We've waited 15 - 30 min. for someone to help my mother use the restroom where employees sat at the desk where the alert lights are used and take their time getting to the room. Mom is often left waiting in a wet diaper. Employees are often in the dining room when it is not being used, unused rooms, or seated in the large gathering space on their cell phones. I particularly wish employees were more engaged with elderly residents in wheelchairs were gathered in the main entry. We were told they gathered residents here for social engagement - but no one is socially engaged. The lack of engagement between residents and staff on their phones makes Lakeview advance patient care a very lonely place to be.

No communication between family member and staff. I am not sure where you got your rating 3 stars, but it did not come from the Rehabilitation floor. Please treat our precious and most valuable senior with respect, no like they do not matter! They do matter!

I chose because it is a beautiful, classy building- maybe for some but my 3 day experience was the worst, most humilitating, uncomfortable, "rehabe" I have and will ever experienced. After total hip replacement I asked for rehab. My two friends went to other places and were given constant appropriate treatment. All this was, was a nursing home. My "rehab" was about 10 minutes a day with PT and OT, when they showed up. I was transported in a junky van, in a wheelchair, barely fit- lots of junk in back. I was wheeled in to find a small room, mattress about 4" thick of some kind of vinyl with only a stretchy cotton sheet that felt like a worn out t-shirt. When I moved it slid. Mattress was hard, very uncomfortable for anyone who had hip surgery, and came with 2 12" "pillows" that kept sliding off bed. Happily my friend brought up two pillows from my house because I was unable to get out of bed and retrieve the "pillows" on the floor. Blankets were like heavy knit sheets that constantly fell down on the floor and end of bed, not retrievable in my stat. . Regular nurse did a good job with meds when she came. CNA's or whoever they were were short, uncaring, and scarce. I resorted to walking the halls with my walker because lying on the plastic mattress cause for much pain. I asked for help getting my blankets back on bed from floor several times. "We'll get to it " My bedside tray was full of used and old stuff- no room for even my box of klenex. I walked out with my walker and asked if my room could be cleaned and blankets on floor at end of bed cover me as i was cold. "We'll get there". Also while i was asleep in mid afternoon someone put an egg salad sandwich on tray. Nothing else. Didn't clean. I saw them come in, and plop it down, but hours later when I woke back up I was afraid to eat it as it had been sitting there for hours. I went out and ask for some other food, Kitchen closed. No food. Tired of looking at the mess and blankets on floor I dumped all I could in waste basket and used my walker to push all the old food, blankets, used stuff out the door. Then went for a walk with my walker to get out of there. I heard one of the CNA's, busy with their cell phones and hamburgers, say "What's that in the hall?" I just kept walking. Ten minutes later they got off their asses and phones and cleaned up my room. I had to ask for a glass of water. Exhausted and in pain. I asked to be discharged immediately. Person was pissed saying "Now I'll have to change all the paperwork". Came in later and said I was approved to go but not until midnight because I had my 10 minutes of PT that day. I called my ex husband and had him pull up in front at 7 next morning. I loaded belongings on walker and hobbled toward to front door, and left. Nobody even knew I was gone. No followup, no calls, I was just gone, nobody cared as usual. Worst experience I have EVER HAD! PLEASE DONT GO TO THIS PLACE FOR REHAB- THERE IS LITERALLY NO REHAB AND THE MOST UNCOMFORTABE, HUMILITATING EXPERIENCE I HAVE EVER HAD! DIANE

The food is absolutely horrific. 95% of the time served cold and that's when you actually get the food that you ordered. They are often out of menu items that were ordered for your meal the night before. The establishment ran out of dairy products for two weeks. I ask you how do you run out of dairy? Unacceptable . They also ran out of soda and chips for over a week. Something that the residents look forward to. There is a juice machine in the common area that is cut off between meals so residents cannot go and freely help themselves. I was told it was a way to save money. They also ran out of Metamucil for a few days. How and why? Upon my transfer, the wrong home address was incorrectly, entered into my paperwork. The dosage for a prescription I've been taking for years was incorrect. I had been receiving an incorrect amount for my months long stay!! Rooms are supposed to be cleaned every day-- they are not. During my rehab my room was cleaned a total of three times. The laundry service was terrible and nonexistent. The only thing the administration is good for is GASLIGHTING!! I've never experienced anything like it! Concerning my care I was continually given false information and led to believe things, that in reality, were never going to happen. I only met my assigned Dr once. I often heard the director of nursing yelling at the CNA's! The CNAs work the hardest. They are all caring and doing the best job possible given their current situation. The nursing staff and events coordinator are caring as well. The CNAs are expected to serve all meals to all patients which in turn takes away from the care of the residents!! OVERWORKED!! Needless to say it's nothing but a toxic environment at Stratford Commons. I only had a one month rehab stay . What about the people that call Stratford Commons "home?" I am worried about them. I would advise staying away from all to Tutera run/owned rehab and living facilities!!

She was able to get up and transfer to the bathroom, etc., but the night staff put a diaper on her and told her to urinate in it instead of taking her to the bathroom. This was only the worst thing they did. We told the director of the facility and the nursing staff managers, and it happened two more times before I identified the specific CNA. The staff protected the identity of the bad apple in lieu of solving the problem. My family was fortunately able to move my sister before it happened again. The owners of the facility are making a ton of money and present their story as a caring family business but are another example of corporate greed. DON'T GO THERE!
