    At the time of admission, her skin was clean and fully intact. Since then, she has developed three…read morepressure injuries, including one that appears to be unstageable. These wounds are serious and should have been preventable with appropriate care. We have also observed multiple care concerns that appear related to insufficient staffing within the facility: * She has been left sitting in soiled briefs for extended periods * She does not appear to be repositioned regularly * When family requests assistance, nurses state that only aides can provide that care, yet aides are not available or responding * Staff frequently appear overwhelmed and unable to meet residents' basic needs promptly I have attempted to address these issues internally by contacting Cat, the Assistant Director of Nursing, through both phone and email. The facility seemed clean, although there was a faint smell of urine in the hallways. She told me they were cleaning and the receptacles were in the hallway at the time. The room she showed me was remodeled. I sent over a list of required care items since my husband has Parkinson's disease and I wanted the nursing staff to have a heads up. I had asked if depends were supplied and she said yes. Upon arrival, no one greeted us or came out to help. He is hard for me to manage, but I got him in his wheelchair and took him up to his room myself. I then had to go back down and unload the car, using a cart that was in the lobby. His room was outdated and shabby. The dresser drawer was broken off the track. The shelving in the closet and bathroom were sticky. The floor was dirty and the room stunk. I went down to talk to the admin to relay this info. I had a flight booked for a family occasion and had to leave. The first time he needed to be changed, he said the nurse was not happy because he did not have his own depends. I had his daughter bring some, plus another pillow and better blanket, as well as some room spray. He was admitted on a Tuesday and by Sunday night had vomited several times, was running a fever, and couldn't breathe. He had no clean clothes left (I brought 11 changes of cloths, but he is totally incontinent. I was told laundry service was supplied, but that never happened until I complained) Thankfully, his daughter was there and she called me. I told them to call an ambulance ASAP. He is in the hospital with aspirational pneumonia. This place is totally negligent, and I feel that my husband was totally ignored. Upon talking to the floor nurse, she claimed she was not given the list of care needs. The admin acknowledged receipt of the list. I'm beating myself up over this decision but these people are deceptive. I would give 0 stars.
