Physical therapy was adequate, according to my friend, a rehab patient. But. Run down building and property. Poor outside lighting in a parking lot. Poorly furnished rooms, with torn chairs and next to no furniture. Terrible bedding. Cafeteria like dining room with institutional type food. No bathroom in the room; only a toilet and sink with a curtain to pull for privacy. My friend recovering my serious surgery, was not helped to shower for the first two weeks he was there. No shower for two weeks! I would never recommend this facility to anyone.

God, please don't let this be a bad experience for our mother, and we pray that you will take control of the process going forward. The Patients couldnt of been in worst shape...people with missing limbs and have compketely lost their minds. there was no support or love or anything for that matter from ANY of the staff. i have never been so horrified in my life (worse that the WORST horror movie you ever saw). i know this is a state line facility and i shouldn't have anticipated much, but wow! is this really how our country takes care of us at the end?! the place is so bad, i would say people in prison are getting a better deal or maybe even a dog kennel. i wouldnt even bring my dog there to WALK AROUND! it really makes you think about the things you want for your loved ones as they reach a ceartin point. its a VERY harsh reality. so people...work hard!, take care of yoir loved ones...MAKE SURE THEY NEVER END UP HERE!!!! ITS HELL ON EARTH!

She was legally blind so she couldn't read or use her phone for the internet. I live in San Francisco. I sent her a tablet hoping she could see the large print but she died before receiving it. Her last days were in a room with no TV, visitors, no one to talk with and in isolation. I realize how difficult this is during Covid. I don't blame the staff, but the Lodge must be seriously lacking in funds if it can't provide TV's for the short stay SNF. Fortunately my sister could use the phone and we spoke daily. But thinking of her last days in such isolation haunts me.

The patient's room doesn't look clean especially the floor. There isn't any clock on the wall and the staff pretends they don't know how to work the remote. The bathroom is a shared bathroom so someone can actually come in while you are in there. There isn't any lock to lock the bathroom while you are in there. The staff does not give the patients real care like a hospital does. Do not put your loved ones in here not even for rehab.

Hello, my name is Jimmie Mitchell a current resident at Fairview Nursing Home second floor rm#226A…read more I am posting this review to tell exactly how horrible it is living here..it has been in the 90's weather wise, and my rooms thermostat is at 79-81 constantly, the air condition system works when it wants to. My roommate and i have no working landline phone and have been without one for a year. The call bell only works if i literally take the cord out from the wall, even then only the light up top the door lights up, no one would be aware of the call bell light because it doesn't buzz at nursing station ..they allowed an employee yesterday that was unknown to any residents to take swabs on their gums clamming it was for a COVID test, which no resident was made aware of nor signed for.. upon information and belief i but not limited to believes that the administrator but not limited to and other staff members and employees are purposely misleading and manipulating residents and now are going above and beyond the calls of duty and are starting procedures to evict and displaced residents who really don't have any safe and secured places to go, clamming money being the primary cause and reasoning for doing so. Mean while the quality of living suffers, dilapidated building, subpar medical aid, and to be constantly reminded that Fairview Nursing Home controls ever aspect of what little life we have left....If you truly love your kin folk and loved ones PLEASE DO NOT PUT THEM IN FAIRVIEW NURSING HOME.

I don't like talking about the attitude of the staff because any one who can work all day with that smell has to be a certain kind of person anyway. On my first visit, I put air fresheners and plug ins in my mothers room because I couldn't stand the smell. Walking to my mother's room, the first thing that I noticed was that more than half of the call lights on the rooms (including my mothers) were on, and remained on for the duration of my half hour visit. The residents look absolutely miserable. My mother was only in there for short term rehab, and I had to get her out of there! I wouldn't send my pet to that facility. The staff (most of them) are not friendly and just seemed uninterested in the care that they are providing. When you call, you have to go through several "floors' before you can speak to the practioners on the floor of your loved ones. For a few days, I was told that my mom's clothes were 'lost' in the laundry room. I started looking for them, and they were in the trash can in her room! When I explained it to the CNA, I was told that it wasn't a trash can, it was where they put dirty clothes. However, when I opened the can, it had trash in it! Along with my mom's soiled clothes that smelled SO BADLY!!! I could go on, but the bottom line is DO NOT SEND YOUR LOVED ONES THERE!!!

Absolutely the laziest staff I've delt with over 20 yrs in LTC…read more 1. Morning consist of many staff being late to work. 2. Next you have some eating their breakfast at the Nursing desk. 3. If someone does call off, you'll be able to hear staff complain most of the day. Then have the gall to inform you their short "today" and unable to get many patients out of the bed. 4. You'll hear a seasoned nurse say, "out loud" and proud "what they won't be doing to assist the staff. 5. Good Luck getting call bells answered in a timely manner. You would pee yourself or worse. I've watched someone come in turn off the light and not ask a thing. 5. Dragging full bags of linen or diapers and NOT closed up, smelling up the entire hall. 6. Curtains on windows sagging or missing hooks. Or the draw curtain between beds with food or god knows that what. 7. You will attempt to take your loved one to the room toilet and find dirty diapers or cloths hanging on the towel rack or in a corner. 8. Most residents are sitting around in hospital gowns or some with cloths so big and their pants aren't pulled up over their behind. 9. You will find patients sitting in the dining room for a couple of shifts and never moved or seen by someone from shift to shift. 10. If you're ever in the need to put a loved one in Rehab or LTC such as these, the only way to make sure your loved one is cared for is to be there daily. 11. Oddest thing is I've never seen mangmt. on the floors, rounding or a walk through. You'll find them in the conference room most of the day. Most floors don't have a supervisor. So, who's watching them actually do their jobs. 12. The Administrator! He'll tell you anything to appease you. And over time You'll get sick and tired of telling him what's what. Apparently, he's not on the flr doing a walk through from time to time to keep everyone on their toes. I feel so bad for many of the patients here, it is apparent they don't have someone coming to see them with any regularity. God Bless them and keep them. Having a loved one here was very troubling to say the least. I've heard that some think the LTS is over regulated. The thing is who's making sure they actually stay on top of all this drama? Really need realiable, caring, empathtic staff. You get a few here and there and in 3-6 months they will be gone. This requires better staffing levels and people actually doing their jobs. "The mice will play when the cats away" It has been said. The ONLY reason I leave 1 star because they don't have a negative rating system. If they haven't been sued it's around the corner.

There have also been frequent scheduling and payroll errors and a workplace culture described to me by multiple staff as retaliatory and hostile, which I believe is affecting resident care. I recommend families verify current inspection reports, ask about staffing levels and infection‑control procedures, and contact the Pennsylvania Dept. of Health or the Long‑Term Care Ombudsman before considering placement.

Another night he asked for a pillow under his knees and was told, "I'll see if I can find one", and the aide never returned. She also dropped the call button on the floor so he could not reach it and call for help. He was in pain all night until someone came in at 5:00 a.m. and got him a pillow. He developed a bed sore that they originally identified as such, but then tried to cover it up by saying it was "an abrasion". He was promised at my suggestion, to walk 2x/day, but most of the time that did not happen. One Sunday an aide walked w/him in the a.m., but when he asked again in the afternoon, he was told "I walked with you this morning. I'm too busy now and can't do it."
