To the Team at Palm Garden of West Palm Beach, I am writing to share my sincere thanks for the care I received during my recent stay. Recovering isn't always easy, but the kindness and professionalism of your staff made a world of difference to me. I particularly want to mention the nurses Rochell, Sierra, Marthe, and Shernette​ for their patience and encouragement. It's the small things-a friendly check-in or a quick response-that helped me feel like more than a patient. The physical and occupational therapy gym is top-notch. I particularly want to mention Robert, Gloria, Kari Anne, Brie, Jordan, and Carlos who inspired me to work hard in spite of the pain and dizziness to recover. The CNA's were hard working and compassionate. They helped with my basic needs and treated me with respect. I particularly want to mention Miriam, Love, and Betina. Thank you for being there. Social services gave me sound advise through my stay. They made my discharge happen. I particularly want to mention Sol and Mickey. I especially want to thank Sol for intervening in a crises. RUN don't just walk away from this facility! It is the worst experience we have ever had at a healthcare facility. If you care about your loved one, find a different rehab. We brought so many issues to the executive team during my mom's two week stay. None of it was addressed properly and some things reoccurred. Here are a few examples of the poor care: 1. This morning I learned that my mom's bipap machine was not connected to the oxygen for the entire evening. Her oxygen level was 80 when the morning nurse came in. This is a dangerous level! Until I asked, this was not even documented so that someone could assess it with evening nurse. 2. Multiple times my mom's IV and pill medications that were due 3x a day were given so late in the morning that they had to do two doses close together instead of 8 hours apart. She would have missed her third medication if I wasn't tracking it. 3. Medication was left out in the open on the medication carts. Not only is this illegal but very for some of the residents who wander through the hallways and could take the medication by mistake. See two of many times I took photos of the unattended medication. 4. They are understaffed. It took upwards of half hour to two hours to get my mom changed after she had an accident. This is so unacceptable! The only bright spots of this facility is Bijou, the respiratory therapist and the PT team. They are good but still not worth risking your loved one's life at this facility. Do yourself a favor and do more research to find a better place. read more

No no no ! Nurses on a particular floor extremely unprofessional, hire better qualified employees, staff employees who have bedside manners and people who really care about the patients ! Employees not eating a tangerine behind the desk when patients are yelling for help!shameful! FOCUS ON IMPROVING CERTAIN STAFF MEMBERS , SORRY!There are several facilities other than this one! Please, please do your research, this facility has a perfect score potential, however this facility doesnot fall into a perfect score facility, it falls short! BEWARE! The first time my husband stayed there as a diabetic with one foot half removed and wounds on the other, he could not walk. They let him use a wheelchair with his feet walking it. The same dirty socks every on his feet time I Visited. And there were small bandaid's always at the bottom of the socks, filthy. When they sent him home he could not walk. So much for rehab. I paid them $1750- and in 4 days he was septic with pneumonia from the hospital. Septic, his feet needed more surgery. The second time the room smelled of poop. Puking and gagging on the phone. He could not stand it and demanded to go home. BEWARE!They wanted me come and get him, so I paid someone to drive us and help me. I'm 76. When i got there.They said he was supposed to go to somewhere else, so they would not let him go. I go home and awhile later a police car drove up with him. Walking in without shoes with a kind policeman helping him. he was naked, but for a non tied hospital gown. He has the start of dementia and refused to stay in a poop room, gagging and retching on the phone. So if you hate your relative and want them dead use Palm Gardens because they almost killed him on me. His feet should have been bandaged with thick pads, gauze and wrapped well. They never did it. so I was dumb enough to pay them $1750.00 to get him in the place second time. This head black nurse was really brutally nasty to me. I have no idea why she was so unprofessional. As I said. Beware! THANKS FOR ALMOST KILLING MY HUSBAND PALM GARDENS. YOU MADE HIM SICKER!! WHY DID YOU YOU BREAK YOUR OATH TO DO NO HARM?????? HORRIBLE PLACE, TERRIBLE FOOD,DISGUSTING. read more

I want to add to the mixed reviews that I see on Yelp that the staff at Palm Garden has been and continues to be a godsend for my brother. He is receiving physical and occupational rehab and after he spent 3 months at a former facility totally bedridden, he is now walking with a walker. Everyone has been incredibly kind to him and to me and to the friends and family who come to visit when we can. Staff is receptive to suggestions and trying new ways of working with and engaging him. I'm Si Liberman. I wrote this on my daughter's account. Shortly after my 100th birthday I broke my left hip in a fall. My wife, Dorothy, 96, fell and broke her right hip four weeks later. So there we were in rehab rooms across from each other at Palm Garden in West Palm Beach. Previously Dorothy had suffered a left hip fracture and other bone fractures and had been able to get around with a walker. Fearful Dorothy might never be able to walk again, I recall telling my physical therapist, Lorena, about my wife's earlier fractures. "Ever treat anyone who had been treated for two hip breaks and got them mobile again with a walker?" I asked. "Yes, definitely," she said. On Feb. 25 Lorena surprised me with a video of Dorothy walking with a walker near a nursing station. Walking behind her were Lorena and Robert , Dorothy's main therapists. I couldn't thank them enough. What also pleased me about Palm Garden were the meals. As institutional food, choices were quite satisfying. If you prefer something in addition to or in place of what appears on daily menus, efforts are made to accommodate you. Cleanliness was also a plus factor. Seemed my room was cleaned almost every day. On occasion, especially weekends, it took longer than usual to get a nurse's prompt response to a call. However, one nurse - her name's Anna - stood out. My husband went here for acute care for a wound with a wound vac. They had no one who knew how to change the dressing or place the wound vac on. There is no pharmacy on premises and limited medications. My husband went without his pain meds for 18 hours until I brought them from home. After leaving the wound vac off for 2.5 days the head nurse put it back on after I threaten to call my attorney for negligence. She was wonderful however after I had him discharged and brought him home the skilled care showed me how poorly knowledgeable, they were in putting the wound vac on. They placed the sponge right on top of the wound which did not allow for drainage. The discharge was just as poor because the antibiotics needs to treat n the infection was never called in by the doctor to our pharmacy. I had to follow it up several times to get it done and filled. The doctor who made daily visits was just as bad. He was supposed to listen to my husband's lungs to make sure they were not filling up with fluid but that was not what he was doing. If you need a place with skilled care this is not where I would be going unless you are on top of them 24 hrs a day. read more

My brother is leaving and going to an assisted living center. He's been there about 6-8 months and I wouldn't let my enemy live there! When he was on the third floor doing rehab it was ok, then when he got better, they moved him to the second floor. it's been a nightmare! They have moved him 3 times to different rooms. The first roommate was on oxygen and needed the room cold, my brother couldn't take the cold, so they moved him to another room, this roommate was by the window and needed care 24/7 all hours, so he wasn't getting any sleep, the next roommate was by the door and needed 24/7 care, they would change him and the odor, well my brother couldn't sleep. Whenever I'd call the Unit Manager she wouldn't call me back after time went by, I'd call the nurses station and as time went by they wouldn't answer the phone. My brother had surgery, and after that he has had about 3-5 mini strokes, because of the stress of being there. I almost had to call 911 for him because he was having a stoke, but I get finally called. I thought everything was going to be good for him, when I first talked to the Unit Manager, but I was wrong!!! Yes this is a long term living place and my brother shouldn't of been there, but the care went from good to horrible! I knew they wanted him gone and he knew that! please think twice about leaving a loved one here. Some are good workers, but most are not!!! Please think twice!! read more

I love this rehab. I know there have been some negative reviews but for me this place is really nice (compared to some of them). The staff is very nice, helpful, it's clean, and pleasant. i've seen other rehab centers where you walk down the hall and there's a bunch of people, crying and yelling (elderly). My husband was transferred to Palm Garden of West Palm Beach year ago. Having him at a nursing home for the first time, was a "shock to the system". However this place turned out to be unusually nice. The floor staff is helpful and kind. at all times. From what I gather, its unusual to have that quality of nurse's aides and nurses at a nursing homes. The only thing I believe needs improving is their phone/email response time. Several phones are unattended, have no voice mail or voice mail goes unanswered. Same with e-mails, they rarely knowledge receipt. Leaving one wondering if they are in receipt of the e-mail and/or taken any action. Response time aside, I highly recommend the place due to the floor staff, specifically the quality of their nurses and nurse's aides. read more

Great facility! I know that when i visit my grandmother here she is in good hands. The nurses and CNAs truly care about the residents. They always have some sort of activities going on so they all stay busy. I highly recommend palm garden! read more

This place is a joke, I literally have seen a resident with what i think is her daughter or granddaughter who comes to visit her and then stays for weeks at a time. Is this place safe for my grandparents when someone who clearly looks high on fentanyl is staying in the room with two other elderly women and saying horrible things to them each time I walk by. About a week ago they could not find the woman, hours later they said she'd been in the bathroom for 3 hours and looked to be overdosing on medication she's probably stolen from the two elderly women. read more

If I could give this place 0 stars, I would. My father had a stroke and had the misfortune of being transferred here, after his hospital stay. From day one, it was clear that the nurses would do their jobs only when it was convenient for them. My father was on the "rehab floor", so it should've been clear that he was there for a short period of time and that the main focus was to REHABILITATE his health. Yet, he only had about an hour of rehab a day, M-F. But, if the nurses didn't get him up and dressed in time, he wouldn't even be taken to therapy. He was prescribed occupational, physical and speech therapy. There's no way he received that in only an hour, if they even took him. My mother visited my father daily, for a minimum of 5 hours. During the time she was there, the nurses rarely came to my father's room. My mother had to feed, bathe, dress and care for my father. My sister and I live out of state. But we had several phone calls with the social worker Dora and the management team. They assured us that his care would improve. It did NOT. After five weeks, the insurance stopped paying for his stay. My mother was told that she would have to start paying out of pocket. She declined, since SHE was doing all of the work anyway. She opted to have him discharged and sent home. On the day of departure, he was supposed to be transported to their home that morning. As of 2pm, he still wasn't home and my mom (who doesn't drive) had to get my uncle to take her to pick my father up. We also learned that since they thought he was being discharged that morning, NO ONE had fed my father or given him care that whole day. This place is what I HATE about America's healthcare system. They only care about money and insurance policies, not the patients. read more

Palm Gardens was adequate in care. The nursing staff did a good job. The plus were the ladies in the activity department. Tina, Maryann and Karen are super stars! They go above and beyond to make life in a nursing home seem ok. My mother loved them! read more

I have called minimum 30 calls in the last 30 minutes to the facility and not one person answers any extension. I called first I guess floor second third fourth and even the supervisor and his voicemail is full. I have tried and tried and tried to get a hold of anyone there. No one answers for at least one hour. read more

Palm Garden rehab in West Palm Beach is definitely where you or a loved one should ever consider going to. With the exception of the Dr. in charge of my Dad and one aide, Bathsheba the employees mainly the aides are the rudest, most uncaring individuals Ive ever met and I manage a Drs office for over twenty years, I live a mile away so I visited my 89 yr old Dad every day, staying for hrs at all different times. Imagine hearing aides complain to the patient that they haven't had their break, that they are short staffed and that they aren't the only patient there and have to wait for help. My father is very alert, no dementia and well aware of what's going on. I sat there for hrs and watched as patients screamed for help, I timed how long an aide came and it was over a half hour. I had to go looking for someone only to stand at an empty nurses station for ten minutes and be told repeatedly that they were either busy or on break. If I could give this facility a zero, I would. It's a sad state of affairs when your loved one leaves in much worse shape then when they entered. While there are a few that work there that are amazing, it was a horrible experience, overall. There is some serious neglect going on there, undiagnosed conditions and serious deep tissue skin ulcers, to name a few. They should shut this facility down. read more

Do not allow your family to go here...absolutely the worst place ever. My dad was there for a month after surgery. I can count on less than one hand how many times they checked his wound. He was readmitted into the hospital for sepsis and surgery. Everytime I visited there were bells ringing for service but no one in sight. Then there were times they were ringing and the staff didn't move. I visited my dad, who is blind, and he had done messed on himself, eventually spilling out onto the bed, and kicked his table tray which spilled. I rang the bell twice and no one came, but it was turned off. It took almost an hour for someone to clean him and over 2 hours for someone to clean the floor. The facility itself is nasty and smelly, even though they were remodeling...I never saw it. read more

If at all possible do not have an elderly parent or relative stay at this facility. Horrible place... Palm Garden is understaffed during the week and on a skeleton crew on weekends, with an inability to adjust for most problems on weekends whether they be medical, practical, or the kitchen staff being able to follow a patient's dietary restrictions. Physicians, nurses and executive staff rarely if ever respond to phone calls or e mails. If they do, do not expect a clean answer, but rather excuses and lies. The nurses stations lack voicemail, and in an acutely serious situation we could not find anybody in the "chain of command" to address our very serious medical problem. The medical record keeping group also has some serious problems along with acute understaffing. The complete medical records we received many weeks after our mother was discharged, were far from complete, and lacked critical information concerning her treatment. read more
