At the conclusion of the services rendered, my insurance company advised Mr. Muschik to issue an invoice/bill for the services so that they could issue a payment. He refused, stating that he wanted me to pay out pocket first because he has been "burned" in the past by prior clients and that he would not provide any of the documents the insurance company requested. He refuses to issue me any type of invoice or itemized bill until I send a payment? In what world will anyone pay for services before receiving a bill? He is extremely difficult, rude, and violating my rights as a consumer and will be dealt with accordingly.

My review was reported to Yelp and questioned whether or not I was criticizing against the practice's business ethics. I do stand fully behind my review as this is my experience with doing business with the practice. As per my review, I explicitly asked (and I say explicitly because I asked this multiple times on multiple occasions for three months) the office manager if I needed to complete paperwork as I was aware that the "terms of service are very clear" in their paperwork and was assured each time by the same person that I did not need to complete paperwork to cancel the reoccurring services as she would just process it in the computer. When I brought the unauthorized charges on my card to her attention, her response was "something must've not saved in the system," which was admission that the error was in fact on the fault of the practices. To date, the practice has failed to take responsibility of the error and Dr. Muschik has not shown an interest in resolving as there has been zero attempt to contact me personally to address the issue.
