I recommend this skilled nursing facility very highly. While my husband was very sick, everyone here (staff & administration) took exceptional care of him. Nurses, CNAs, doctors, therapists & administrative staff are professionals who care about their patients. All employees work together to help the patient & their families. This place needs a lot of changes and upgrade. CNAs here are lazy....especially the 2nd and 3rd shifts. Most of them have two different jobs. They're there just to sleep. They don't change their patients. You can come in the morning to found your loved ones wet through the mattress even with water resistance pad. There nurse supervisor doesn't want to take any responsibility. You report to them all you want. They don't listen nor take action. They constant changing staff....They bring in unprofessional and inexperience staff....really....it's speechless...Especially in healthcare...you feel helpless even those you know they get money running the place with bad care all around but you don't know where and how to report them. I have been visiting my family member in tgus facility and this is the first time i have encountered such disrespectful and inpatient staff. An aide named Kimberly was walking behind my family who btw is using a walker and was rolling her eyes impatiently because she wanted to pass through us. If you want to pass all you have to do is say excuse me. My family again requested to walk and we didn't know there was a code going on. When my family passed by again this Kimberly was making faces but now she was doing hand gestures raising her hand in disbelief why family was there again and take note i was behind my family. Great facility -- very clean, with spotless floors and no smell of urine. Recently remodeled with new artwork and wallpaper, it feels welcoming. But what really makes the difference is the staff. They truly care, and many nurses and CNAs speak Cantonese and Mandarin, which made my family feel at home. Highly recommend read more

When my aunt first came here, I didn't think too much about nursing homes other than its cleanliness. The social workers, nurses, and food servers here, I learned, are just as important. My aunt has been here for almost a couple of years already. I thank the good Lord that her health here has improved. When she first came, I felt that things such as her health could or would get worse at any time. In a way, it's true because most elders living here are at their twilight. I could only pray for God's perfect will upon them. Now, I honestly feel that nursing homes can be a good thing. The entire staff here are professional. The nurses are patient and nurturing to their patients. I see that throughout the years of either working for the facility or living there, friendships and bonds have been developed between the staff and their patients. They all seem happy most of the time whenever I go visit. My aunt knows almost every staff member their! Amazingly, they enjoy talking to each other. Anyhow, the facility is very clean and well organized, as it is always busy with some event inside. My grandma is a resident here and this place provides adequate, safe care for her. There are occasional issues like leaking rooms from the rain or COVID outbreaks but I'm happy to think that they try their best to minimize risk and fix the problems asap. They also have nurses who speak multiple languages which is helpful since my grandma does not speak English. We do still need to be diligent as a family and speak up about things but in general things get addressed in a timely manner. read more

From day one, my sister and I dreaded having to look for a place for a skilled nursing facility for our mom. We spoke to Fay; she invited us to tour the facility. She walked us around, took us into the rehab gym and dining rooms, and looked at the bedrooms. Fay assured us that Heritage Manor was not what we thought it was (previous bad experience with our father from another nursing home), so we decided to put our mom in Heritage Manor. It was the best decision we could have made. You could see the change in our mom immediately; she started to thrive. She was shown compassion, respect, and love. I visit my grandmother everyday for more than 3 hrs. Including my mother and my sister. We bring home cooked food. We have acknowledged their facility and staff. The quality of care lacks for their patients. My grandmother called for nurses help in the middle of the night stating shortness of breath they took close to an hr to care for her. The nurses gave her CPR did not inform my family or her Doctor. I am THE EMERGENCY CONTACT! I will take for consideration they have a lot of patients. My grandmother is sane and alert and aware of her settings. There is shady shit going on here! The final night my grandmother stayed there. It took for my sister and i to take it into our own matters. My grandmother was panting and shown shortness of breath. The Director of Nursing(supervisor) arranged a paramedic on the way but said it would take an hr to come. The time they arranged it 8:10PM. The car they called would be 9:30PM. She could died!!. Just because they are calm. They didnt have the sense of urgency. So we demanded they call 911. They didnt even take her vitals right. They half fast the job until the paramedics came, heritage took it serious (so they dont look bad). The therapy they promised was not promised. They put the patients feet hanging off the bed for a few minutes and laid them back down on the bed. The 7 days she was there, i took better care for my grandmother than they did. Especially changing diapers as i watched them do it.. i can still wipe and the wipe was still yellow. read more

It is very difficult for me to trust any care facility when it comes to providing care for my loved ones. I did a lot of research hoping to find a good nursing home for my 90 years old grandma and I was grateful that Heritage Manor met my expectation. I met with the admission coordinator Fay and right away she made me feel so comfortable and was very reassuring that grandma will be in good hands. She was very sweet, soft spoken, and she answered all my questions and concerns without the feeling of being rush. Fay took care of all the paper work and made the admission process for grandma so smoothly which saved me a lot of time. During the facility tour, I was able to observed the staffs and they were very attentive to their patients and genuinely showed compassion in their care treatment just by the way they spoke to the patients. I also like the fact that most of the staffs were able to speak to grandma in her own language (Mandarin). The facility was clean, no odor as you entered the building, rooms were decent in size. Activity areas very spacious. Reception area were tidy and well organized. Grandma had an issue with getting her medical equipment and I was not able to resolve this matter. Fay referred me to their social worker Michelle to help with the matter. Michelle was very supportive and immediately made some phone calls and grandma was able to get her medical device the next day! I will definitely continue to refer my family and friends to choose Heritage Manor as a care facility for their loved ones. Grandma still talks about Heritage Manor and even asked if she can go back again as she has made friendship with many other patients in the facility during her stay. This place is a place I would not wish on my worse enemy. My mom was transferred here from a hospital after the doctor there insisted she needed it. My mom was completely mobile and was covid positive. I couldn't see her at the hospital because I wasn't allowed to due to her having covid. People started blowing up my phone with calls telling me she had to be sent here. My mom was lucid and agreed. I told her it would just be for a few nights. Kevin from Heritage called me and told me all about the place and how wonderful it was and how I could do a tour before her transfer which was happening that evening. Basically giving me no time to check it out. I checked out their website and they have an amazing web designer because it looked like club med. I got to Heritage Manor in the evening and she hadn't been transferred yet. I walked in (no security) and no one even noticed I was there. Finally some lady yelled at me in Chinese which I barely understood five words. My mom arrived with an ambulance and was put into a room by herself. That room was filthy and didn't even have sheets on the bed. When I asked for sheets they seemed annoyed. I killed a bug (should have taken a picture). I wanted to take my mom home and not leave her there but it was so late she couldn't be discharged. My poor mother was promised a shower (but they ran out of hot water). Then she couldn't sleep all night. At 5am someone barged into her room to use the spare bed to fold linens. My mom had covid... why would you fold sheets in a room with a patient with covid! Needless to say we got my mom out the next day. This place is not for someone you love... not even someone you hate. I wish there were organizations checking in on these places... they need to be audited frequently. Avoid Heritage Manor like the plague! read more

My grandma was transferred here around 2 weeks ago because when I was looking at the options between the choices the hospital gave us in a days notice, this one had the highest recent reviews. I regret sending her here now because this place is not great. They moved my grandma twice within the last 2 weeks and lost all her belongings, which include her upper and lower dentures, glasses, and the clothes and shoes we brought which were clearly labeled with her name for her plus the ones she came in from the hospital. When I found out about it, no one seemed to know where it went or took responsibility for losing it. They didn't give us any updates about if it was found, and we kept having to ask about it. None of the workers could locate it, and they keep saying to check with Michelle, the director of social services. When I met with Michelle, she said she didn't know anything about this, and also told me she doesn't keep anyone's stuff, but will try to look for it. I told her that it's pretty irresponsible of them to lose everything and that they needed to take responsibility to replace everything. I heard from one of the CNAs that another patient here had all his/ her belongings lost, too. Most of the nurses and workers here are rude and irresponsible, nor do they want to own up to their own mistakes. Parking for this place is horrible too. read more

My mother was a patient and contracted scabies at this facility. She did not have this condition when she arrived. She is still trying to heal from this and is suffering miserably. I don't write lots of reviews, but when I do it is because of something/someone outstanding. My mother was admitted to Heritage Manor for physical therapy. From Faye in admission, Michelle, in social Service and countless unnamed nurses,therapists, aides and all support staff must have been sent from heaven. My mother tends to be a little difficult, but they have promptly responded to all her requests. My grandma was a long term care patient since 2019. We want to express our gratitude and appreciation to the whole staff. They have a great team of staff, nurses, supervisors, and activity coordinators. Their team treats patients with respect, care and love. The care, attention, and love they gave to my grandma in the past 5 years made her feel comfortable and like home; especially Luz Cristobal and Dee. Both of them took care of my grandma in different rooms. IT SHOULDN'T BE LEGAL FOR HERITAGE MANOR TO RUN. My grandmother is back in the hospital now due to their negligence, WHICH THE HOSPITAL DOCTORS HAVE CONFIRMED. PLEASE trust the negative reviews on here if you want to save your loved one's life. We had to admit our grandmother into Heritage Manor because it was the only rehab center with any bed available for the month. There's a reason why nobody wants to come here. Yes this place is understaffed. One nurse per floor. Anyone else you see in scrubs is useless, literally knows NOTHING, and just tells you to ask someone else. Pressing the red button for the nurse does nothing. My grandmother fell in the bathroom 3x over the weekend. The weekend nurses are more useless than the weekday ones. She had pressed the button to signal for a nurse to help her when she was still in bed. No nurse, so she slowly made her own way to the toilet. When she fell, she pulled the string for the nurse. She was on the ground for ONE HOUR before a nurse finally came. The nurses had the audacity to blame her fall on her slippers. When in actuality, it was because the nurses don't communicate during shift changes, which resulted in too much blood pressure meds being given to my grandmother, leading to dizziness. Once they realized the wrong meds were being given and cut back, my grandmother was back to her normal independent self. Yet the nurses were adamant on blaming her fall on her slippers. My grandmother had a low blood sugar of 40 during the night. She had no juice or snacks in her room, so she pressed the red button for the nurse repeatedly. Of course, it took a nurse 45 minutes to realize. My grandmother sat in her bed shaking for 45 minutes because her blood sugar was so low. I can't last 5 minutes with a low blood sugar and I'm in my 20s, I can't imagine what she went through that night. These medical staff are so negligent that they write in patient charts without actually doing what was written. On one occasion, I stayed overnight with my sisters and we were waiting for the nurse to give my grandmother her insulin. Nurse never came even though we kept asking. The next morning, the nurse said my grandmother got her insulin and blamed it on my grandmother being old and forgetful. We told them that we had been there the whole day and night and never saw a nurse give the long-acting nighttime insulin. Nurse said it was written in the chart that it was given. My grandmother's blood sugar was high that day. BECAUSE NO INSULIN WAS GIVEN. On the first day of admittance, we were told there were translators on hand over the phone. I was also told they would call me every time they needed to tell/ask my grandmother something, no matter the time. I found out later that the translators are only available for 5 hours a day. Even so, the nurses are too lazy to pick the phone up and call for a translator. My grandmother once used her cell phone to call me so I could translate to the nurse. A nurse named MS. Hernandez hadn't given her meds that morning, and it was already 1pm. When my grandmother handed the phone to her, Ms. Hernandez swatted the phone away and refused to listen to what my grandmother wanted to say to her. Ms. Hernandez finally brought my grandmother her medication after I told her to continuously press the button. When my grandmother motioned to Ms. Charles asking if she should do two puffs with her inhaler, Ms. Charles SCOFFED AND ROLLED HER EYES AT HER, WAVED HER HAND AT HER, AND LEFT THE ROOM. As if none of this was enough to deter you from steering clear from this horrible place, items went missing when there would be someone coming to change her sheets. Sheets were only changed on Monday mornings, brand new unopened ointments and lotions we brought in for our grandmother disappeared on Mondays. Her phone also disappeared. All the other days, nothing was touched. Avoid this place. Doctors are paid in cash to send their clients here. read more

I want to give a heartfelt shout-out to the staff at Heritage Manor in Monterey Park! My Mom stayed at the Heritage Manor recently after being discharged from a hospital for a serious infection that was unrelated to Covid-19. She's 93 years old and, being very independent, she insists on living by herself. Following her discharge, her doctor recommended Heritage Manor so that she can receive around-the-clock care. She was frail, discouraged, and could hardly walk without assurance. When I went to Heritage Manor yesterday to pick her up following her discharge, she appeared strong, high spirited, and was walking on her own! From the moment my Mom checked into Heritage Manor, I noticed how dedicated the staff at Heritage Manor is. Michelle (not sure of her last time), the social worker, was my first point of contact at the facility. Her professionalism and caring nature assured me that my Mom is in good hands and that I did not have to worry about Mom. Since there's no visitation due to Covid-19, I could only call or FaceTime with Mom. Each time I called, I felt the love and care of each staff member whether it is the receptionist or the nurse. My younger daughter is about to become a LVN soon. As a former US Army helicopter medic, I want to salute all the health care professionals. You are our essential friends and our heroes. Heritage Manor is a great facility that offers nursing care and rehabilitation services (physical, occupational, and speech therapy). I rarely write yelp reviews due to my busy schedule, but this facility deserves a five-star review. I've been there quite a few times and I've never smelled the usual "nursing home" odor. During Covid, this was an awful experience. I attribute my mothers death directly to how she was treated and ignored at this place. After a hip replacement January 19th, they said she needed to be discharged to a sub-acute facility (just three days later). It's what they do with medicare patients. The social worker gave me a list of of recommended partners and Heritage Manor was first on the list. Since I'm being pushed out the door, I picked the one closest to my home. I've had experience with these places as my father in law was in one 5 years ago. It takes constant family attention as these places are often understaffed. But during covid, family wasn't allowed. My mother, who had early onset dementia, needed more care then they were capable of providing. Once she got there I was told that I wasn't allowed in due to covid. I didn't realize this would be the case and feared what was to come but it was too late for me to do anything at this point. I called her everyday, twice a day, to speak to her. However, two weeks in, she  seemed off. Very tired and confused. Said her hip was hurting. She had not complained of pain since the initial surgery so I became concerned. It was a Saturday so, of course, there were minimal staff and no doctor there but I told the nurse that something was wrong and she told me that my mother seemed "alright" to her. That night, the manager (Marie or Maria) called to say my mother fell in the bathroom after being left alone. When I asked why she was left alone Marie said she told my mother that she was not supposed to get up until the nurse came back. I reminded Marie that my mother had dementia and likely didn't understand and also and that I noticed she was off all day. Marie simply told me to have a good evening, very dismissively. I repeated my concern and Marie, again, told me to have a good evening. Now... imagine being left with that note from the manager of a nursing facility that was supposed to be caring for your mother... The next day I wrote her care supervisor and said I thought something was wrong and that my mother should go back to the hospital. That I didn't believe the facility was capable of dealing with her medical needs. I wrote that I understood Covid has made things difficult and they were likely understaffed but wanted her back in the hospital as it seemed she was declining. This was Sunday so, of course, I wouldn't get a response until Monday. Rather than send her back, they said they would have a doctor look at her. So Tuesday they took x-rays and said it would take a couple of days to get them back because they had to be sent out. My mother was still sounding weak and confused so I called to ask about transferring her back but no one seemed to think my concerns were valid enough to put a rush on it. Fast forward to Friday, I get a call that my mother was going to the ER because her an issue with her "wound" (this was the incision from the surgery). Turned out she had a horrible, oozing infection. The ER staff was horrified by what they saw and asked me how long she had been like this. I explained she came from this hole of a rehab facility and that I had been trying to get them to send her back to the hospital for several days because she seemed off. The ER staff concurred that this had to have been festering for days. The surgeon concurred (at the time but when asked to put it in writing he refused to do so). While a lot of terrible decisions were made on the part of Heritage Manor, in terms of pushing her out the door, the worst was recommending. This woman needed more attention to her medical needs and this facility dropped the ball. They were not nearly capable of doing the job. They dismissed my concerns and request to send her back to the hospital for nearly a week before she ended up going to the ER anyway. The manager, Marie, should be fired an not allowed to work in another facility like this again. Frankly, it should be shut down. NYU should be ashamed for recommending them to family to begin with, especially when pushing an elderly patient with multiple medical needs out the door right after a surgery like this. read more

My husband's grandmother has been staying at Heritage Manor for the past year. The facility is clean whenever I come to visit and staff has always been very professional. It's also convenient to get to, right off Garfield exit off the 10 fwy and parking has never been a issue. Garfield Hospital is right across the street, so if anything happens emergency help is right there. The last time we took his grandmother out for her birthday party, the staff was really helpful with accommodating a wheelchair for her to use while we took her out for lunch. His aunts and uncles have also mentioned that the staff have always been courteous and sweet. So, overall a pretty good facility. read more

My grandmother was a resident here. In the beginning, we didn't know what to expect, it was the first time that any of our family member had to stay in a nursing home, so we were all very nervous. The staff and nurses took all those nerves away. They were all very friendly and welcoming. Especially, their Director of Business Development, Jennifer! She always went out of her way to come in and greet my grandmother and make her feel comfortable, especially during those days when she was most anxious. My mom was recently transferred to this facility for rehab since she had hip replacement surgery. I was very skeptical at first cause i heard so many horror stories about these convalescent home/rehabilitation center. But after seeing how caring the staff was from the nurses to the therapist and a special shout out to Michelle the social worker. She is very professional and knowledgeable in her job. The only complaint i have is the food. I made it very clear to them that my mother is a Buddhist and does not eat beef and time and again they still serve beef to her. But overall this is a very good facility and I am thankful to your staff in helping my mom in her recovery. read more

my grandmother lives in this facility and she really enjoys it. she's 102yrs old and needs help with everything so we really trust the staff will take good care of her. The nurses are so attentive and caring. It also helps that most of the staff speaks Chinese so they can attend to her when she needs something. The place is always clean and free from oder. Just want to give an update since reviewed last Feb, 2017. The place is amazing and had been remodeled since. The staffs continue to provide excellent care for my mother in law. She seems to be healthier because of the excellent care provided by Luz, CNA and other nurses that care for her. The front office staff is also very helpful answering any questions we have.
