregular brushing, malnutrition, fungal infection on his face from irregular baths) that outside doctors and dentists told me were likely caused at his prior facility. Fairly certain nursing abused him in the shower, he was terrified of baths in the new facility. Only 2 months after discharge, my dad died of health complications likely caused by Pasadena or should have been caught by them and weren't. Please, if you love whoever is ill, don't put them here. I regret it everyday. read more

In visiting my grandmother I was very impressed with this facility. I have been to other nursing homes where the place feels dark and gloomy but as soon as I visited here I knew this was a great place. Right as I came in the entrance I felt at home. An open vibrant lobby, huge fish tank, and lots of natural light. It seemed that everyone working was very kind and helpful while accommodating my visits with family. Everything looked and felt clean. My grandma loves her nurses and also the BBQ chicken they make. They really care about everyone there and it shows. read more

It was explained to us that the rating is mainly from the old place. It was the old Marlinda. It is totally remodeled and has been under the new owners for a little over a year now.
