We needed a spot immediately, and I feel the price reflected that need, it was very expensive, maybe unfairly so. I feel they put her in a price point higher and had assessed more care than what she truly needed, and getting that downgraded was more difficult than it needed to be. I feel part of the reason for this was some the forum was in the transition of new ownership which makes things bumpy. When she first arrived was the forum at Knightsbridge and when she left, it was the estates at Knightsbridge? To be honest, I'm not sure what the name of the place actually evolved into everybody still calls at the forum. I have since moved her out of the facility I can say that when she was in their care, her experience was good. She never was treated roughly or had any incidences that I needed to be aware of. The food seems to have gone downhill and I do not like that the assisted living across the street where she lived is not as nice of the public spaces as it is across the street for the independent care that is my biggest ripe of this place the people who need more care and pay higher cost have a lesser aesthetic in their common space and that did irritate me. I'll be up. It irritated me more than my mom. We moved her to a different company/facility not because of care or quality of care but more due to location. I do like that the forum has memory care and skilled nursing. It was a nice place for my mom, but I would explore pricing of other places when you're looking at this place, this place is good but also is expensive comparatively.

We also switched to a facility in Upper Arlington for a 6 or 7 month period during that time. The people at The Forum cared for my Mom in so many ways. They went from being friends to being caretakers during her transition. I could not recommend The Forum highly enough for how they took care of Mom. We moved Mom to the facility in Upper Arlington after she broke her hip. Great physical therapy team and great facilities. But the one thing they lacked was humanity. They didn't care about Mom, just the checks my sisters and I were writing. Back to The Forum. Not the gold standard for facilities, but certainly good enough and they are upgrading constantly, but more than the Gold Standard for the assistance we received and the help from the entire staff. Mom passed a couple of weeks ago. The nurses and hospice people that took care of Mom were in tears. Genuinely. Love this place and you'll be blessed if a family member has to be there.

I've seen the daily routine of care and you can see the aids are in experienced and seem careless at times leaving her for hours of unattended checks on her for bathroom visits. This leads to her room smelling of foul smells. This is the reason local family members for not wanting to see their grandmother. I'd give five stars to of activities department. Because of the one great RN named Dawn, she should get five stars. So, that's a total of two stars for the rest of the place. For the price we pay, this place should have better aids and provide more attention vice leaving them to exist in their rooms to sit in filth.

If my aging parents lived in this state, there's nothing that would give me greater happiness and peace of mind than seeing them retire to FVD. Imagine a super cool college campus with great people, fun activities and awesome food - but NO job, responsibilities or stress whatsoever. That's what retirement living at FVD is like. You move in young, healthy and spry in your 60s and basically have the time of your life. As you get older, additional care and support is there when you need it. The new expansion they're doing, while obviously causing a bit of construction noise/mess now, is going to be AMAZING once it's complete. The Health Center (nursing home) side is wonderful too, once the time comes for that level of care. The people are great - both residents and staff. I love this place and I'm so glad I get to work with the team and residents as part of my day job!

The facility and staff in the mc unit were great back to my grandmother even though she was nonverbal due to Alzheimer's. My interactions and time at the facility were brief but they stood out because of those positive interactions. My 2nd mom who I've been taking care for the last several years is currently a resident at Smith's Mill for rehab. Unfortunately her dementia has progressed so I wasn't sure what to expect from her once she was transferred from the hospital. She's slowly perking up except for when Ashley Everson stops by her room each morning to check on her. She's much more talkative with Ashley and more like herself. That alone was enough for me to leave a review. As former fire/ems, I've seen all kinds of facilities and staff so I came in to sign paperwork and ready to ask a hundred questions. I left without asking any of them because Ashley covered everything I was going to ask and went over things I hadn't thought to ask or even thought about. It's rare to find a facility that is as nice as they appear on the outside. It's even more rare to meet someone like Ashley that genuinely cares about not just the residents but making sure the family members are informed and kept in the loop. When visiting my mom, I saw Ashley pass by the room and stopped her to ask the various units and then asked about meds because my mom said she hadn't been given any pain meds. I took that with a grain of salt because dementia. Ashley could have said she would send the nurse in to talk to me about meds but instead, she talked to the nurse and let me know when the last dose was given and how long before my mom could receive another dose. In that same visit, I was greeted by people left and right while walking to my mom's room. I brought her dinner for her birthday and she was just finishing it when Myshawn came in with dinner and wished my mom a happy birthday. She was slightly confused so he showed her the meal order with her info on it and the happy birthday at the top. Asked how her birthday was and if she enjoyed her dinner then said he had a piece of cake for her. She was all smiles during that interaction with him. Those moments where she's smiling ear to ear while in rehab are fleeting so it means a lot when that happens. I've told a number of friends that have aging family members about my experience over the years any time my mom goes to an SNF so they have options when needed or when they start the process of moving a family member who needs more care. I've since told all but a few of them about Smith's Mill and if they decide to check it out, ask for Ashley because she is not only a font of knowledge when it comes to elder care but she is genuine when it comes to caring about the residents. That and the food has looked delicious every time I've been to visit during meal time. Honestly a little envious of my mom on that front.

Everyone pitches in to help where needed--you see that most clearly when help is short in the dining room-- aides and activities people, guest tours person and even the front desk lady pitch in. Most of all, the executive director Dennis and the new supervisor over the aides, JD, are out there leading by example. Are we missing a nurses today/tonite? Dennis Is also a nurse, and he'll step in, day or night, and JD does the same with the aides--and no complaints--they do what needs to be done, and if there is garbage to take out or a mess on the floor that needs to be taken care of---it is, and no task is 'beneath' them--all pitch in when they see the effort from the top down. Dennis also seems to be holding everyone to a high standard, and JD is following, and the staff I interact with, almost all already excellent. are stepping up and responding more quickly to my call button, and other needs. I know that finding staff is hard, but that's true everywhere ( from things aides have shared about other facilities.} New aides and nurses are arriving now, tho, and I am happy with the current situation.The food is very good and varied, and problems I had with transportation and meds have been taken care of positively. I chose this place out of 4 choices and I'm glad I did. (and so is my cat.)

    WARNING: LONG REVIEW…read more We first connected with the Avalon because of a need for respite care for my mother-in-law while my wife needed to go out of town for her work for a week followed by surgery and recovery for a 2nd week. Most assisted-living facilities, memory care facilities, and nursing homes do not offer respite care. Those that do, require a 30 day stay. We did not need a 30 day stay. The Avalon only required two weeks. I am surprised that more facilities are not as accommodating as the Avalon. Many people needing respite care will eventually require their loved one to live in such a facility. What a great opportunity for people to connect with the Avalon. There is a huge need for respite care for caregivers. We applaud the Avalon. More places should be as accommodating as the Avalon. My mother-in-law ended up spending two more weeks for some needed physical therapy. At the end of her being there for four weeks, we determined that her dementia had progressed to the point that we could no longer provide adequate care for her. She had lived in our home for four years, and it was our hope that we could care for her until the end of her life. We had come to the place where that could not happen. Roxanne and Colleen handled the administrative details in getting the respite care in place and in transitioning to long-term care. Both of them were exceptionally kind, accommodating, and effective. The facility Nursing Director Tasha and the memory care floor Director Debbie were very good about partnering with us to make my mother-in-law's stay healthy and comfortable. They had outstanding attitudes whenever we had question, a suggestion, or a concern. On the whole, they were good about getting back with us in a timely fashion. The memory care unit nurses and staff were amazing. They were beyond kind to my mother-in-law and us. They provided loving care. My mother-in-law went into hospice care while at the Avalon. The hospice case manager said that she is in a lot of assisted living, memory care, and nursing home facilities. She said she had never seen Staff continue to be so concerned about a resident after transitioning into hospice as was the case with the Avalon staff in regards to my mother-in-law. When my mother-in-law took her last breath, my wife was by her side with a few staff members behind her shedding tears along with my wife. Basically, staff showed us and my mother-in-law great care and love. Foundationally, isn't that what we really want for our loved ones that have to transition to assisted-living, memory care, or nursing home? During the many times we were there, we never saw a staff member treat any of the residents in an unkind way. It was impressive to watch how they were so patient. People in memory care can be a challenge. The Avalon staff were exceptional in how they handled matters. The Avalon has a contract with a physical therapy company. My mother in law received exceptional therapy sessions from a therapist named Harneet. In addition, she was very kind to my mother in law and us. We were impressed by her personality and how great she was about consistently communicating with us. What a jewel she is. When you enter and tour the Avalon, you will notice it is very stylish and clean. The hallways are wide, and there are multiple lounges on each floor where families can gather with their loved one. You are always greeted by an exceptionally kind concierge at the front desk. Before we decided on the Avalon, I visited approximately 10 assisted-living, memory, care, and nursing homes in the north Columbus area. I spent time on the phone with many more. The pricing for the Avalon is similar to that of other facilities in the Columbus area. We visited some facilities that were even more expensive. The meals are good and they have an alternate choice each meal for those that do not like what is being served. They have a tall van so they can take residents to area restaurants and outings. They can also take residents to nearby doctor appointments. Was the Avalon experience perfect? No. But, I cannot stress enough how comforting it was for us that they considered us to be their partners as they provided care for my mother-in-law. It is important to remember that they have to provide consistent and detailed care day in and day out. It is not an easy thing to do. It would be our recommendation that the families of people going into care draw close to the staff and administration. They do not need to become your best friends, but get to know them. Initially, visit your loved one frequently and at different times during the day. Staff will see your concern, and you will get to see how things are handled. Show kindness to the staff. Don't look at a staff person as another "faceless person" in your life. Learn their names; greet them. As we moved my mother-in-law's things out of her room and left the Avalon for the last time, we were hugged by the staff as we left.

But the moment I stepped into The Inn at Bear Trail, I knew I'd found "the one." And I'm not just saying that because it smelled like fresh flowers and good decisions--though it absolutely did - even the Memory Care unit! (trust me that's a key clue that they are on top of things). What sealed the deal? The staff. These folks are the real deal - Starting with Mike Harvey who runs a tight ship over there and is super attentive via text or voice at all times. His aids and nurses are professional, warm, and genuinely there when you need them. They move through the halls like a well-choreographed dance team, always available, always positive, and always one step ahead of what my mom needs next. They're proactive about her care, catching details I'd miss and implementing solutions before problems even arise.     We are paying a significant amount for care, yet the overall experience has been disappointing. The…read morequality of the food is minimal, and residents are restricted to eating only during designated meal times. If a meal is missed, there are no adequate alternatives provided--sometimes only broth and crackers are offered. Services that are supposed to be included in the monthly fee, such as weekly cleaning and laundry, were not consistently completed without repeated reminders. It often required ongoing follow-up with staff to ensure these basic services were performed. There have also been concerning issues with care. On one occasion, a nurse attempted to administer an excessive amount of medication to my parent, which raised serious safety concerns. Additionally, the facility manager has not been particularly approachable or helpful when addressing issues. Billing has also been problematic. The most recent statement included additional charges we felt were not owed, requiring us to escalate the matter to corporate. To their credit, the corporate office was responsive, professional, and understanding of the concerns raised--many of which extend beyond what is outlined here. We took her out on a little ride and she could not wait to get back to her room. She has had a pretty rough 85 days being in and out of hospitals and other rehab facilities that were lower than below the lowest low for facilities. I wish we had found this one the first time around. Dr. Deneen is the epitome of professionalism and he really does care about the residents! He introduced himself within 10 minutes of her being there. He saw her almost every day. Every day. The nurses are AMAZING!! One night I was leaving from my mom's and saw 3 sitting around one of the tables having dinner with a resident. They asked how her day was and what she had done. Ami came in and hugged my mom and sat down beside her and told her how much she was going to miss her. That is what you want to see in a facility you are leaving your loved one in. Beth had to call me to set up a care conference and the first sentence out of her mouth on my voicemail was that this was a non-emergency call. Now that may not seem like much, but when you are on pins and needles because your loved one has been very sick and has ups and downs for months; a phone call from the facility they are in usually is an emergency. She really did not have to say that is my point. However, she did and it was very much appreciated. The whole atmosphere is very homey and calm. They have bright call lights above each room and they have signage that shows that at the nurse's station which room needs assistance. Wild turkeys, deer, beautiful places for family to sit inside and outside. It has been impressive from day 1 and I could go on and on, but I do not have enough room. Our mother was taken care of just like we would have and we felt at ease during the day knowing she was there. They care, truly cared about her and helping her get back on her feet at the highest level they could help her get to. We've sadly taken her to other facilities in the area and were disappointed beyond belief. Not at New Albany Rehab. I hope she NEVER has to need that type of care again, but if she does, she is definitely going there. Thank you very much to: Kristin, Rebekah, Lorraine, Latoya, Kayla, Dr. Deneen, Cheryl, Annalynn, Joe, Liann, Judy, Cathy, Miriam, Ami, Brian (OT), Jeremy, Brandy, Katie, Jasmine, Tammy, and Beth.
