But after touring the facility and now having two family members cared for here, I can confidently say those reviews did not reflect the reality I experienced. As a healthcare professional, I understand how these environments work. Rehab centers are not hotels. They are clinical settings caring for people who are often very sick, sometimes incontinent, and recovering from major medical events. Some reviewers seem to misunderstand that. Yes, you may occasionally smell something unpleasant, but that is part of the nature of this level of care. What matters is that the staff respond quickly, and at St. John's, they do. I also noticed several complaints online about the food. What many people don't realize is that meals in rehab facilities follow medical diets, safety guidelines, and physician orders. If something isn't working for a patient, the dietitian is available to discuss alternatives, adjust preferences, and help find options that meet both nutritional needs and personal tastes. Complaints about food are understandable, but they are also easily addressed if you communicate with the team. The nurses, aides, therapists, and administrators have been consistently kind, respectful, and attentive. Any time I approached them with a question or concern, they addressed it. They cared for my mother after a serious lung infection and helped her regain her strength through their excellent PT and OT department. She left functioning close to her baseline, which is a testament to how hard the rehab team worked. My father is now recovering here after a hip fracture, and the rehab team has been just as dedicated. Pushing him appropriately, aligning goals with me, and keeping his recovery on track. I owe a lot to St. John's. If you choose this facility, stay engaged, communicate openly, and set realistic expectations for your loved one's recovery. My family's experience has been nothing but positive, and I'm truly grateful.

They always calling her by name, engaging her in meaningful conversation, and paying attention to her preferences and routines. I visit several times a week and have never seen anything but genuine compassion in their interactions. What really stood out to me was how clean and peaceful the facility is. Her room is bright, well-kept, and always smells fresh. The common areas are warm and inviting, with comfortable seating, make the space feel more like a home than an institution. I know how hard it is to place a loved one in a facility, but Kisha (Guest Services Coordinator) has given me peace of mind. From the Therapists, to the Nurses and Pysch, they take a heartfelt interest in their clients wellbeing. As for the rest of the staff, there is a lot left to be desired. Aside from a few shining stars the majority of the administrative and tech staff behave as if they don't want to be there and its just a paycheck to them, to outright treating the Clients like inmates, cussing at them, threating them ("if you don't like it, pack your shit and get out") ignoring them by not following up on requests and grievances to total and utter disrespect. I've heard it said "you don't pay for this your insurance company does". I for one take great exception to that as I worked for over 45 years and paid into the system to be able to take advantage of programs I paid for (How dare they say that). Also, when asking a question I was often told "that's not my department or its above my pay grade)??? I would then asked to whom do I need to speak with to get my answer, their response was mostly "I don't know" How do you work somewhere that you don't know? The buck gets passed so much you can't recognize the bill any longer. Which brings me to ownership, The Husband and wife team of Mr. and Mrs. Gedale Fenster, a self proclaimed, "successful podcaster" and "motivational speaker" expounding his wisdom as if he were the next Ghandi, throwing out concepts that have been around for hundreds If not thousands of years. I for one found the man to be boastful, arrogant and disingenuous. They don't have any compassion or empathy for their clients, their entrepreneurial opportunity in this business is their primary reason for entering into this field. Complaints of living conditions go unaddressed, from inoperable washers and dryers to basic maintenance items. They hire drug addicts, exconvicts and alcoholics whom have little to no management experience, other than they were clients, in a field where that should be a priority. They allow clients to "Audition" for jobs instead of going to group to work on themselves or maybe its their way of getting cheap labor. They think by putting "Lipstick on a pig", by purchasing new furniture and putting a fresh coat of paint on it will solve a lot of the issues, think again. Just last weekend one of the units on the second floor of the IOP residence water lines broke and flooded that unit. When the client staying in that unit offered to go buy the necessary parts to fix it and have one of the clients whom are auditioning for a job, fix it the client was told no because the director of facilities said "I'm not going to allow a client to fix a repair, it will be taken care of Monday, and to have the Clients living in that unit put a bucket under it and dump the bucket when it was full, well you can imagine what happened, the downstairs unit got flooded and did a lot more damage than was necessary, if the director would have just let the Clients fix it. That's what you get when you hire incompetentence to run your facilities. Then there's the human care aspect of this, or the lack thereof. They treat clients as insurance payments. I was directly affected by this. I was told this past week that unbeknownst to me I was automatically enrolled in Medicare as I turn 65 on November 11th and Medicare trumps my current insurance "Florida Blue" and was unceremoniously discharged, with no resolution from Evolutions other than "No Dice" you have till Thursday latest Friday to pack up and get out. An unconscionable way to run a Mental Health and Substance Abuse Treatment center. Also, when I was leaving yesterday, I had to leave my bags there for a period until I got settled into a hotel, when I retrieved my bags one of the bags was missing that had all my toiletries and medication in it, after I informed a tech he went room to room looking for it, then my bag suddenly appeared, however, when I got to my destination and opened the bag one of my medications , and some of my toiletries were missing. After calling back to the IOP residence and informing them that some of my things were missing, they did nothing about it. They market themselves as "South Florida's Premier Treatment Center", it's nowhere near that, it's a run of the mill treatment center. I would like to caution anyone, if you are contemplating Mental Health or Substance Abuse Treatment, to do your due diligence and find a more suitable Treatment Center. I would not even rate this with a star, that would be saying they have a glimmer of hope. They also have a sober living facility in Fort Lauderdale where they let former male and female clients live together in the same apartment as couples all the while discouraging coupling, and whom also happen to work for Evolutions in supposedly high ranking decision making positions. Ownership holds no one accountable. What do you expect.

These are the facts, I wouldn't send my worst enemy here.. oh and if you ask to be removed from their follow up/alimni list they'll tell you "we can't do that".. the audacity of these people it's just a people farm and all they care about is getting your insurance $$$. Lastly when I left after 24 hours they held my valuables hostage, phone, wallet, electronics etc. I had to come back a few days later to retrieve it. Mind you if I was a young girl in Miami Gardens walking around with no way to call for help what an awful situation to put someone in, and don't forget about the rash and bed bug bites all over my body. 0/5 wouldn't Recomended if it was the last rehab on earth.

Based on reviews, I chose Wilton Manors Health and Rehabilitation Center. Unfortunately, our experience was extremely disappointing. Upon arrival, my mother received poor care and was not provided with meals appropriate for her diabetic needs. The food served to her and other patients was unacceptable in both quality and presentation. When I raised concerns, the facility's nutritionist responded with an unprofessional attitude. It is alarming that such standards are tolerated in a healthcare setting. Several nurses responsible for administering medication displayed poor bedside manners and showed little concern when my mother's blood sugar levels were high. The rehabilitation program was inadequate--she received only five minutes of therapy after four days in the facility (arriving Thursday evening, therapy began Monday). This lack of timely intervention severely impacted her recovery. The only positive aspect was the kindness and professionalism of two staff members who took vital signs and changed bedding. They genuinely cared for the patients. If you have no choice but to select this facility for your loved one, I strongly recommend being actively involved in their care to ensure they receive proper attention. After 10 days, I removed my mother from the facility and brought her home. Sadly, she still cannot walk and will now require physical and occupational therapy at home to regain mobility.

It is simply a place for people who have no family to lay in bed all day every day and die. Luckily I was able to get my mother out of there after three weeks of her health deteriorating. She is now in memorial regional Hospital, which is wonderful. The nastiness of the employees at this facility is something I've never experienced in my life. They treated my mother like garbage and they treated me even worse. this place needs to go out of business.

They are available to the family, return phone calls and immediately deal with any concerns. Cody the Executive Director is always available and incredibly caring. Perry the Food Director, does a great job to please the patients' palate. The Activities Director is engaging and did a great job for Mother's Day. The PT staff is exceptional and helped my Mom so much. The only problem is that she only received 1 hr. per day. (possibly due to AETNA's policies. So if the only issue you have is mobility you may want to go to a facility that specializes in mobility. Cecile the discharge nurse worked very hard to get Mom what she needed upon her release. Lastly, the staff is human so always be involved in the care, medication etc. of your loved one. If a mistake is made this group of people immediately try to rectify it. I would use this facility again if needed

This is a very clean facility with a number of dedicated staff. The food is acceptable, but not…read morenoteworthy. A new patient must quickly learn the ropes or care could be perceived as unsatisfactory. IT IS ESSENTIAL THAT PATIENTS HAVE AN ADVOCATE to work with the facility. Nights and weekends appear to be staffed at minimum levels. Several aides only do the minimum required unless prompted by the patient or advocate. Physical and occupational therapy here is excellent with 1-1.25 hours daily for each. The Director is easy to talk with and does care about problems. The same is true for the day shift unit manager. Discharge requires a strict protocol along with a home evaluation. The facility has a strong tendency to have Medicare purchase much unneeded equipment, despite objections. Oxygen may also be "forced" to be used even if the patient did not require it at entry. Summary: your loved one will receive proper care, but may experience delays in care at times due to staffing. Final discharge was a very disjointed process.

LONG POST ALERT How…read moreis this a "CareRite" facility when they can't even provide care right? This place is straight up fraud. As someone who works in the medical field, it is disappointing what I witnessed during my dad's stay at this facility. I'm going to break down my complaints by department. Nursing- Most of the nurses are either new or incompetent. I encountered a few nurses that knew what they were doing but on multiple occasions I found myself having to elaborate on how to do things for my dad. I have seen nurses ignore doctor's orders for my dad and when I got to the facility I would always end up having to report it to the nursing supervisor. I have pictures to prove it. Luckily the nursing supervisors there are nice. The CNAs also for the most part kept my dad clean but he did tell me he had to get up on his own multiple times risking falling just to go the restroom because his call light would go unanswered for a while. Lastly infection control there is not good. Multiple patients on isolation precautions but no nurses in sight wearing the proper protective equipment, just going from patient to patient. Case Management- I honestly don't know why they are there because they never provide any updates on the patient's status. Weekly meetings are done yet no updates provided to the patient's family. Not only that, they switch patient's insurance without the consent of the patient and bill for unnecessary services. Fraud at its finest. Discharge is so unorganized since they use a third party company to arrange everything. Patients are at risk of leaving the facility without the proper things arranged at home which cause unsafe discharges. God forbid the patient stays after their discharge date due to their lack of planning, then they threaten you with a bill. The case management team is rude and not helpful except for one person. The grievance officer there also useless and rude. Housekeeping- My dad came into the facility with a duffle bag full of clothes and belongings and left with pretty much nothing. They lost everything and use items on other patients. I reported missing items multiple items and was told a grievance would be filed and if the items couldn't be found I would only be reimbursed if I had receipts of everything I bought that went missing. Absolutely absurd. PT/OT- The therapists are nice but the therapy provided to the patients here are a joke. My dad honestly was stronger went his first came than when he was discharged. Front Desk- I have no complaints. They were honestly the best part about this facility. Very nice and professional, very helpful. Overall if it was up to me my dad wouldn't have stayed at this facility for as long as he did but he needed some type of rehab and was limited on choices based on his insurance. This place boasts about how they provide such good care and therapy and how they involve patients family on progress but it is all lies. Avoid this facility. UPDATE: I got a phone call from the administrator trying to resolve the situation regarding the missing belongings. I was promised reimbursement and still haven't received it. That was 4 weeks ago. If the administrator can't even resolve a simple issue, I wouldn't trust putting your family here!

It's really been an excellent experience, 5 stars all around. The front desk / office experience was superb. There's a very 21st century check in kiosk that has very cool technology ( an AI program recognizes that you are nodding yes to answer or spells out what you said...sort of like being checked in by Siri). The main reception is very businesslike. The care staff is also uniformly friendly. You can just tell from just about everything they do, that most enjoy what they do and genuinely care about getting good outcomes for patients. My friend says the food is quite good, especially by hospital / healthcare commissary standards. The design layout is also well done, with almost every room and hallway getting sunlight, including a large central courtyard. Signage is easy and clear. The most impressive and surprising thing was how fast everything fell into place, and the organization. Within 15 minutes of my friend's first day arrival a fellow named Sam (who's sort of the case manager / concierge) came by to introduce himself, followed by other staffers pretty quickly.
