During the entire year that Right Choice provided services, not a single supervisor ever came to home to observe caregivers, ensure they were providing appropriate care, or coordinate with his Personal Assistance (PA) and Independent Living Skills (ILS) providers. Not once. Because there was virtually no supervision, we made attempts to operationalize a routine for my brother with the caregivers. A checklist of things that should be done to create structure and routine. Communication from RC was none existent. Calls were not returned, although Leslie was responsive and did her best to help. Right Choice failed to provide the supervision, accountability, and quality of care expected. Regional Center came to rescue and got us an another vendor. Our mistake as newbies is ask questions before entering agreement for piece of mind.

When my father was hospitalized for a blood infection in December of last year, part of his release required at home care. A daunting task to say the least for any child of an elderly parent, especially around the holidays. It was relayed to the company that my father has bouts of confusion, a high risk of falling, issues with using the restroom/showering on his own, cooking meals, and driving.... Due to this, the cost was (in my opinion) extremely high.... But what choice did we have on such short notice? The company had a minimum hours per day of 6 and a minimum of 4 days per week at $37 an hour. Which is much more than we thought we needed. They required us to sign a 3 week contract. Short on time and options before his release, we signed the contract. The first day, they sent a care giver who had no car, and barely spoke English... when I got home from work, I was horrified to hear that my father had not eaten that day, did not receive his medication, and was unable to pick up his prescriptions from the pharmacy due to the care giver only having a bus pass as transportation... I called 1heart who said they would "send someone new tomorrow". They did not offer a reasonable apology, a refund/credit, or even a discount for that day... nothing. Just a "we'll send someone new tomorrow". The next day, they sent a new caregiver, who showed up on foot.... I was blown away. I spoke with the caregiver over the phone who stated that she only lived a mile away and chose to walk due to the company not communicating to her the need for my dad to have transportation. She actually was a very sweet lady and walked home to get her car to take my father to the pharmacy. Two hours later, I get a call from her stating that my dad had fallen at the local CVS. She was unaware of his fall risk due to the company not communicating my fathers care needs. She did not have his walker with them due to her car being a two seater... Again, I was extremely upset by this. All of this was explicitly communicated to 1heart before signing the contract. A week later, he was taken to the urgent care and given an x-ray that showed multiple broken ribs from the fall. What did 1heart offer for this when questioned? Not an apology (certainly not a discount), but a handwritten "incident report" on a lined sheet of paper from the care giver explaining the fall.... Zero concern or accountability. The bottom line is, 1heart does not provide quality, affordable, compassionate, or reliable anything... choose another company/agency.

The staff at this facility specifically told me to email my referral order through - victorsantosptfax@gmail.com I sent my referral order containing all the relevant information about the injury. I called a second time asking if the information that I sent was good for my appointment, the front-desk lady on the phone said it is okay so I showed up to the appointment. Upon my arrival at the facility. I said I have booked an appointment and emailed my referral order which was confirmed prior to the appointment date via phone call. The lady on the front-desk asked again if I can re-send the referral order at 10:51am (appt was 11am). I filled out the sheets and 10mins later the lady said that it will not work and would need a referral order note OR authorization. I showed them the paper that they printed out and in BIG FONTS at the top, it said REFERRAL ORDER. She then proceed to say that we can call your insurance and you may need to pay some co-pay -- I said its fine. A few minutes later she said we need authorization. This could have been avoided if they did their job thoroughly and checked all information rather than blindly telling a new patient that it is okay.

The physical therapy team is truly top-notch. They were patient, encouraging, and tailored a program that dramatically improved her mobility. The nurses and CNAs are equally wonderful--they were compassionate, responsive to her needs, and treated her with great dignity. The only major drawback during her stay was the quality of the meals. The food was often unappetizing, bland, cold to bad. While the medical care was five-star, the dining experience could use significant improvement. Overall, I highly recommend Sherman Oaks Rehab for its excellent rehabilitation services and dedicated staff. If you have a loved one staying here, I highly recommend bringing in outside food or snacks to supplement the meals.

This facility has 1 star out of a possible 5 stars on medicare.gov which is a "much below average"…read morequality rating by Medicare standards. It is owned by Longwood Management, a corporation with 38 rehabs in California. Of those 38, 13 have 1 star Medicare ratings and another 12 have 2 stars. 66% of their facilities are rated by Medicare as substandard, due primarily to serious health department violations. Longwood Management operates this way intentionally. Poor care improves their profit margins. When the health department heat becomes too much at one facility, they move dangerous staff to another of their properties. That's happening now at this facility. Patients and their families - beware. And add the California Department of Public Health to your contacts, because you'll be opening investigations into this place frequently. Know that the allegations against Longwood are most often substantiated, thus the 1 star rating with Medicare.

The nurses and health aids were wonderful, helping him with showers, tending to his wounds, and showed up when it truly mattered most. Once we told them that we would be switching companies, they harassed my brother with several calls blaming all there mis-doings on everything else (the nursing facility, my mother, a nurse being sick, etc) but we were with them for a few weeks and had seen their lack of care. Almost immediately, the hospice nurse went to my mother's hospital room asking her to call us and tell us how happy she is with Inner joy and to undo the paperwork with the new company. After their attempts failed, they woke up my mother at 5am to remove her bed, with no warning to the facility or the new hospice company. Leaving my not mobile mother stuck in a wheelchair while everyone scrambled to find her another bed. They are not concerned with their patients well being. Now that we have a new company, we're really seeing how bad their service really was. Please get your loved ones a better hospice company. Zero stars
