Because I'd been with CHI/Alegent since I can remember, I decided to look at the MDs at this clinic that's practically in my back yard. I'm very glad I did. I chose Dr Jacobs because, of the doctors at the clinic, he has privileges at both Immanuel and Bergen Mercy. I thought that was pretty good. Little did I know, I'd end up with so much more. The office staff is wonderful. They're approachable, non-judgmental, and warm. The medical support staff are equally amazing. They're easy to talk to, knowledgeable, and genuinely care. I have no doubt they'd be as warm and human if I ran into them at Harold's down the street over lunch as they are in their scrubs. It's very easy to get an appointment, and they offer an on site pharmacy for those who might need it. Dr Jacobs, however. This man is the pot of gold at the end of a rainbow. It's well documented that women spend more time waiting to be diagnosed, listened to, and healed by the medical community. Dr. Jacobs is doing his best to dismantle that statistic. He's taken the time to know me as a person, not just comorbid conditions. His consults are storytelling sessions where I can be honest, without fear of judgement, and fully involved with my own care. He values that I try to be as informed as a lay person can be, but ultimately respect his opinion and expertise. My last appointment ended with a genuine, supportive smile, and a "you got this" high five. Every woman, every person, deserves to be treated like this by their medical professionals. I will be a patient here as long as they'll have me.

I've injured (or developed chronic pain) every joint on that same side of my body in those years, with the exception of my elbow. Working with CORE physical therapy has given me hope that the pain can be contained, if not resolved. The providers always offer me a bite-sized tip every time I visit that is relevant to my situation and helps me make small adjustments regarding movement and recovery throughout my week. I had never done dry needling before but I can't recommend it, or this clinic enough.

There is a rehabilitation side where the patients do not live here but are here for an extended stay. There is a large therapy gym with a pool on this side and each hallway has its own nurses station, nurses supply closet, own seating area with a tv (there are tv's in bedrooms too) and its own dining room. I really like the elegant touches. The other side of the building has long term patients. Each hallway is the same as the rehab side and there is a smaller rehab room. There is a lovely pond on this side of which I put my two pet turtles in when I moved away. Patients saw them as a sign of good luck and enjoy watching them. I hear they eat popcorn often being that on each side of the building there is a popcorn machine. (there are also ice cream machines. There is a chapel, rooms to reserve for private family birthdays/holidays, there are activities like bingo and cards, and overall this is a great place. I was once the person in charge of supplies. I know that we ordered the best quality of products and if we didn't have it I'd run out and buy it. Our Kitchen is high end and our house keeping department has a great room with washer/dryer, a large folding table and so on. You really just have to go and see it for yourself. Jack Vetter the founder visits often, his mother stayed at the sister facility Brookestone Village. The Administrator is a very knowledgeable and nice person. He had been a nurse for several years then was the director of nursing before the administrator so he's done the dirty work and knows what it takes to provide quality care. The Administrative assistant is a kind professional woman, she had her father stay for rehabilitation while I was working there. I could go on about the staff, HR is great, and so on. I'd stay here if I needed rehab.

Friendly, laid-back, well organized. Front desk staff were helpful and the way they handle billing was sensible and organized. I felt like I could call and get in quickly for an unscheduled appointment if I was experiencing unrelenting pain. I recommend them.

I was very skeptical when I first walked into their office. I mean, I'd already done this how many times?! Now, three months down the road, I could not be more grateful to Jake at Horizon. He didn't just do some stretches, pop a TENS unit on me and send me out the door. He tried many different therapies and devices until we found what worked for my neck pain. When it came time to get a home unit for me to continue the work on my own, he worked his butt off to get everything squared away before the end of the year so that it would be completely covered by my insurance. I will never be able to put into words how grateful I am to have my normal life back without constant pain getting in the way. I will never go or recommend anywhere else again!

All were knowledgeable, friendly and personable. Look, if I have to go somewhere 3 times a week for an hour I would like it to be sort of enjoyable. The staff were all very accommodating. If I had to cancel because of the weather, they understood and would try to work a reschedule as soon as possible. It is true, what they say, if you are hurting more after PT than before you went in, you aren't doing it right. Listen to the people here, they know what they are talking about. I did the work there and at home and I am almost back to normal after a short 4 months after surgery! Added bonus! They are within hobbling distance to some of the great places in Blackstone!

I can't believe the improvements in my daily work routine, sure I'm still sore. I wasn't expecting a miracle, still it's a noticeable improvement. Looking forward to the next 4 weeks to see what gains are to be had. Hoping to find surgery isn't needed if I stick to the exercises that I am being given. The hours work with my schedule and I'm very thankful for that rather than taking time off at work. So far I've worked with 3 therapists and all have been extremely helpful in reaching my goals, not the doctors. Mine are much higher. So give them a try..
