After what I've seen I know I'm referring all my patients only to them. I've never seen anybody being this careful and dedicated to provide high level of care. However , she was difficult to understand her due to her heavy accent. I asked her for the hospice packet and there was none. She did the best that she could but the language and cultural barriers were evident. Part of their advertised service includes a Bereavement counselor- none supplied . It also included a hospice chaplain that initially refused to come because it was Palm Sunday and offered to perform last rites over the phone- someone claiming to be a pastor or religious person ended up coming an hour later. With the current national focus on corrupt hospices, this agency should take care in the future to supply the services offered on their website- since we did not receive the hospice packet that are typically given to families prior to services being rendered.

The last thing families need during these difficult stressful times is added stress from terrible cavalier, and lacking service. They refuse to accept their horrible mistake which is the TRUTH. I did NOT have a problem with mom getting discharge I am very thankful for it as I am thankful she transitioned from hospice to home health, I did have a major problem with their disgusting discharge procedures, their refusal to take responsibility for the wrong information Maria gave me would affect my elderly mom's well being. My mother's discharge was atrocious and very poorly handled, yes she was given a 2 week supplies and equipment after discharge however this does NOT excuse them for the terrible treatment the last day. I would even consider it inhumane. 2 days before the medical equipment was supposed to be picked up a nurse named Maria informed me that NO date was actually set for moms medical equipment to be picked up. (Wrong information) 48 hrs. later and about 1 hour before mom's bed time a receptionist by the name of Samantha informs me by telephone that law enforcement would be called out if I did not surrender the hospital bed, I was shocked and panicked and in utter disbelief. Yes I elevated my voice I was outraged for the handling of their mistake and the misinformation they gave me and to be told the police would be called out if mom did not surrender the hospital bed. I had to call public health to investigate their terrible discharge practices, ** the investigator advised me on 2/16/2024 to share my experience so others would be made aware of my experience. They did NOT care when I informed them that mom would be sleeping on the floor because of their terrible mistake, the case manager named Vera korkanovic declined to speak to me to defuse this matter, or to find a resolution. The owner Tali Fayfel refused to speak to me as well. She later calls screaming at me. The receptionist screamed at me talked over me and repeatedly told me moms hospital bed was going to be picked up that evening no matter what! In panic and in the evening hours I began taking out some medical equipment outside, I had no idea how mom's health would be affected sleeping on the floor that night. ****The only nurse that stood out from everyone was a nurse named DAMARIS she was consistently caring, professional, and patient. ** I did email hospice to let them know I would be sharing this horrible experience so other families would not have to go through this. Sorry if you view this as threatening. * Social worker did not return calls/ text messages for days. * Case manager refused to take my calls a number of times when I needed guidance for discharge. Based on direct knowledge and firsthand accounts, I have serious concerns about how residents are being treated. One resident I know personally was not properly cared for after a fall. After that incident, she was reportedly not assisted with meals or monitored to ensure she ate for three days. Her room has flooded twice, and because of how often this happens, it reportedly smells of mold, which raises serious health concerns. After the second flood, she was abruptly dismissed from her room so it could be cleaned and was not allowed to gather her phone, shoes, or any personal belongings. She was sent out in the middle of December wearing only sandals and no sweater. This showed a complete lack of concern for her safety, comfort, or dignity. What is most disturbing is the lack of empathy. This is a facility entrusted with the care of vulnerable seniors, yet the focus appears to be convenience and profit rather than basic human compassion. PLEASE STAY AWAY FROM THIS FACILITY!!!! Please choose another place. No one's loved one deserves to be treated this way.

Where to start......My mother in law has been living at AvantGarde for 2 weeks - she has Dementia…read more(short term memory issues) and it was getting to the point where she needed not only care but stimulation. The staff here is amazing, they don't just do their jobs (and they do them well) they are all very kind, supportive and loving toward her. This is a woman who would not leave her apartment, slept until noon, getting her to get dressed was a big task but her nature is social. She was famous in her youth and a singer / dancer. I could not believe that she was up and out of her room (DRESSED) by 8am and didn't return until after 5PM on day 2. EVERY SINGLE DAY she is kept active, she's singing with the pianist, playing bingo, watching movies, doing arts and crafts AND they told me she was participating in the morning walk! One of the ladies there sent me a video of her singing with the pianist today - she is in her element. Her Doctor comes to her every month (more if she needed him). I was weary that they would not be able to get her out of her room and get her dressed. AvantGarde has surpassed my expectations. I can travel without worry, I can visit her without me being the caretaker, I can rest easy knowing she is having a great time and in loving, capable hands.
