I am currently at Fountain Valley Acute Rehab Center after my knee replacement surgery, and I honestly can't say enough good things about this place. The entire staff is unbelievably attentive and genuinely focused on doing what's best for their patients. You can feel that they truly care. My wife's uncle was placed here a couple months back. He was on the 3rd floor and had a roommate. The front desk looks amazing since they upgraded from Manor Care. However, I know they are still making changes since the building if I am not mistaken under new ownership. Anyhow, upon arriving to his floor, there were CNAs and the nurses desk near by to see if we had any questions. The staff did get his room ready as he did have Enhanced Barrier Precaution isolation. So we are thankful for that. The food was good and the care was okay. I wish the staff was a bit more attentive instead of hanging out in the hallways having conversations about their personal lives. In that setting, it was a bit disappointing and unprofessional. We visited in the 3 weeks he was there, and no major red flags or concerns on our end. If I was to be asked if we would readmit him there again, I would look around for options first before opting to go back. Thank you to the team that did take care of him, and ensured he was safe to come back home! Not ever nursing home is the same or has a horrible back story! Ours was just okay... and I am happy to leave the 3 star review for services. read more

I recently had a knee replacement and was placed with Fountain valley Post facility.I stayed here for rehabilitation after surgery, and I want to thank the nurses and staff for the excellent care. Everyone was kind, patient, and attentive to my needs. Recovering is stressful, but the team made me feel comfortable and supported. I can't speak highly enough about Samar at the front desk of Fountain Valley Post Acute. She is always pleasant, helpful and just a lovely human being. Recently she went out of her way to help me with an issue that was very important to me and I think it is great that the face of Fountain Valley Post Acute is often someone who is lovely who is thoughtful who is just a beautiful human being. The therapists were knowledgeable and caring. Also the CNA's were helpful but management for the most part could have been much more efficient, Idid have a woman in Registration that was very knowledgeable and caring. In particular Dr Nguyn. In telling me that I was being discharged she was very rude and didn't want to hear what I had to say. (She definitely should not be the primary nurse.) I would never again go to this facility or recommend it to anyone. read more

My sisters Doctor placed a stage 4 cancer patient in Manor Care now known as Fountain Valley Acute/rehab facility. This is NOT the place for someone struggling with various cancers. This facility is more of a convalescent hospital / rehab. The staff was NOT prepared for end of life patients and we immediately recognized this from day one. Be careful where you place your loved ones, it took weeks to get her out of there and it wasn't necessary. Doctors are NOT always right. read more

I dont typically leave reviews, but this place is concerning enough that I feel lives could be at stake. Unless you hate your family memeber/ friend and want to see them suffer, DO NOT under any circumstance have them admited to F.V. Post Acute! You have to give a place a star to be able to post a review, I hate to give it even one. This place sould be thoroughly inspected and more APS (adult protective services) calls need to be made! My mom fell and ended up with a couple of fractures rendering herself unable to get up and move without assistance. This place left her in bed for hours without help, never coming to her need when the call button was pressed (multiple times & multiple responses of we will be right there) This included having to wait for hours for trips to the bathroom, often resulting in accidents and having to lay in it for a long time before someone showed up. The food was inedible, piles of color that were often stuck together in one solid lump. Things you couldn't begin to chew, still frozen or cold, or that would basically dissolve in your mouth. You need to supply your own snacks if you want them to be able to eat anything. 2 weeks into her horrific stay at this place my siblings and I started receiving incoherent calls from her at 330 in the morning. Ramblings, mutterings and non complete sentences. She was hallucinating and all kinds of erratic behaviors. After the 6th time calling (I was told prior they don't always like to pick up the phone and to NEVER leave a message) the nurse in charge finally picked answered. She sounded angry that I was bothering her. Instead of letting me know what was going on, she apologized for my mother's behavior and brushed it off as she is old and that's what old people do when they are away from home and to not worry. This wasn't my mothers typical behavoir and I knew something was wrong. I rushed down there. The only person there with a heart and compassion found me and informed me very discreetly that my mom had been in this state for at least 24hrs prior and that it was a good thing I was now there. These places are supposed to let you know immediately if there are any sudden changes in the patients behaviors, or conditions and nothing of the sort was done. STAT bloodwork had been ordered many hours prior and still hadn't shown up despite the fact that her heart rate had continuously skyrocketed upwards of 106 (resting) to 150 at times for days!! The nurse I spoke with was very upset with my arrival, never once coming to check on us once I arrived. Lies were started about having already spoken with my sister about things. No one had contacted any of us. My sister and I speak everyday & would have been there the second we heard this was going on, just like I did when mom started alerting us. After a confrontation with Dr Nguyen the Nurse Kim and others, the Dr finally admitted (even though she had already seen her 2x that morning due to concern) a 911 call should be done. Full sirens arrived and she was taken to the hospital. She was admitted and has been there for the past 3 days, unsure of when she might be discharged. Unfortunately they were so focused on it being a stroke, or an issue with her brain, no one ever thought to do a toxicology report. Speaking with much more knowledgeable and professional medical professionals outside this facility, it has been speculated that due to the state she was in, it is most likely she was given the wrong medications, or an overdose of her own. The communication from here is horrible. No one answers phone messages or emails timely. Waited over three days for a return call from business office never received a call back. First day we asked if we could meet with the case worker. The person doing the check-in told us absolutely come back tomorrow at 9:00 a.m. so both my brother and I took off work and we were there at 9:00 only to be told they are in a meeting every morning at 9:00 to 11:00 and no one could speak to us. We repeated we were told to be there and we took the time off work so the social worker met with us. The social worker promised us a phone would be put into our mother's room, she would be engaged in activities, and have weekly reports from them. For the next 5 days I called and emailed trying to find out the status of the phone as my mother's friends and other family outside of the area were not able to get in touch with her because the phone had yet to be installed. A week and a day later we had an in-person meeting and during the meeting they finally brought the phone after my brother again reiterated where's the phone. Oh and the social worker and management is only there 9:00 to 3:00 which is why I could never catch them because unfortunately those are the hours I work so the phone calls and emails were the only thing I could do. I would had to take time off work to try to catch one of them and have a conversation because apparently that's the only way you can do it is to show up in person. This place has lots of activities, but they never once offered the activities to my mother. I was there at least four times during activities. They never even attempted to come in the room to offer Bingo or any of the other activities. Mom was scheduled to be released Monday and no one on the floor was aware. But the business office was aware and I had left a message the Wednesday before to get no return phone call . Took them a while to do the paperwork and said that if I left before they had the paperwork done I would be leaving against medical advice which could mess up getting the referrals. So confused since the Business office was fully aware we were leaving on Monday but apparently didn't put the order in until Monday morning while I was sitting in the social services office trying to find out about her referrals that also had not been done yet. Outpatient therapy referral not done at discharge social services said they would call me with it in a few hours. Here we are over 24 hours later not a word from them. I had to do all the follow-up and finally was able to connect with the company a day and a half later to be told, Now that they have the referral it'll be another 24 to 48 hours so we're talking home for 5 days with no services. Food is horrible!!! You request items to be delivered instead of the normal food and half the time you don't get what you ordered so why ask you if they're not going to follow what they ask? Even the nurses said bring her snacks as good as awful. So why keep the person? Why are they still employed? Why don't you get another cook? Can't you give these people good food? Especially at a rate of $550 per day for room and board which doesn't include anything other than the bed and the food If I could have given this place a lower rating than a one I would have. I would never recommend this to anyone which is very hard for me to say as I've had friends and family stay at this place before and it was a class act, but that is no longer the case. I'm not sure if it's because they are no longer Manor Care or what is going on but I would never put a person I know in this facility. I was even told by the occupational therapist they don't have time to check their messages or their emails. So then when you call and I call you back because you lefy me a message what good does that do? I asked her ana she copped an attitude basically telling me that I should have been available when she called if I wanted to know things. So apparently you have to be available 24/7 and can't have a life or work so they can get a hold of you on their schedule. The front desk gal was super friendly when we first arrived and the next day when I went to visit showing me how to check in. She was the first impression of the place which gave me high hopes I just wish management could understand that we are family members that work. While I understand it is their job where they're at, they have lost the humanity to see us as family members who aren't available 24/7 and we're counting on them to take care of our loved ones and sometimes have questions. Their lack of respect for that and their lack of empathy was extremely disappointing. I sat in the social service office while we were doing paperwork and listened to phones ring that don't even bother to answer them. But I know that because I had left multiple messages never to get a response. My overall opinion is avoid this place at all costs. read more

The care provided at Fountain Valley Post Acute was generally satisfactory in terms of nursing, food, and physical therapy. However, I encountered significant issues with the social worker, Mireya, and her superior, April, both of whom are case managers (with April likely being Mireya's supervisor). I am a family friend assisting Tom, who had been in the ICU at Memorial Care, then transferred to Oncology, and eventually to Fountain Valley Post Acute for rehabilitation. Tom underwent a triple bypass surgery in 2023 and was later diagnosed with Stage II esophageal cancer. The cancer treatment nearly led to his death, requiring him to be brought back to health before his transfer to the rehabilitation center. Tom's current landlord expressed discomfort with his weakened state and fear of him falling while walking upstairs and terminated his stay and therefore he is need of finding another place. My concern was to have his Medi-Cal, Senior Housing and IHSS in place at the same time, however; 45-90 days would be when the papework would be processed and I mentioned that the hospital has a fudiciary obligation to make sure the patient had a safe place before releasing him, instead they got angry and discharged him to another facility and did not care or want to help. What if this was your realtive? would you want a rehabilitation center to treat your loved ones this way? Its very unforetuneate that when we all get old, that the elderly has no say so, they are relying on other people to help. I am a good friend, speak up for the unforetuneate and fight for unfair treatment. At Memorial Care, the social worker Christina was exceptional--professional, helpful, and responsive. Unfortunately, my experience with Mireya at Fountain Valley Post Acute was quite the opposite. Despite my efforts to communicate via email, Mireya responded dismissively and unprofessionally, indicating that Tom had no viable options due to lengthy wait times for government programs and providing no alternative resources. She insisted that Tom would need to find his own resources for housing and financial assistance. My subsequent inquiries were ignored by both Mireya and her manager, April. After providing honest feedback about Mireya's unprofessional conduct, which I felt was not suitable for working with elderly patients, Tom was informed that he would need to be transferred to another rehabilitation center by Saturday. This response seemed to be a consequence of the negative feedback, rather than a genuine concern for Tom's well-being. It is disheartening to encounter a lack of empathy and support from a facility that caters to elderly patients. The focus here appears to be more on filling beds and making money rather than providing comprehensive care and assistance. I strongly recommend seeking alternative care options for your family members to avoid a similar experience. Don't go here !!! read more
