The toilet seat slides off the bowl... then there are the two screaming men across the hall. It's terrifying for anyone to listen to that. I just want them to fix their things, and answer The call buttons. I know patients abuse it, but in this case he got hurt because of the neglect.

They constantly ignore the call button and blatantly ignore the residents. They're on their phones constantly and have such an attitude! It was so heartbreaking when I overheard one of the nurses tell a plus sized patient that she is disgusting because of her size. On top of that she refused to help her get dressed because she could "do it herself". She had then apologized but only because I had informed one of the managers. It's sad to see almost all the residents being treated like they're animals. Tell me, would you feel comfortable seeing so many residents being yelled at and talked about as if they're not in the room? As if they aren't a human being? They place has now changed ceos or top managers I'm not sure, and I can tell they want the place to change for the better but like I have stated before, the nurses there do not care about the residents. I tried my best to help residents in any way I could even though it wasn't my job. Most of the residents would not receive their correct food which was such a big issue because of food allergies and difficulties with solid food. There has been many times where patients wouldn't get their food at all! And the nurses would refuse to get them what they asked or get their correct food. There needs to be more activities for residents who cannot attend in person groups! It's sad when I would walk by rooms and patients would be staring out the window because they were tired of watching tv all day. I can tell the activity department only liked the residents who were able to attend the group in person. There is so many residents who would complain to me about not being included! Lastly, I advise if you do send your loved ones here to keep in contact with them to find out what goes on. There has been so many times where staff just like to do whatever they want when visitors aren't around.

I spoke to the Administrator about when I saw at least six staff members parking in front rather than around the back so there was no room for visitors, He was almost rude and pretty much stated it wasn't his problem. Food was less than stellar. They were supposed to take orders so you didn't have to eat the meal of the day, sometimes that happened and sometimes not. Way over salted but when my husband mentioned that he was told they didn't salt anything. Might want change who the food is sourced from. Beds were so uncomfortable according to my husband. Activities were not something most would want to participate in, It was better than some but there is still room for improvement and if the higher staff members don't see a problem then nothing changes

Upon arrival, she was served a meal consisting of a hoagie roll with two very thin slices of turkey, a piece of Kraft cheese cut in half, cold fries, and fruit that was unrecognizable. This raised immediate concerns about basic care and nutrition. The goal of short term rehabilitation is to help patients regain strength, improve their health, and return home to their normal activities. Unfortunately, our experience was the opposite. My mother became weaker with each passing day. Given her condition, it should have been immediately clear that she required assistance with basic tasks. However, it took until the following day for staff to recognize that she was unable to feed herself. When assistance was requested for toileting, a CNA stated in front of me that my mother could use her diaper and would be cleaned later. This appeared to be standard practice, as she was frequently left in bed until the afternoon and not regularly assisted to the restroom. As a result, her hygiene declined significantly. A physical therapist later expressed concern about her condition, noting that at 3:30 PM she was still in bed and had not been properly cared for. Communication throughout her stay was extremely poor. There were constant staff changes, and no clear continuity of care. Nursing and physical therapy appeared to operate in separate systems with little coordination. Despite multiple requests, we were never able to speak with a physician at any point during her stay. During this time, my mother developed pneumonia. We have since moved her to another facility. Within the first 30 minutes of her arrival at Orchard Park, she received more care and attention than she did during the entire two weeks at Avamere. I fear for patients who do not have an advocate, as they may be left with little to no care. Overall, the level of care, communication, and coordination fell well below expectations.

My husband was a recent patient at Rainier Rehab for a hip fracture…read more We had problems from the beginning getting the staff to move him safely, feed him a diabetic diet, administer his medications and weigh him at the proper time. Most of the staff were very accommodating. However, because the doctor and nurse practitioner were unwilling to adjust his medication when he needed it, my husband has been hospitalized for a week. He developed huge water blisters and sores on his legs as a result of their negligence. At one time, he was left on the toilet for so long and the nurse did not respond to the call light, so he had to get up himself. There was constant urine on the bathroom floor because the equipment was old and inadequate. Many times there was noone in the halls or at the nurses station through the day. Wait times for a response to a call button many times exceeded an hour. It is only pure luck that he did not fall again undertheir poor care. The facility is old and outdated. The phone didn't work properly due to being old and dropped so many times. I had to bring in many things from home because they provided a bed and old nightstand period. We chose this place because we were told it was a 5 star facility with a doctor and nurse practitioner 7days a week. There was no doctor or nurse practitioner on site on the weekends. Be very careful if you place a loved one here. The staff is friendly and seem caring, but when you need real medical help, the doctor and nurse practitioner make the decisions. In my husband's case, their negligence caused him a week in the hospital.

My mother was at Franke Tobey Jones, first in an apartment, then assisted living and finally the…read morenursing home part of the facility. She enjoyed going for her daily walks participating in classes. She developed a deep friendship with one of the other residents. Staff were kind to her. The beginning of the tragic turmoil began during her transfer to the HCC. She was shocked. She was excluded from having any part in decision making for the HCC. When I took her to see her new room she sat down and cried, "Please don't let them put me here". I told the nurse, but they went forth with placing her in the HCC against her will. FTJ has a history of not disclosing that the nursing home (HCC) is far more restrictive and that the resident may not need this level of medical care. In the age bracket of my mother, residents are transferred to nursing homes, in order to access nursing home insurance. Through rapid transfer, isolation, immobility and restricting communication, my mother lost her level of functioning rapidly into the parameters of the insurance. The transfer to HCC, was over $10,000 per month. My mother was sent to live in a constricted small room along a dark hallway. When she looked out her windows she saw parked cars right in front of her room. After placement in the HCC, FTJ could then rent out the beautiful assisted living unit she had lived in with views of Mt. Rainier over the fountain. One of the administrators informed me that residents are often transferred before they need that level of care. A direct care provider told me how residents are placed with no contact with friends or family for two weeks. When I came to visit and support my mother two days after her move, I was told to leave immediately or they would call the 'authorities'. I asked to speak to the CEO, who was a compassionate man. I was told he was not available and I must leave. There was no consideration for our mother's needs for support from the family. She later told me she thought she would never get to see me again in her lifetime. Another staff member told me how residents are transferred and cry for a few days. She went on to say that staff give them lots of attention and gradually they adapt. After transferring from assisted living to the HCC I observed that residents were no longer looking up and engaging with others, like they did before. From my perspective the move against my mother's will, isolation, restraint to wheelchair is 'kidnapping'. I never would have believed before that FTJ would do this to her. My mother grieved, with her loss of freedom, choice and respect. She had been a social activist for equal rights of minorities in Tacoma and president of the YWCA. She trusted the medical system, as a professional in the field herself. How ironic that a citizen who fought for equal rights would have these taken from her when she became an elder. My mother had been promised with the move that she would get to eat with her friends. Never again. She was no longer allowed to get up and walk on her own with her walker. She was placed in a transport wheelchair, with footrests. My mother was restrained through taking away her means of mobility, unplugging her telephone, not allowing her to eat or visit with her friends in assisted living. My mother told me, "I am imprisoned. I have no more rights. I cannot speak at a deep level. I must be superficial in my conversation or they will confine me even more." "When I die I want you to be free. To go where you are loved and never have to come back". The new Nursing Supervisor, Jenny Dooley, RN, had been my mother's Nursing Care Coordinator from Sound Options. She put further restrictions in place. My mother could no longer eat with anyone from outside FTJ staff, including healthcare professionals specializing in this field. My mother wanted to go out with her sister to eat and go with myself to her beach cabin. This right was withdrawn, even with accommodations including adaptive equipment and care providers. As a retired ARNP, who was a rehabilitation disability specialist, it was so hard to simply stand back and watch it all happen. FTJ lacked the knowledge and provided unsafe behaviors in eating and drinking, toileting, transfers, and working collaboratively with family. When the struggle impaired my health I had to give up on trying to support my mother's duration of life. I needed to adapt her quality of life needs to simply visitation in her room, and never again to go visit family or even go to her own home. I even needed to get an attorney to quote the Washington state laws in order to be with my mother as much as SHE wanted 24/7 for her last month of life. FTJ had requested no more than 2 hours a day, which was devastating for my mother. FTJ forgets how one's life expectancy is significantly reduced when opportunity to be with loved ones is restricted. My mother felt trapped and at FTJ's mercy until she passed. Then she was free.

When my sister, Pamela, and I could no longer safely support our 95-year-old mum due to her…read moredementia and 24/7 health care needs, we visited several holistic (active, residential, and memory care) assisted living communities in Federal Way. We selected Gencare Lifestyle (GCL) Federal Way, because of their reputation, focus on caring for the whole person (body, mind, and spirit), and the quality of the team! Our mum was initially reluctant to try GCL due to all the changes in her life (especially the death of her beloved daughter and my sister, Pamela), but the residents soon learned of my mum's loss and rallied around her. Mum is now in PEAK, the memory care unit of GCL, led by Director, Rachael Payne. Rachael is the consummate professional and dedicated to knowing everyone on her team and every resident in her care. Rather than use drugs to manage challenging residents more easily (as some other assisted living facilities sadly do), Rachael, Nicole Malchow (Assistant Director), and the PEAK nurses and staff, figure out how to meet each resident where they are, and then leverage creativity and the "patience of Job", to create a respectful and engaging environment so my mum and her fellow residents can enjoy a health span more equal to their life span. In addition to Rachael and Nicole, I'd also like to thank publicly the GCL PEAK and Assisted Living nurses and care professionals, Nicole Courville (Wellness Director), Jordy Schow (Culinary Director) and his team, Valinda Horstman (Community Director), Michele Christenson and Ariana Diaz (such welcoming Concierges), and Angelo Abela (an experienced hospitality GM who cares about creating an environment where the team and residents can flourish), for their love and service these last 12 months to our nearly 96-year-old mum.  It's been an incredibly challenging transition for several of our family members; GCL has been there for us! I decided to reach out to Caycare, Lisa Doyle, for guidance on Memory Care facilities in Federal Way. She provided me with a few adult family homes and a few assisted living facilities to look at. I looked at all of them and was really impressed with GenCare Lifestyle Steel Lake in Federal Way. I had a tour with Valinda Hostman who was very knowable and understanding of my situation and I told her I want nothing but the best for my father. Of all the homes and facilities, I took a tour of I chose GenCare Lifestyle of Federal Way. The Peak Memory Care facility is a secured facility. It has a couple of large common area verse very small common areas of other facilities. I couldn't put my father into a shoe box facility as I knew he needs a large space so he didn't feel confined. The outdoor space was much larger than the other facilities for the residents to go outside in a secured space to walk the path or sit in the sun. The rooms are shared but very large giving each resident plenty of space to call their own. The Peak Memory Care area is very clean, organized and well kept. There are many activities daily for the residents to participate in along with snacks throughout the day. I have had a couple meals with my father and the food is very good. The best thing that I like is the staff run by Rachael Payne, Director of Peak Memory Care, they are so caring and loving with all the residents. All the staff care about each resident and spend time each day with them. It's pretty amazing to see so much love from the staff towards the residents. It's like one big happy family there. They are very supportive with the family members knowing what they are going through. I couldn't have chosen a better place for my father, I feel he is in a very safe environment and well taken care of. The love there is just amazing. I'm so happy to have chosen GenCare Lifestyle of Federal Way for my father. In addition, I felt that the monthly room & care fee was fairly priced verse the other facilities. I would highly recommend GenCare Lifestyle of Federal Way Peak Memory Care facility for your loved one.
