I submitted documents for an ADA Accommodation to this office and was charged $50 to have the…read moredocuments filled out. The documents my employer provided me for said Accommodations were filled out in a half-hazard manner. The Diagnosis code was not even filled out. The woman in a Gym-jumpsuit with the fancy expensive bracelet gave me attitude and informed me "You dont have a diagnosis" Which i responded I did and even provided the proof of the Diagnosis. I informed her that she specified "No" in the section asking if i had a limitation and she rebuked "Thats not covered by ADA" which i corrected her and told her it was. She took the document, put her initials, scratched out the "No" and put the Dianoses on the line and STILL did not completely fill out the documentation where the document asks her to specify what adjustments to the work environment would enable me to perform essential functions of the job" This is entirely unprofessional and unbecoming and borders on being negligent and not meeting the standard of Care. As Such, If the Accommodation request is not approved, I WILL be filing a grievance with the Texas Medical Board for Professional Incompetence, Unprofessional Conduct, and Sub-par standard of Care. It is inexcusable that they can Charge for documents and when someone pays, They take a half-assed approach to providing what is needed. They also took around 4 days to respond (they did mention it would take that long) but they did not leave a voicemail to let me know of the completion of these time-sensitive documents. The same person answered the phone and put me on a recorded message where I was asked to leave my DOB, number, etc., and they would get back to me. I have not heard from them.
