This is an outrageous facility. My loved one is there NOW, has no suicidal or homicidal risk, and is being kept there by force. No one will answer the phones, they doped him up on LITHIUM which is dangerous and unsafe he is NOT bipolar and never has been. I am a licensed LPC, I know the rules, they cannot forcibly keep him after the 72 hour hold when he has no risk. This is absolutely outrageous, I am reporting this to the Ohio counseling board AND the behavioral health board and getting a lawyer today. I have never been so disgusted with a facility! My husband was sent there by University Hospital because he said he was feeling suicidal now they are keeping him there against his will they won't let him bathe it's an unclean unhealthy environment he says. 1 star is too high to give this place.. My husband was in ClearVista to adjust his meds. Instead he was drugged. I tried to get him out. They wouldn't let me see him at all. They wouldn't return phone calls and when I did get someone they told me they had authority to keep him despite my poa. I complained to the state and also told them they would not be paid. They let him go after that. But the state did a review and cited them for all four of my complaints. If you care about your loved ones DO NOT let them go here. read more

This place is one of the worst inpatient facilities I have ever came across. The mental health techs have been giving patients their vapes and weed for pens to calm down patients since there's no cameras in the rooms they won't ever find out about this or about staff physically and mentally abusing patients. The rank and file staff are caring and compassionate, however, the management team is cold, terse with family and very uncaring including the CEO. They seem to be on a power trip and they lie. I have no idea what patient rooms are like as visitors don't get past the conference rooms where patients are brought to spend 30 minutes with loved ones. I called the Social worker director to talk about my wife's mental health issues. It took 9 calls over 2 days to get anyone to respond to me! Unimaginable! As she's been missing for 2 months now after a DV arrest and refused to talk to me about me telling them her autentic issues. Saying my wife dosen't want to talk to me, I was calling to talk to a clinician or Social worker to just give her Doctor a history of her issues. I would never recommend this place! I left you 2 messages for the CEO To call me.The CEO didn't call me back. I went in there to talk to CEO at 4pm to make an appointment. I got lambasted and harassed by the same social worker. The front desk told me They would call the police as I am stalking?again by your social worker. Who threatened to call the police on me for concern to over making a visit for an appointment? Social worker Saying "When we dont want to talk to you we dont call you back".Q Social worker verbally fought with me in the waiting room when I have a medical power of attorney, she told me that she my loved one could make her own decisions. My loved one has multiple issues your not aware of because the doctor hasnt had a conversation with me, ive been With Elizabeth every day for 18 months, maybe the doctor treating her should listen to my imput! I was admitted here by an unknown person on 7/28/2022. As I wasn't there voluntarily for 72hrs I remained for 11 days not being discharged until 8/8/2022 During my time I was terrorized by other patients (residents) with death threats & antagonized 24 hours a day on the Adult unit the 1st 60hrs I was there. The staff response to any behavior was slow and dismissive regarding the agitators. Their solutions were for the harassed to go to their rooms while they allowed the bad behavior to escalate. There was sexual activity between patients, drug use and violence that was categorically ignored by the staff. I was finally moved across the way to the Geriatric Unit with the idea of a more peaceful setting. Unfortunately the Geriatric Unit has its own set of serious problems. The elder neglect and abuse was immediately obvious. Men and women's rooms were mixed up in both hallways. The 1st evening I found a young Autistic man masturbating in my bed with my personal property. I began to realize that no matter what happened to you, if you reacted in any way, you would be staying at ClearVista longer & longer. This is because I was at one point unable to contact my family. I could not call any phone number that I had previously called if the Nurse or MHT working at the Nurse station decided that caller had upset me. When I confronted the Nurses I was screamed at by one and called pretty nasty names in a very unprofessional manner. Again, as was done on the Adult unit the more aggressive or threatening the patient (resident) became towards the rest of us - we would be the ones forced to suffer while the offending person would be allowed to escalate in their behavior. When a man continued to walk into the women's rooms the decision was made to lock all of us out of our rooms instead of assigning a MHT to watch this disruptive patient or many other options. So often we would all suffer when 1 patient would monopolize the already under staffed and 12hr shift employees, having to wait extraordinary amounts of time for medications, food, and even just water every day, every meal. One evening I was physically Assaulted in the face by a Male who was one of the worst antagonizers of them all. The assault happened on Weds. Aug. 3rd at @ 9:24pm. The staff witnesses refused MEDICAL aid, refused to CALL police as well. I called Information # 411 for Lorain P.D. Dispatch myself. The staff was very annoyed by this because it meant they had to send ppl to their rooms since Police coming inside with guns was a High Risk situation to them. I found this actually amusing and offensive at the same time considering I was just disgustingly assaulted and they did not care one bit. This man, Tyrone Munn, coughed up so much flem he filled his hand with it, ran up behind me, slammed me into a wall, held me there and smeared it all down my face from my eye- into my nose and into my mouth before he got off of me. I was so shocked it took @ 3-4 seconds to get away & turned around to realize what he had actually done. He also had a Nicotine lozenge in his mouth so my eye was burning immediately. I ran to the sink by the counter & frig to immediately wash my face but again was refused the right to medical attention. When the Police arrived I begged to be allowed to go to the E.R. and brought back but ClearVista employees said it was NOT THEIR POLICY TO ALLOW the Police to take me and I would have to wait for a medical Dr. until the next day. I NEVER received medical attention ever for my assault from any medical Dr. at ClearVista. In the next days there I watched 4 diff. patients come and go to get E.R. treatments and wondered why them but not me. I spent 5 more days there. I watched my attacker be released before me as well. There were definitely mentally ill people there that needed help. However 2/3 of the patients, including my attacker are actually only there because they are homeless or looking for sexual partners. I was shocked to find out how common this actually is. I have never been committed to a mental hospital before especially against my will like I was to ClearVista. I was literally drugged and woke up at this facility. The last thing I remember before ClearVista was having a seizure at Elyria Hospital. Somehow, for some reason I was "pink-slipped" and this is what happened to me. The place also took me off the Rx drugs my Dr.s had me on for my medical issues and started me on twice as many drugs all of which were for mental health disorders, none of which I have. These drugs made me go back & forth with mood swings and zombie like feelings. It was so confusing. Since my release and a two week follow up with the Psychiatrist of THEIR choice, I am restarting back on the medications for my actual Dx medical issues and already feeling more like myself again. read more

We have just had a horrible week with my spouse at Clear Vista in Lorain, Ohio. From not being "allowed" to speak to the Doctor to not being able to see his room (wonder why) to being told vastly different stories by staff members concerning medications and behaviors...we felt completely uncomfortable. The nurses did not want to hear about my spouse's medical history which compromised his care. He was so medicated, he fell out of his wheelchair and during visiting hours could not open his eyes or speak. When I finally got him to a different facility, the medical information they sent for him did not give all his medical problems, including the fact that he is diabetic and did not include mention that he has a hidden prothesis. We would not recommend this facility under any circumstance. read more

When my brother was at this facility he was asked to fill out a good google review by a Nurse named Ashley. In my opinion attempting to bully sick patients into writing positive reviews is unethical! I wonder how many of these 5 star reviews have been forced to be given by patients to scared to say no to staff. read more

I worked as the maintenance director for two years there. It was a constant struggle with purchasing materials needed to do the job. Sometimes they would have you wait till the end of the month/year to get what you needed, only one maintenance person in a behavioral health location so it was always a constant struggle. Salaried position and there was hardly any weekend where I wasn't bothered at home. Even though only maintenance person you are also considered management, so there's tons of paperwork and meetings. They also want a four week notice if you leave. I gave the standard professional two week notice and my banked vacation time/PTO was not paid out. Just know what you're getting yourself into. Corporate office is in Cincinnati and they work the same as well for the most part. You will always hear things are going to change and get better but they never actually do. read more

I WISH THERE WERE NO STARS!!!!!!!!!!!!!!!! After my mom was sent their due to acting out at a nursing home. The first time visiting with the staff i had my doubts ! During the first meeting i had asked to speak with the dr. and was told that she would relay info to him ( SO NO) also she didn't write anything down as i was informing her of my mom current situation and medical/Mental history as to why she was sent their to begin with. she seriously acted like she didn't care at all. Long story short about 8 days in they told us that she was being released back to the nursing home that she was ok now. Well all they did was dope her up so she would sleep all the time instead of working with her to try and find the right meds ect. to get her with it enough to be lucid enough to function. AGAIN i asked to talk with the dr and again to no avail. HORRIBLE place to be. The dr's and nurses have no Communication with family during the treatment , no care plan discussions short or long term , no updates , no notification of meds changing no notifications of medical changes like her having a UTI they had to treat her for and ect, not even allowed to speak with a dr. Clear Vista is a disgrace to mental health care facilities. My son was admitted there in 2021. He has dealt with mental health issues since 2010, so I have experienced many behavioral facilities in that time frame. Covid restrictions were in place, so my contact was restricted to phone calls. There was a nurse named Kathy who was blatantly the rudest person I have spoken with in a long time. My son also said she was not kind or empathetic during his stay. My son called me after two days into his stay, very rattled. Apparently, the social worker Tamika gave him legal counsel to take me to court to remove me as his legal guardian. I called Tamika several times, and she never returned my call about this disturbing behavior on her part. I then called the CEO, Mr. Slaughter. He called me two weeks after to discuss. I was unavailable to speak. That was the last I heard from Mr. Slaughter. The majority of employees were not concerned about the needs of the patients. There was a nurse named Dan who was truly kind and informative regarding my son's progress. Sadly, he was the only one. Please steer clear if this facility if you can. If your loved one is in need of inpatient care and this facility has the only bed available... refuse until another bed opens up somewhere else. ANYWHERE ELSE! read more

Had to leave at least one star but this place really doesn't deserve any!!! I MEAN , ZERO , ZILCH , NIL , NONE NIX , NOUGHT , ZIP , ZIPPO , DIDDLY SQUAT , I was just released from this joke of a recovery facility or whatever THEY want to consider themselves. Some of the staff genuinely care (housekeeping staff is very friendly)but the majority could care less. It is totally disorganized & needs to have someone who knows what they are doing redo their complete system. If you need something & ask for it, you WILL be pacified with a LIE. They seem to regularly loose patient's belongings and valuables When they finally found my clothes & offered to wash them one MHT said the washer was broken the one that offered said she could send them out a third one said it was not broken. This was about 2pm ask for them at 10pm was told they were in dryer I had some legal papers in my pocket they disappeared NEVER got them back & no residue in pocket. There is NO sign of maintenance most of the chairs DO NOT have pads on the legs so they DO NOT slide they dig into the flooring. Patients that threaten others with violence are allowed to roam freely. So outside of a few and I mean D.A.M. few this place is HELL ON EARTH I would never recommend this place to anyone who is looking for a place to put their loved one or their pet !!! read more

This facility is totally lacking, not only in compassion, politeness, and concern for the patients but also the laziness of their aides who constantly were just sitting around. I constantly had to ask the aides for the most basic things like TOOTHPASTE for my loved one and even the one day my loved one had paper towels stuffed inside their slacks because no one had given them enough adult paper pants like Depends to wear. How ignorant are these people that they do not periodically check to make sure each patient has the basics. SHAME ON THE STAFF HERE!! Not one employee who worked here (aides, social workers or even the RNs) ever approached me when I came to visit to say hello, tell me their name and what their job was. TOTAL LACK OF COMMUNICATION. They do not know how to communicate with someone visiting their loved one. What are they afraid of? Probably, visitors seeing that the care was lacking, obviously. We were never given a family consultation with the staff or dr. or even given the basic information on what the diagnosis was for our loved one. We had to ask each and every time for the basic information and even then the RNs would say "I don't know". Well go get the damn chart for our family member so you can then relay the information. That is only common sense. And several people working behind the desk out in the reception area when I came to visit and to sign in were so rude and arrogant. How hard is it to put a smile on and say hello and just be plain POLITE! If you are not happy with your job then GET OUT and find something else. The aide Mowanda was very polite to me and did go get toothpaste and more Depends but we always had to ask for everything. And your auto robot reponse to people who post reviews is more polite than a real human working at your facility. Take a look at yourself and your facility and your conscience to make some very basic changes. Its only COMMON SENSE or maybe you don't know what that is!! THIS FACILITY IS VERY LACKING IN EVERY WAY. I was admitted to the psychiatric portion of the hospital (it is also a home for the elderly). There were a few staff members here who absolutely did not know how to interact with psychiatric patients. The only one I can remember the name of is Allison. She treated us more like prisoners than patients and ALWAYS had an attitude no matter what. That being said, there were a good amount of caring, helpful staff. My favorite staff members were Karen & Leonie. When you ask one of those two for assistance they deliver quickly! The same cant be said for the rest of the staff who tend to forget requests a minute after u ask. Also, the food was horrible. The kitchen would NOT pay attention to specific dietary restrictions half the time & when they did they seemed to have no idea what foods to provide. Overall, being at ClearVista was a needed step to help me recover, but there are some issues with the staff. read more

Clear Vista is horrifying. I thought my mother would get help with learning to take her medication better and maybe some counseling since she has not gotten over my dad's passing yet from earlier this year. She was denied her medicines at the times that the PCP and Cardiologist had set up for her. The staff were extremely rough and rude to her to the point that she came home covered in bruises!! They allowed unstable patients to go into her room at night and terrorize her and throw her toiletries around and when I confronted them with my concerns and said I was taking her home. I was made to feel as if I was a bad son and did not care for her. They told me that since I didn't want to leave her there till they wanted to let her go then I need to pay they entire bill not insurance!! Stay away from there if you love your family members. They will either die in there or come home more injured. read more

Clearvista has been a nightmare since it opened. I would truly reconsider if you're going to place an elderly patient at this facility. The nurse manager is not mentally stable. The CEO is aged herself, and really doesn't want to know if the goings-on in the facility. She does back up staff, nor does she care about the patients. I've witnessed patients fall I -on the floor bleeding, staff over-medicate pts because "bigwigs are coming in and they don't want them making a scene. " Staff lacks knowledge and experience w/ patients, to truly be able to articulate and care for them. The sad truth is that, Clearvista is for profit, and loves insurance! They will typically keep your relative for up to 15 days in order to get the most out of the insurance. Chances are, that your relative will be sent back to you the way she came. Unfortunately there's not always a lot we can do with dementia or even Alzheimer's patients. Bottom line is this facility is not equipped to handle your family. Go somewhere else and find a real doctor or someone you can trust to help your loved one. Good luck! read more
