IF I could give this place zero starts I would. II am extremely disappointed with the experience my parent had at Fulton Commons. From the moment we arrived, the condition of the room was alarming. The bedside nighttable had a white film on it that looked like dried milk. I ended up cleaning it myself with hand sanitizer and paper towels because I didn't want to place any personal items on a surface that looked so disgusting. The wall beside the bed was filthy, covered in dried blood and other stains (see attached picture). The staff was another major issue. The workers at the front desk were rude and acted like you were bothering them whenever you asked a question. If they even bothered to answer, it was with an attitude. On the positive side, the nurses, physical / speech and recreational therapists were nice and professional, but unfortunately, they were the exception. The private aid we hired to sit with my parent had her own disturbing observations. She said when she requested a diaper change for my parent, she was met with an attitude and the diaper wasn't changed for a shocking 7 hours--from 11 am to 6 pm. This is totally unacceptable, especially for someone who needs consistent care. After seeing all of this, it's hard not to conclude that this is a Medicare-draining facility more focused on cutting costs to maximize profit than providing decent care. A Cash Cow for the owners. If patients ask to be taken to the bathroom, they're told to just relieve themselves in their diapers. This is not only disrespectful but also likely contributes to unnecessary health problems like UTIs. If I had known how poorly run and filthy this place was, I would have never considered placing a loved one here. It's clear they're more interested in lining their pockets than actually providing quality care. I deeply regret this decision and would strongly advise others to look elsewhere. If I don't come EVERY SINGLE DAY- it's like when the cat's away/ the mice will play! Filthy. Unkempt. No one takes any blame for the mess and criminal neglect Disgusting. My Mother was released from their care and sent to the hospital and no one contacted myself or my sister in regards to my Mom , and how she was doing since being transported to the hospital from your facility on 11/6 at my request, a day before you assessed her and said she was able to go home. Two days before she was released, I was visiting her and noticed that blood had been drawn and two days later when she released, we still did not have the results of that blood test from the staff. That same arm that was used to take blood was so swollen and bruised, that I cannot believe the staff, when drawing that blood, did not think that was an urgent issue and alert a doctor or a nurse. Prior to that blood being drawn, I had made a request twice to an aide to ask someone to look at her arm. It wasn't until my 3rd request that her arm was finally looked at, and even then, we never received any response from the staff of the results of the alleged ultra sound that was done, as to what may have caused that, nor did we ever get the blood test results from you. I cannot understand how a rehab could not see that clearly my Mother was not well. She slept all day, everyday for the last few days of her stay there. She was not in a right state of mind and was in severe pain. The communication from the staff was consistently poor and important updates were constantly delayed. There were countless times that myself or my siblings went to the desk for help, and no one was there to talk to. Thankfully I insisted that she be taken by ambulance to the hospital after seeing her condition the day before you were releasing her to go home. That email was never acknowledged until we received a call that my Mom was already in an ambulance on her way to the hospital. Not one person called prior to putting my Mom in that ambulance to advise us that they had agreed to do so. They just called the ambulance, transported her to the hospital and THEN advised us she was in an ambulance. Once she was admitted to the hospital, we were advised within 2 hours, she had a UTI and the Flu and was dehydrated. When my Mother was admitted to your facility it was for physical and occupational therapy. She was alert and for the most part as healthy as she could be for 87 years old, with an underlying heart issue. When I walked in to see my Mom the day before she was being released, she was laying on her side, naked, wrapped in a sheet, and waiting for the aide to come back to dress her and reposition her back in bed. When I grabbed an aide for help, she advised me that the aide that was changing my Mom went on break!!!! That's not care. That's an utter disgrace. That same aide advised me she could not put my Mom back in bed by herself and would get another aide to help. I waited 15 min for her to come back with another aide, but that never happened. I had to chase her down in the hallway and insist she help my Mom immediately. I had to help that aide, clean my Mom and reposition her back to a comfortable position. The facility is understaffed and it shows. There was a time that I walked in the room and her bed was on the highest height it could be, she was laying there at the edge of the bed and she was a fall risk. No one paid attention to her. I rang the buzzer because I was too scared to leave her alone that way and it took them 20 minutes to come. That's disgusting behavior for a facility that is supposed to care for their patients. The aides are not properly trained and do not show any compassion for the patients in their care. I watched one of the aides fight with a resident as he was trying to maneuver his wheelchair around his bed. She grabbed that wheelchair and pulled it away from that man very aggressively and then proceeded to yell at him. My mother was left sitting in her wheelchair naked after a shower, in the shower room and had to scream for someone to help her and get her dressed and back to her room. The facility is dirty. There were numerous times I was visiting my Mom and my shoes were sticking to the floor, and her table was covered in spilled liquids and old food. The resident bathrooms were filled with old urine filled underwear and sometime the toilets were not even flushed. Basic care needs were often overlooked. There were times my Mother waited 30-45 min sometimes just to use the restroom. Or she sat in a urine filled diaper, waiting for your staff to change her. If she needed to use the restroom while your staff was delivering meals to all the residents, she had to wait for the aides to finish serving meals to bring her to the restroom. That is shameful. Hygiene was neglected. Her hands were always dirty, her sheets/blankets were stained or wet and her call button was constantly on the floor. Being that some residents were bedbound, it is their priority that the aides make sure there residents were able to call them if they were needed. They neglected that duty as well. These are not minor oversights. read more

Ever since the state monitors left the facility it's back to its dirty old self. The food is disgusting and the social workers don't care about anything but raiding rooms instead of getting people ready to go back out into the community .I think the state monitors should have never left. read more

My developmentally disabled brother was a patient in the summer of 2023 because he had a broken kneecap and was in a full leg cast. Since he couldn't advocate for himself he was not properly cared for as far as his hygiene. He was discharged with a bad finger infection without the proper medical orders in place. He developed gangrene less than a month later and had to have his finger amputated. I can't rate the PT he received but it was probably adequate. read more

Inpatient physical therapy. The physical and occupational therapy is great. Everything else about this place is awful. I have requested a room change for 4 days because my roommate blasts his his TV all night and refuses to turn it off. He does nothing but curse all day and night. So far the only answer I get from admin is "We're working on it". read more

Patients are neglected. Facilities are worn down, filthy, and is run in a extremely disorganized fashion. Medications are not given in a timely manner. They have in the chart indicating that the patient is diabetic and has high blood pressure, but have been feeding the patient a high sodium, high sugar/ carb diet. Who in their right mind feeds a diabetic frosted flakes and a cookie for breakfast, then disappears when insulin is needed?! One morning we advised the nurse the patient was in pain (before 9am), they said pain meds would be given in about 15 minutes. After 1pm, the patient was still not given her medication and I had to follow-up with the nursing supervisor. This is not a rehab, this is a place people are put to rot away until their final days, which will obviously come sooner after the way they treat their clients. read more

A horrifying experience I would not put my worst enemy through. For the sake of life itself don't put your loved there. And most of all DON'T go there for rehab for knee or hip surgery. You will REGRET IT!!!! If you ring for help forget any response right away. RUN OR CRAWL AWAY... Forgot to give you'll my story Went there after knee,I had to inform the nursing staff that I had an infection. I was informed that there wasn't any antibiotics on hand and would have to some. This is on Friday and they told they won't get any until Sunday. To make the story short and not leaving out the lack of pain meds and finding out from doctor after I left, that he sees that I received two weeks pain meds in 4 days. Go figure NIGHTMARE read more

If you want really good CNAs and Drs for your aging parent this is not the place. Communication is horrific with both.

Stay far away from this care center. Most of the staff are nasty people. Meds aren't given at the proper time or not for hours after. Can't get any answer from upper management if they even call back. Food isn't even fit for animals Please, Don't put any of your loved ones in the center read more

My father went there for short term rehab and he needed an iv antibiotic twice a day and they were ok on the weekdays but weekends they didn't give my dad his iv and he has endocarditis and needs his antibiotics! DO NOT LEAVE YOUR FAMILY MEMBER HERE! my husband was in rehab for 1-1/2 months last summer It is disgusting! I myself had to bring cleaning products to clean the bathroom and his area. Every night i would go and change his bed and blankets myself because the aids would disappear and not change his bedding. I brought in a cooler so he would have food to eat. Yogurt, pb&j, etc... along w snacks. There were a couple of nurses that would go above and beyond to help and make sure he was taken care of but those handful that were there when they were off were the worst! My husband was afraid to sleep at night because if something were to happen, the night staff would not know what to do. He actually witnessed the man in the next bed, in distress.. the nurse aids had no idea what to do nor could they find a nurse. A couple hours later the morning nurse came in and went into panic.. first thing she said is 'did anyone check his sugar'? His sugar was so low, he may have gone into diabetic coma.. we are not sure as he was rushed to the hospital. We did find out a day or two later he did have a stroke. and why my husband was afraid to sleep. The Management are a bunch of bullies and look at the Insurance $$..had many a run-ins with them. With all these bad reviews how is Fulton Commons still in business??? it took me a year to write this. My Dad was in this facility from Nov 2019- Jan 2020. THEY DO NOT HVE ENOUGH STAFF. Took over 15 minutes to get help for bathroom, did not keep eye on patients. My dad kept falling out of bed so we had to HIRE a person to stay at his bedside at night. You have to be there 24/7 to make sure they are cared for! Sad. one specific nurse was great and the people in rehab kept you informed if you asked questions. Lots of other patients families at the facility were very disappointed with this place. I went to the top many times-they were no help!!!! read more

If I could give zero stars I would. The new owners of this place could care less of the quality of care of the patients. Straight from one of the staff members was told it is a money business for the new owners and they just want to get people in the beds regardless of how under staffed they are. The food, not edible for a dog. I will not discount the staff members who are caring and friendly, they are just run haggard. If you drive around back there are piles of empty crates and boxes scattered across the parking lot and property. This place should be shut down. Although all clothing are labeled, don't expect to get all of them back. Two nightgowns, a bathrobe, 2 tops, and slacks went missing. We received other peoples clothing, with their names labeled in them. One item was torn. Good thing we didn't send a pair of good earrings, because her cheep pair went missing along with her denture cup. This is the worst by far! The only thing that was somewhat good was one or two nurses but that's it. I could go on and on! I have pics of how filthy her table was in her room! Everyday I would go there would be wrappers from her food tray and wrappers from her needles or meds she was given. There was one day I went there and bruises were on higher part of her chest. I would have to pull Information from them nobody and I mean nobody would call me about changes unless I asked. I also asked about her medications because I noticed that she was slurring her words and she was never like that before they kept telling me no they weren't giving her anything out of the ordinary then I find out they were giving her some thing that I told them I didn't want her to get anymore because she has never has gotten it, but they didn't listen to me with that either. I felt her going in there mentally she was fine going in for rehab only and it ended up with pneumonia in her lungs and went downhill and then get a call she was in cardiac arrest and to this day I don't believe that they tried to bring her back but that is my personal opinion!! Don't allow your loved one to be cared for by this establishment!!! My mother was transferred to Fulton Commons on August 20, 2022. She was sent there for rehabilitation after a stay at St. Joseph's hospital for a raspatory. My family was in the process of looking for a nursing home for our mother before our mother's hospital and this is one of the facilities that the hospital social worker suggested. Our family agreed that she would rehab here and then transition into long-term care. That was the worst decision of our lives for our mother. On September August 31st my sister received a phone call at 5am from Fulton Commons informing her that my mother was having difficulties breathing and that she was going to be transported back to St. Josephs for treatment. My sister received a follow up call at 6:55am stating that transportation was finally taking place and that she was going back to St. Josephs. The individual that called didn't state that this was a manner of urgency and the fact that they are transporting her to a hospital that is not the closet one (NUMC) gave us a false sense of it isn't that bad she is just having a COPD flare up and once they give her some treatments or steroids and then she would be transported back to Fulton Commons. When my sister did not hear back from Fulton Commons she decided to head to St. Joseph's to see how my mom was doing. She arrived at the visitor check in and requested to see if my mother had been admitted or if she was still in the emergency room. The staff took my sister outside to inform her that my mother had not been there since August 20th when she was released into Fulton Commons Care. My sister is now completely confused and calls Fulton Commons to verify where my mom was transported to. My sister reaches Fulton Commons and they inform her that my mom was never transported and that she was not sure why she thought that, to which my sister replies that I was called and told she was going to be transported. The staff member asks my sister to hold while to goes to check and then gets back on the phone to say she wasn't there and they did not know where she was moved to. Now my sister is frantic and decides to call her husband who works for Nassau University Medical Center and asks him to please check if her mother was an admitted patient there. He verifies and informs her that she was admitted and is in MICU and to get there as soon as possible. My sister arrives at the hospital to find our mother on a ventilator and that my mother had to be intubated at Fulton Commons because she was found unresponsive and that her carbon dioxide level was 97%. It took Fulton Commons almost 2 hours to transport our mother and we don't know how long she was laying in her bed unresponsive. This is an absolute case of abuse and neglect on Fulton Commons part. At no point did they try to reach out to us to see if we were able to locate our mother or even try to explain what happened. We went to collect our mothers belongings and asked to speak with a supervisor and even at that point we just kept getting the same response of I was not on when this took place so I can't answer your question. My mother was on a ventilator for 3 days and when she finally came off she has absolutely no memory of being brought to the hospital. Her memory is in a horrible state. She thinks she is trapped in a basement or stuck at her job that she left over 20 years ago. I just want to caution you to please do not place your loved on in Fulton Commons care. When my sisters and I would go to visit our mother she was either left sitting at the table in the dinning hall or in her bed. The aides walk around with air pods in their ears and you don't know if they are talking to you or someone on their phone. I understand that pay for this type of job is a joke for what they are expected to do but if you don't have compassion or at least care for those who need you then you should not be working in that field. We are now trying to pick up the pieces and figure out what is best for our mother. She has a long road ahead to recovery and we just pray she gets her memory back. read more

When a loved one is forced to spend time in any facility, there's always concern. Fulton Commons did not let us down! From the moment we entered the facility, my mom and I knew that we had to stay very close and advocate for my dad. My dad (91 ) had fallen and sustained 4 rib fractures. He's dropped off at Fulton Commons by a ambulance that dumped him in the bed wrapped in a fitted sheet like a sack of potatoes. We arrived 2 minutes behind the crew to find him confused and terrified , and no staff present! Half side rails with no fall prevention measures in place! It was 2 hours before any staff came into the room. In short, with multiple disappointing incidences occurring from the onset, my mom (82) spent 20 days there (all day) to advocate whenever necessary. It was clear early on that the majority of these patients were totally dependent with all ADL's, yet there was very little ancillary support to render this care in a timely fashion. Laying in ones feces or urine for an hour is unacceptable! My mom having to assist my dad to the bathroom (fall risk) because no one was available to assist. Even at 91, his dignity is still important! He did not want to defecate on himself! Not having any other facility to compare Fulton Commons to, my advice is to stay close and be prepared to advocate! read more

If I could give this a negative stars I would but its not possible. I don't usually write reviews but for this place I have to. Please, please do not even think about putting your loved one into this place. I was lied to by a lady named "Risky." Her name should have been my first clue. She was so convincing I believed her. She said all the bad reviews were in the past and that a corporation named Grand had taken over and it was now an outstanding facilty and that I could trust these people with my husband. Oh my God. Not only is the place a dump, the nurses and aids are so mean and uncaring. Especially one named Nicole who screamed at me when I tried to get help for my husband who was there only 2 days and was starving and dehydrated. Please no matter what they tell you avoid this place at all costs. The Nursing Supervisor who worked on Saturday, March 18th at 2:pm should be fired. She ignored my pleas for help. I called the police who sent an ambulance and got my husband the hell out of there. read more

This place should have negative stars. Please, even if you do not like your loved one, do not put them here. It is a slow torture of death. The state must get involved to shut this place down. Do not be fooled by the outside. It is beautiful. Then once your loved one is there..(My mother.) it is an endless torture room. You sit in your own feces for hours. There was even a point where they pushed my mother and kicked her. They do not answer their phones, and the room phones do not work. So when you leave, it is as if you will never be able to get in touch with them again unless you go back in person to see your loved one. The stereotype of being pushed in a corner to drool all day is really happening at Fulton Commons. I could not be happier when she got out of there. read more

This center is as good as it gets! In 2017, I was in Long Island Rehab centers FOUR times. Three of the times were very trying, not conducive to healing at all. Then I arrived at Fulton last November and December, and the healing they provided physically, medically and spiritually helped to make me the survivor I am today! read more

My Mom had dementia but was admitted with a fractured pelvis. Long story short she is no longer with us. After a month of their neglect and overmedicating she passed away from sepsis and liver and kidney failure. She arrived there ambulatory and continent. Before her death she had diapers and a wheelchair. We had to insist that she get showered and changed regularly. The Dr was a fan tom who we never met. I asked for his phone number and was told he would call me. He did so the day after she passed. I could go on and on but the bottom line is check the reviews before you or your loved one go into a rehab or nursing home. read more

IF I COULD GIVE 0 STARS I WOULD. Let me start off my saying, my grandmother has UNFORTUNATELY been a resident here for almost a year now. The facility is so disgusting, there has been blood spots on my grandmas floor since she was moved to this room which shows the floors are never mopped. My family brings in cleaning products to clean her bathroom ourselves. And we also do her laundry because her clothes went missing went Fulton commons was doing it. -Next, the nurses on the floor DO NOT CARE about the residents. My grandma will hit her call button to use the bathroom, and no one will show up until an hour later or they don't show at all.I've seen this with my own eyes multiple times. They take so long to the point that she has to get herself out of bed and she fell and got hurt - she had to drag herself across the DIRTY floor to get to her call button.. she laid there until someone finally showed up. The nurses put the residents to bed at 7 at night so they can sit in the break room and hangout. My grandma likes to stay up to watch the rangers games or her shows, and everyday they try and force her to bed right after dinner so she is one less resident to worry about. -Next, the food served is terrible. My grandma has lost over 40 pounds since being here, and so has multiple of her friends who are residents. Everyday my family needs to bring either dinner or lunch to make sure she is eating. Her friend eats a pb&j everyday for every meal because it's she will eat. -Next and finally, the icing on top of the cake! She was given an Easter basket, which every item including (bath and body works perfumes and lotions, toiletries, room sprays) were stolen besides the candy. My grandma is sharp as a tack and is FULLY AWARE to her surroundings she did not misplace any of these items which is what administrators told us. Her friend who is also a resident, said a ring and bracelet were taken from her nightstand draw. -in conclusion, my grandma has called my family crying far too many times. And all I really want is for SOMEONE,ANYONE TO CARE. my family and I feel as if we are badgering the staff to just do their damn job and do it correctly. I like to base my reviews on my first impression. It's not to say that this place may not improve on future visits, but I have some doubts. Upon entering the main lobby, you have a 2 level atrium with a security guard behind the desk. This is probably the nicest part of the entire place. When getting to the floor I needed to get to by elevator, I was let out into an empty lobby that gave a choice to go one way or another. It basically felt like a typical hospital. I went to the area where I was to visit. When arriving there, the person I visited was not in their room, and no one was at the main desk. The desk seems to be left empty when meals are being served ...even though there are still people in their rooms sleeping and/or not deciding to eat in the dining area. It was strange to see no coverage at the desk...also, personal items and office materials at the desk were not monitored by anyone, so a patient may be able to access these things when no one is looking. Not very reassuring. The food, while they make it seem well-balanced and creative, didn't smell very good, and the quality seemed subpar to average at best. The residents agree that the food is nothing to write home about. I'd rather eat in a shopping mall food court, but that's me. With that said, I'm sure these patients are paying a good dime to stay here and they deserve better. The activity selection for patients is very narrow...maybe one or two choices of things to do during the whole day. The nurses that I engaged with didn't want to know about helping you with anything outside their normal routine, which is sad. Ive heard the same thing from residents...also, dont make the nurses mad or they will find a way to get back at you somehow. There are no walls separating you from your roommate, you must rely on a curtain, like a hospital. If you are looking for a nursing home, I'd research this one very carefully, and do a complete walk-through a few different times of the day. This way, you can make a good decision whether you want to enroll here or not. Overall, a strong two stars. read more

1 star would still be giving Fulton Commons Care Center more credit then it deserves. This facility is poorly run from the top down. I had a loved one who was admitted for Rehabilitation and stayed for just under 2 weeks. In that time I was constantly being the middle man between my loved one and the facility for simple things like ringing the bell because they could not get out of bed themselves to use the lavatory. I was told they were waiting for close to 15 minutes for someone to help them to the point they picked up their cell to call me so that I could call the facility to let them know they had to go to the bathroom. And then I was placed on hold for 20 minutes until someone from that unit could pick up the phone to even listen to my request! I am not a healthcare professional but that seems a bit excessive for a place that claims to be in the upper echelon of care facilties. The administrator will have you fooled on these review responses making it sound like they truly care when in reality, when you call to speak to them on certain matters, they cannot handle confrentation and will immediatly put up a wall of defense of their facility. If you catch them on a good day, they might even give a condescending chuckle at you as you try to describe an incident from your perceieved point of view. I am not on here to sway anyone against using this facility, but I would truly do your research before deciding on this place. I am not even a person to post reviews, but I am so regretful of choosing this place and I do not want anyone to be in the same position looking back on it. read more

My mother was a resident of Fulton Common Care Center since Oct 2018 until her recent death on March 8, 2021. This facility is probably one of the best I've seen on Long Island, clean, bright, friendly. Although, we saw some tough times during Covid with limited staffing (due to quarantine), no visitations and suspension of activities the administration kept on top of keeping us informed. Of course, no one wants to be in a nursing home or restricted to their room due to Covid for a year but my mother was comfortable and well cared for. She had all her mental capacity and we spoke almost daily. She remained positive and up beat often complimenting the care she was receiving. Although her mental state was excellent she had several co-morbidities that put her at high risk of Covid. The Administer (Ms Doyle) was very proactive with protective measures put in place long before our Governor mandated for NH and communicated the status of the residents via daily robo calls. I also received a weekly update from the resident nurse. They kept my mother safe from this terrible virus but unfortunately she passed unexpectedly in the hospital due to other health problems. read more

If you're young and able to chase after the nurses and staff for help, and know what medicines and foods you are required to receive, you might be okay. If you are older, and need to rely on the staff, whose job it is to make sure patients are cared for, try someplace else. Unless, you are willing to hold it in while you wait fifteen minutes for someone to help you in the bathroom. Unless, you are okay with eating food that you're not allowed to eat. Unless, you are okay with discussing your aftercare and release, not with those that can help, but instead with the activities director. If you're not okay with the above, do not consider this place! read more
