One of the office ladies lost the paperwork we gave her in September. This was frustrating since the disability coordinator kept sending messages to turn in this form, after we already did. After two and a half months of going back and forth with two offices and having the paperwork lost, Kim C., one of the receptionists, came to the rescue. In less than five minutes, she completed the online form for us that no one else could figure out was available online. She also trained another employee on how to fill out this form. Everyone in this office is friendly, but huge kudos to Kim. My husband was finally paid for his last week of disability, thanks to her! P.S. During my last visit, Dr. Provenzano mentioned that he would retire soon. I wanted to make sure he knows he is appreciated for his work and hope he enjoys his retirement.
