After the procedure, the tooth became painful to chew on.  I chose this office because I believed a holistic and "conservative" approach would emphasize precision and preservation of tooth structure. Unfortunately, my experience was the opposite. What started as a small hole turned into a very large and deep filling that now covers a substantial portion of my tooth and is painful to chew on.  I waited about 10 months before writing this review because I wanted to give the tooth time to recover and during this time I sought second opinions from other dental professionals. An endodontist told me that a root canal is not currently necessary, but they advised being cautious with additional work, such as a crown, because further treatment could potentially "push the tooth over the edge." At this point, my tooth is still sensitive to chew on, and my understanding is that treatment options are limited, expensive and extensive.  I also found it concerning that immediately after the procedure with Dr. Khaki she told me I needed a "deep cleaning" because my gums were bleeding significantly. When I got home, I noticed a fairly large cut in my gums near the area where the work had been done. I did not get the deep cleaning done, instead I went to a different dental professional who told me my oral hygiene was very good and that I only needed a routine cleaning. I understand dental outcomes can vary, but based on my personal experience, I regret having this filling done at Division Street Dental.

For about a month he experienced horrible pain, so he went back and Sims said he still had fragments of the old filling and offered to fix it for free (as he should). During the second attempt not only did Sims split his lip to the point of bleeding while he was numb, but he buffed away the dentin just below the gum line, which caused severe pain. When my partner went back to see him since he was in such pain Sims threw his arms in the air and said, "what do you want me to do about it?" After seeing a new dentist, they suggested MI paste which helped immensely. Why Sims did not think to suggest this or show any amount of care, I can't say. Fast forward to today, my partner's tooth broke during breakfast. I don't know if these two incidents are related, but it makes me wonder. Regardless, Hamilton Sims did a poor job buffing away dentin, and when asked for help he didn't apologize or show any sign of compassion or care. DO NOT GO TO THIS DENTIST!!

I received treatment at Macadam Dental in June 2025 after experiencing tooth pain. A pre-determination for crowns and buildups on teeth #2 and #3 was submitted to Delta Dental on July 3 and approved July 8. I was not notified by the office that the approval had been received. When I asked about it at my September 8 appointment, I was initially told the office had not received it. After I explained that I had received the approval, staff located it in their system. Macadam Dental then referred me to an endodontist for root canal treatment prior to crowns. The endodontist subsequently requested that Macadam Dental perform tooth reduction before the root canal procedure. On October 2, Macadam Dental spent several hours performing tooth reduction on two teeth. During the procedure, the dental handpiece grinding my teeth kicked back while it was in my mouth and was briefly loose. I experienced immediate distress and screamed. After the anesthetic wore off, I began experiencing ongoing and increasing pain involving at least two teeth that had not previously been symptomatic. I was concerned by the poor response I received from Macadam Dental afterward and sought evaluation and treatment from additional dental providers who were more responsive to my distress. Resolving the situation and determining a path forward took months. During this process, I discovered that Macadam Dental had submitted an October 2 insurance claim representing crown services as completed, even though the definitive crowns had not been seated. I had a temporary restoration placed by Macadam Dental that day; the permanent crowns had not yet been made or delivered to the office; and the root canal treatment that had been identified as necessary had not yet occurred - so far from complete as Macadam Dental represented to Delta Dental. I disputed the claim with Delta Dental. Delta Dental subsequently recovered the payment from the provider. When I spoke with a Delta Dental representative by phone, he told me that he considered the billing issue to involve fraud. I am reporting that as what was communicated to me; I am not independently making a determination about intent. I also requested copies of my dental records from Macadam Dental. I did not initially receive them. After I filed a complaint with the Oregon Board of Dentistry, I received records, but I continued to identify what I believed were missing or incomplete portions and had to make additional requests, with the Oregon Board of Dentistry copied on the correspondence. My concern is not simply that something went wrong during treatment. It is the combination of the delayed follow-up regarding an approved treatment plan, what occurred during the October 2 procedure, the subsequent pain and difficulty obtaining appropriate care, the insurance claim for crown services that had not been completed, and the difficulty I experienced obtaining a timely resolution and complete records. Since leaving Macadam Dental, I have consulted additional dental practitioners about what treatment is actually necessary. One subsequent practitioner evaluated me and determined that I did not require the root canals that had previously been recommended. I was also advised that the extensive tooth reduction that had been performed was not necessary. I am now dealing with the permanent loss of tooth structure resulting from that treatment. I am sharing this experience so that other patients can ask questions, carefully review treatment plans and insurance claims, and understand what treatment has actually been completed before proceeding with additional care. This review reflects my personal experience, my understanding of the records available to me, and statements made to me by the professionals involved. I am not attempting to make an independent determination about professional misconduct or intent.
