My mom lives in the Assisted Living portion of the facility. She absolutely loves it there. The staff is so tentative and kind.

My Mother was a patient here for over 5 weeks. During that time we watched her slowly deteriorate as she was consistently neglected, with a serious lack of compassion from the majority of nursing staff (namely BR and TONYA), as well as management (Brooks, Melissa and Marisol). Despite repeated concerns raised to all of them, including 3 of the therapists, their responses ALWAYS focused on how good her insurance was, and "that she should stay to get a little stronger". She only got 1 hour of therapy a day leaving 23 hours filled with poor nursing attention and inadequate care. Another major concern was the response time to calls for assistance, which could be anywhere from 1-2 hours regularly. Then when someone did show, they acted so put out and were extremely rude- we were not rude with them by any means, knowing they would only take it out on my poor Mother. It became so bad that we ended up having to step in and perform the basic care tasks that should have been handled by the nursing staff. As her health greatly declined, the facility delayed transferring her to the hospital despite clear signs she had become increasingly ill. It took Tonya, head nurse, going to Melissa, assistant director of nursing, before they would even call an ambulance. Even then they said they did not agree that she needed to go. By the time she got to the hospital, we were told within 12 hours she would not make it. I would highly advise anyone taking their loved one here. I am giving 1 star and that's for the nurse aides who were completely overwhelmed as they were greatly understaffed. They seemed to be the only ones who showed any form of compassion. This will not bring my Mother back but I pray it saves other lives. read more

DO NOT send your loved ones here! The facility is ok looking, but the beds are terrible. Nurses and PT staff are good but understaffed. The office workers refused to let my dad leave and kept my parents waiting for 4 hours in a room AFTER their orthopedic doctor had the paperwork for him to leave. They were rude to them. Definitely just wanted residents there to get more funds, but even after our surgeon put the order in for him to leave, they were adamant for him to stay another 7 days! Then they want to charge him for the days that he will not be there! The food is terrible. Do NOT bring your loved ones here. read more

This is the best skilled nursing and rehabilitation facility that I have been in thus far. The physical therapy and occupational therapy departments are especially good. Physical therapists, Sandra and Maria, provided encouragement and and insured that I perform exercises in a safe manner. Bethel and yadira work with me on strengthening exercises and were enthusiastic about my improvement. Phong was exceptional in his role as physical therapy leader. He made certain that each member of the team was on the same page regarding the goals I wished to meet. read more

My wife spent over three weeks there. The care was excellent, food was good, and nursing staff was outstanding. Other caregivers were very responsive to her needs. She was extremely ill when she entered there and I credit them with saving her life. The reason for a three star review is due to the office and administrative staff. I have never encountered a more rude, uncaring and dismissive staff as they have there. They make themselves unavailable for even the most basic questions and will not return phone calls. It was extremely frustrating to deal with them and even now they won't return calls and refuse to discuss final billing issues with me or the insurance company. read more

This company will not pay employees and not return a phone call. I was hired to work here, went in to do my first day of orientation, filled out a time sheet, had it signed by the ADON to approve the time and turned it in to HR. I was supposed to hear from them for my next orientation day and get my login information for the computer. I received over 60 emails from the company for all of my onboarding training, yet never received logins. I called HR multiple times yet never could talk to them. I always left a voicemail and then gave up trying to reach them. The woman who originally was HR that hired me was no longer there, it was the guy from a sister facility helping out. Then I reached out multiple times to receive pay for the hours worked that one day and guess what, they never call back. I left multiple messages for HR and then also the ADON in an attempt to get someone to call me back. My phone has never rang. On the advice of my attorney, we are filing a complaint with the US Department of Labor. All this to have to go through when all I wanted to do was provide nursing care for people. read more

This facility is fantastic! I just had a welcome conference call with all of the staff that will be taking care of a loved one. They each introduced themselves to me, gave me an update on her progress and then asked if I had any questions. I was a patient most of Jan 2023. rehab therapy was good. Food was terrIble and most often cold. Staff spends more time talking among selves than with patients. My cell phone was stolen from my room. Despite numerous promises of reimbursement, no payment has been received and no messages are taken. It is now July. VERY POOR! FWT read more

Unless you are close and can check in on your loved one daily, do not pick North Houston Transitional. While it is seemingly well designed and modern, it is dirty, understaffed and the only folks on staff are CNAs who provide a very minimal service if any. read more

Don't leave your loved one here if they need a lot of assistance. They simply will not receive it. They will receive roughly 20 min of rehab a day and after the 4th day the staff will call you and say your family has not made much progress. The cna's are understaffed and lazy. The nurses just push pills, they don't really pay attention when something is wrong. Jeremy, a cna, left my family member in the shower with the doors to her room and bathroom wide open and I am pissed about that. Just do home healthcare if you can. This place is not fit for anyone. read more

Do NOT use this facility. It is impossible to reach a nurse or administrator by phone. They do not return phone calls. You have to track them down in the facility. And they don't want you to do that - there are signs that say to remain in the patient room at all times and press the call button. But they don't respond to the call button. My husband was here for three weeks. He became septic while in this facility, and was very confused and out of it. I had to make them call an ambulance to take him to the ER. The ADON Melissa assured me they would be doing STAT labs to see what was going on with him and she would call me. They did not, and she still has not called me a week later. The food was served in the room in flimsy white styrofoam containers - COLD. It looked worse than the food I would feed my dogs. Dining room not in use because of COVID. Visiting hours are very restricted. When I couldn't reach anyone by phone and no one had called me back about the STAT labs I arrived at the facility to check on my husband. They would not let me go back because it was not visiting hours. I had to demand the charge nurse. The charge nurse knew nothing about the lab orders and had not seen him. She agreed to let me go back and when I got to his room he was so confused and out of it, he did not know me. I had to make them call for an ambulance. They did not want to transfer him out. He almost died from Sepsis. I never saw or spoke to the "doctor" that supposedly sees patients there. No one could ever tell me when the last time he saw the doctor. The nurses do not want to answer questions. The messages I left for the DON and the administration were never returned. No one has called to check on my husband since we left the facility. Do not fall for the hype. The facility is beautiful and has a good medicare rating - but don't fall for it. The rehab staff was great and my husband made good progress in strength while there - thank you to the rehab staff for doing their jobs! Unfortunately, we have lost that progress with him become septic and nearly dying while there. read more

This facility is hard to review as the nursing and therapy would be a 4 and 5. We especially felt Dan, the therapist was exceptIonal and communicated each week. That being said, the communication from here is terrible. It is hard to get in contact with anyone on the administrative side. The case manager, Ms. Scott , never called back until after multiple calls over multiple days. She didn't even visit my Mother when she was there, yet was doing her care coordination and discharge. After discharge , tried to reach billing and left multiple messages and calls were never returned until we reached out to a referred contact. When calls were returned the response was short and curt. It is a difficult time to have a loved one in a facility so one that communicates and is responsive would be help in this experience. read more

I arrived at North Houston Transitional Care after spending 4 weeks in a hospital bed recovering from surgery and other health issues. Over the past year I had also spent a lot of time in bed because of my health. By the time I arrived at NHTC, I was very weak physically with little muscle tone and mentally was not sure of my future. I could not function very well at all. I want to thank all the rehabilitation team here at the facility as they have brought me back from weakness and despair. I was assigned three therapist which I have worked with much of my stay but have also worked with most of the team during my time here. All the rehabilitation therapist have been very respectful and very encouraging throughout my recovery. My three main therapists were Jannelle, Asha, and Dan. Jannelle has been my occupational therapist. With her compassionate spirit she let me know each day that everything would be ok. I am not sure I really believed that at first, but over the time I have been here, she was right. I have continued to improve. Each day I work with her she encourages me to do more than the day before and I continue to get stronger physically and mentally. I would have to say she has been my mental lifesaver. I can also remember the day my right arm started to really sync up with the rest of my body. Thank for that Jannelle. Asha was assigned to me as my physical therapist when I arrived. Asha with a never-ending spirit of joy and energy has been wonderful to work with. I am not sure I would have come as far as I have physically with out her. She has pushed me to give it my all and I am happy to be sore at night as I know my body is getting better. I believe rehabilitation is something you do to get better, not leave the same way as you came, and I am seeing an improvement in the way my body is functioning. Recently I started working with Dan every day. Dan has had me doing things I had no idea I could do. One of those things being surfing on the parallel bars. Even on days I am not feeling well, Dan has not given up on me, but works on what I can do so I do not lose what I have gained. Thanks, Dan, for helping get me to where I am right now. There is just no quitting sense around here. All of the therapist are kind, encouraging, uplifting and I have never heard anything discouraging from any of them. Their goal seems to be to make you better from the difficulties which brought you here no matter how long it takes. There is just no giving up. I hope this does not get lost here at the bottom, but a big thank you to all the nurses and caregivers who have helped me on my journey back to the living. Facility itself is clean and nice. Front staff, nursing staff and therapist were pleasant and helpful, but head nurse wasn't very friendly or helpful. It was challenging to get a hold of the social worker and I had to corner her in the facility one day but once I did get her, she was at least helpful. It's been nearly impossible to get a hold of the billing person. We left multiple emails and message and have to chase them for weeks. When we do get a response, it's usually short and curt. Why do we have to chase someone to PAY a bill. It's ridiculous, especially given what families are having to go through during the difficult time. read more

This facility was highly recommended to us, but unfortunately, did not live up to the hype. There were some very exceptional employees, but overall, the attitude there is poor. Administration is hard to contact and does not return phone calls. We were left waiting for two hours for a meeting at one point to discuss how my mother was doing and when she was scheduled to come home. Almost like they wanted to keep her as long as possible to keep the money rolling in. She was also told to use the restroom in her Depends and they would change her later. Her therapy schedule was never consistent, and she has made much better progress in walking since coming home. I would recommend looking elsewhere. read more

I can not believe that this facility is able to get rid of patients with dificult diagnosis and treatments, instead of helping the families that trusted them with the care of their family member. If wrong doings, or mistreatments are reported by a family member, patients, are subjected to Managements retaliation, and eviction when all along the answer is to properly care for the patient. read more

My mother came here from another skilled nursing facility, Solera. We left there due to poor response time of staff among others. The good news first. New facility and clean. Therapy people are great. Asha and Janelle are top notch. Workout area is clean and spacious. Food is average but does offer choices. Better than previous facilities. Chef Roy is good. Nursing staff was really good. Kingsley is the best. Helped in all areas. Charles was good as was Tran. CNA staff was nice but not that good. Jovi, head of housekeeping is Great. My 93 yr old mother spent 2 weeks here for rehab after a fractured hip. The entire place was on lockdown due to Covid (my mother could not leave her room for the entire 2 weeks!). I certainly understand the need for Covid safety measures to be implemented, but I don't understand why her physical therapy only consisted of a couple laps of walking from the window to the door once a day (and I even think a few days were missed)....no strengthening exercises at all. I asked 3 times to have the therapist contact me and it never happened. I asked for her therapy records as she was leaving and was told I could not have them. Now that she is home with in-home PT, I am furious that the exercises she is doing now were not done while in this rehab facility. They could have easily been done in her room. She basically wasted the 2 weeks right after her surgery. The mattresses were very uncomfortable and the room was dull and depressing with Roku TV's that no one over 60 could possibly figure out how to use. I will say that the caregivers there were very nice and helpful and she really liked the food. read more

My Mom was transferred here from the hospital due to breaking her leg bad enough to need surgery. She couldn't get up right away, so she had an accident in the diaper. She sat in poop for 3 hours. She called on the call light and no one came for 3 hours to clean her up. The wound care nurse and lab came in & she told them she needed her diaper changed, but they didn't help her either. When she could get up by herself, She called for water, towels and a pull-up diaper and waited 3 hours to get them. I came to visit & she barely had any water. The only things this place has going for it is a nice looking building and Mom said physical therapy was helpful but was worried that she was not strong enough to get up by herself, but no one helps her so she has to get up by herself. I tried to call to address the neglect but no one answers the phone & the director of nursing's mail box is full. Hopefully Mom will be ok here. Iwould just take her home but she needs physical therapy and I have to work. I'm sad that she has stay here. read more

I would give them a lower rating, but I know for a fact that there are some really bad places in town and they are great, by comparison. My wife was in this facility for about 2-3 weeks. It seems like they need more staff. They did a good job, though. The thing that was the most difficult was communication. I left many messages and texts over the course of time and received calls from them twice. I don't know if they are understaffed, or if they are hiding, but their lack of communication made me distrust them. Much of what they told me was not true. I have to say that I don't trust them, the administration. Also, they had her on the wrong prescriptions, things she had reactions to that we had told them, but apparently no one noted. Some of her things came up missing, rings of no consequence, but still, they were missing. It took days for the mystery doctor to make changes. PT, OT, ST, and medical staff were good. read more

I would not recommend this place. My 79-year-old Dad was transferred here for skilled nursing, rehab, and wound care. The rooms are nice as well as the facility. My family and I take turns staying with my Dad because we didn't want to leave him alone. He has a large wound on his heel that has kept him off his feet so his body was deconditioned. He also was getting over pneumonia. He wasn't assessed until a couple of hours after we got there. The staff didn't come around much and when they did they had a scowl on their face. When we asked for things he needed, like wound care on his heel or a mask that fit for the BiPAP machine, we were told they didn't have time for that and would come back later. Never did. When my sister attended the evaluation meeting after about a week he was there, she asked about wound care and she was told they didn't know he had a wound on his foot. REALLY?? One of the wound care's staff is very rude. I was talking to her while I was on hold with my Dad's doctor because he's a diabetic and his sugar was over 250 for 3 days and needed insulin. They wouldn't give him an insulin shot and didn't bother calling his doctor. Anyway, I asked her a question and she didn't answer and i said, hello? She gave me a go to hell look and said I heard you! They lied to us about things we requested that he needed like a bipap machine and mask. It's sad that this place has the potential to be the best around but there's no way I would ever put my loved ones in there again. read more

I was told this facility is great but it's not. The staff in the evenings and weekends are careless. They don't clean the rooms. They leave trash, spilled items, feces, and body fluids on the floor. I got into the habit of checking my mom's bed after I once found that their staff had made the bed but left urine soaked bedding underneath. They were going have her sleep in that! One of the CNAs told my mom to just defecate on herself and they would just come change her later. My mom isn't bedridden. She just needed a little help walking into the batrhroom. What person wants to purposely sit in diahrrea? They offer laundry service but multiple staff members told me to never trust the staff to do laundry because items are guaranteed to come up missing. Meal times are mass chaos. They will easily forget to feed your loved one of you don't watch them. My current issue, the staff either stole some of my mother's belongings or they're careless and threw them away. Either way it's ridiculous and the facility hasn't reimbursed me. read more

Don't let the ratings on medicare and other review sites fool you. I researched extensively before sending my loved one to this SNF. I was relieved and grateful when my loved one was accepted. However, that relief soon diminished. If your loved one requires multi levels of therapy and Rx distribution, I would NOT recommend this place. The facility is beautiful and clean, food was good, admissions and the receptionists are great, and the physical therapists are good. However, the disappointment in care far overshadows the good. The distribution of medication was wrong, every single day! My loved one raised concerns daily, as well did I (when I was able to actually speak to someone). They make no consideration to medication that need to be given exclusive of any other medication, or medication that needs to be taken within a certain amount of time before eating. They just bring all the a.m. meds at one time, and p.m. meds much earlier than they need to be taken. The occupational therapist really needs to evaluate her technique and line of work. She said really ugly things to my loved one and walked out on her several times. The OT stated everything was in my loved one's head! She had absolutely no compassion. A PT overheard the OT berating my loved one and stepped in to tell her to back off. The nurses were hit and miss. There were a couple that were really good and nice, but they were overshadowed by the nurses who didn't seem to care at all. I left several messages for the various department heads and rarely received a call back. When I had the care plan meeting, the respective departments were very defensive when I raised my concerns. I was supposed to receive 1-2 calls a week with updates on progress, but only received one call the entire time. Again, left messages, no return calls. When it was time for discharge because insurance was up, I received the initial phone call, but all phone calls and text messages by myself and the facility my loved one was being transferred to went unanswered for over 24 hours. May have been coincidental, but my loved one was discharged on the day insurance expired, whereas the transfer could have occurred 2 days prior. All therapy was done in room, so no access to the gym...was told this was COVID protocol, but it is really a disservice to patients when other facilities have COVID protocols in place too, and still utilize their gyms for therapy needs. Was told equipment would be taken to the room, which was never the case. I am sure the facility is good if your loved one is there for assisted living, but not so much for skilled nursing. read more

My grandmother was placed here after hip surgery for rehab. She declined rapidly after surgery. The staff were seemingly either not concerned or not paying enough attention to realize she how bad she was declining. When we requested to speak to the Dr the staff gave us his phone number and told us that we could call him. The next day, after still not speaking to the Dr. we were told he would come that evening. He never showed up. At this point my grandmother was unresponsive, unable to eat, drink, speak, or squeeze my hand. We insisted that the staff call the Dr. who informed us that he never told anyone he was coming to the facility. After speaking to the Dr we agreed to have her transferred back into the hospital. She passed away less than 24 hours later. The lack of compassion and critical thinking displayed by the individuals caring for her was concerning and disheartening. After many stays at different rehabs for my 85 year old father who is finally well, I have to say this place BY FAR is a fabulous facility! It's beautiful, the nurses are great, the therapy department is top notch and from the beginning of our stay to the end, they did everything to make sure my dad was taken care of, he was happy and attended too! I cannot say enough wonderful things about this place. If I was ever in need of rehab again, I would 100% return here again. Highly recommend!! read more

My mother was admitted to North Houston Transitional Care on February 4, 2019. When she arrived at the facility her blood pressure medications were not available. I was informed that the medications would be delivered to the facility before midnight. After arriving at the facility the next morning I was informed that my mother did not receive her meds because the pharmacy failed to deliver them. They knew that the physicians at the hospital had a difficult time regulating her blood pressure. It was very important that she receive the medications as prescribed. During her visit, she was place in a room without a working heater. It was in the 30s during the time there was no working heater in her room. I requested that they move her to another room. She was supposed to be changed every two hours because she was a double amputee. Urine was dripping from her wheel chair on two occasions. She had an Aorta repair and received a Pacemaker and her incisions were not to get wet. On her fifth day in the facility, she was having difficulty breathing and needed to be checked for fluid in her lungs. They informed me that they had completed a chest X-Ray. I went to the facility and call 911 to have her transferred to the hospital where she died on February 28, 2019. After being admitted into Memorial Hermann Medical Center, it was determined by the emergency room staff that she had the start of a bed sore. I would not recommend this facility to anyone. read more
