Hear me out , first of all don't rush me off the phone because I shared with you that I went to see my PCP about a sensitive issue . Instead of saying ,"we don't need two people in the kitchen " AND "you can get your PCP to do that from now on ". ... How about you be compassionate and listen and we resolved it . Instead you're on a time crunch to get to your next patient and didn't wanna hear what I had to say . Also , isn't it your job to treat ALL the diagnoses . But that's okay you are a nurse practitioner NOT A doctor. That's why I was respectful and said I will get my PCP TO continue to take care of my needs . I been diagnosed ADHD combination my two other places but he " didn't see it " ; so he chose not to really get to the root of my issues . I'm glad I'm medicated because that could've went WAY worse . This is because upon leaving my appointment while I was scheduling my next appointment I brought up needing a note stating I'm in compliance with my treatment and am cleared for my foster parenting licensing. Well the font desk people asked me to wait while they ask about that and had left and come back several times with different answers. First it was that we would have to pay for that note of clearance which is quite silly, then they said I need to see them more than once to get that note which I can understand how that may make more sense but then they came back and said foster care requires a extremely lengthy and in depth evaluation which is a downright lie and I don't appreciate being lied to or misled we are literally almost done with the foster parent licensing process we're done with classes and our certifications and the agency told me that I only need a simple SHORT note saying I am in compliance with my treatment which can be done in like one sentence, then they came back again and said they can't provide that kind of clearance because they don't provide an evaluation that far in depth which is a frivolous claim that is completely false because a therapist or even a counselor can give me the same clearance. If they didn't want to genuinely help me instead of misleading me they could have just admitted they are not equipped to treat me instead of lying and leading me to continue seeking treatment in your office because all I am is a quick buck and I wouldn't be able to get adequate treatment with a psychiatric medical professional that genuinely cares about my health and well being. My son now has problems even ordering from a restaurant menu. Dr. Matlack left my son a shell of his former self, the only medical issues my son previously had was anxiety. Once we switched Doctors and requested my son's medical records from Dr. Matlack's previously named business (this doctor has changed business names several times) we received a folder with approximately eight sheets of paper. The only doctor notes we received stated that we (the parents) were trying to get a disability recommendation from him. Remember; my son was a patient for 1.5-2 years. We never requested any recommendation, we were attempting to have my son treated for anxiety only. Please don't use this person who claims to be a psychiatric doctor. Yes it's expensive, but there's nowhere else that would help me or prescribe suboxone and a benzodiazepine. I know folks frown on this, but without one of them I would be a miserable anxious loser. I was driving to the methadone clinic everyday, having to wonder how long it would take, rush to work and repeat. With dr Augustine I am able to get a months worth of meds at a time, address my anxiety as well as keep a business and family going. When I was a heroin user I would easily spend $1500 a week and would be in a constant fear of my guy running out. He helped me get my life back and helped me help myself and my family. I gladly show up to pay $400 for the life I have now

These has been little warning via her office of these cancellations. However, the cancelation today was beyond a rude inconvenience as I was only informed of such upon arrival -- and was told that, as were the other incidences of cancellations, she "called in sick." I was told that she "had left me a message of the cancellation on MyChart." I don't log into my MyChart on a daily basis. And, when I do, it's usually because I'm informed via text that I have a new message regarding an upcoming physicians appointment. This behavior from a clinical mental health professional in unhealthy and immature.
