Asked another company to come take a look at it, and they indicated that the roots are still good, and the grass will come back in the spring. Additionally they weren't concerned regarding the type of grass. They indicated that it was strange that they could still find traces for fertilization that was done months back. I think the company screwed up and won't take ownership and fix the issue. I'm just happy the grass will come back this spring.
