This home was for my father, and it was our 4th attempt at finding somewhere that would meet the standards of care we had for him. The home itself is inviting and welcoming with a large backyard where we could sit in the sun with my dad and a very safe neighborhood to go for walks in as well. The food quality was far and above any other home and they made each of the meals themselves. It always felt more like a family than someone taking care of my dad. Jamie, Leo and Amanda were incredible with my dad and I always felt so peaceful knowing he was being well cared for, never being left unattended or neglected. The kindness they all showed not only to my dad in their everyday interactions with him, but also to my mom, sister and I as we went through the painful process of watching him die, was amazing. At the very end of his life, he didn't wake or eat for 7 days, and if there was a moment we weren't by his side, they were right there. Jamie personally stayed with him through the nights singing to him and holding his hand. I can't describe the peace we felt knowing that while we went home to try to rest to regain strength, he was never alone. Jamie, Val and their staff operate with the utmost integrity, kindness and love for everyone who walks through their door-both the patients and the family. Losing my dad was the hardest thing we've ever gone through, and having him get the best care possible made that journey just a bit more bearable. It's difficult making life decisions for another, so I was pretty nervous about the whole thing, but Jamie and Val have proven to be absolutely wonderful. Not only have they taken great care of my mom, but they've helped her so much with her rehab. The location is bright and smells clean, the staff always welcome us when we arrive and have listened to us when we have had concerns or ideas in the past. Residents are welcomed to join each other at the dinning room tables of four, and Canfield also offers games, activities and exercise. There are groups who come in to the center to play musical instruments, sing, sell small items like candy/cookies, and perform. The trivia games are one of my grandfather's favorite activities to participate in. He tells us about a new game every time we visit. There's often Wii Bowling that is fun to watch as well as participate in. The nurses and medical staff at Canfield have made sure that my grandfather is able to have his medicine daily, on time and the amount that has been prescribed to him. This is an additional fee on top of the monthly rent, but has been worth the extra to make sure that every i is dotted, and t is crossed. We can feel confident in the attention that he's getting thanks to this helpful addition. The staff check with my grandfather's doctors about medicine updates, and are able to order perscribtions to arrive at Canfield instead of family members having to pick up from a local pharmacy and deliver them to the center. The option to have a personal car drive my grandfather to appointments has been another comfort that we're willing to pay for. There is a small fee, but it's personal service and the level of independence that my grandfather feels is huge. We love that he can tell us, "I went to the dentist today," thanks to this service. Each resident at Canfield wears an emergency call button on a bracelet on their wrist or around their neck. When my grandfather found that he couldn't get out of bed from his nap he pushed the button and immediately the staff determined that they should transport him to the hospital. Thanks to this quick thinking and response my grandfather has no lasting affects from this episode. The dinning room offers a rotating menu with options that appeal to my grandfather's tastes. The kitchen has recently been remolded and the dinning room space is open and bright! The residents are also able to pick from standard items on an additional menu. There are always options like oatmeal and bacon or eggs for breakfast. If it's between meal times there is tea, coffee and ice cream available. If a resident were to want to have a meal delivered to their room that is an option for an additional fee. There is also an option to have a staff member walk the resident to the dinning room (another additional fee). All of these options make the care at Canfield just what each resident needs. We have enjoyed holiday dinners hosted by Canfeild too. You are welcome to buy a meal and join residents year round. We loved the Easter brunch they served and my parents were so excited to share Thanksgiving dinner with my grandfather at Canfeild, NO DISHES to clean up! Bonus! My grandfather's room has a little kitchenette equpped with a microwave, refrigerator and sink. We originally picked Canfield because my grandfather was able to bring his small poodle with him (that's right, pets are welcome!). Canfield offered us a room where my grandfather is able to exit on the ground floor to let the dog outside, but they also offer the option of having a staff member let the dog out if a resident isn't able to (for a fee). We all know how important pets are, and how they can really affect our quality of life for the better. This option made Canfeild a no-brainer for us as a family when my grandfather was no longer able to live on his own. We are thankful that my grandfather is at Canfield and always know they will take great care of him!

From personal experience, when you have a parent with Dementia and things get to the point where…read moreyou can no longer provide for their needs in your own home, it can be stressful, confusing, and overwhelming. A good starting point is to pick up a copy of the Retirement Connection. It is a free publication that can be found at libraries and senior centers. The online version can be helpful if you are checking to see which facilities have current openings. Farmington Square is listed in here, though it does not distinguish between Memory Care and Transitions. There are 3 levels of care at FS. The first tier is Assisted Living. There are 26 private rooms in building B each with it's own bathroom. The next level of care is Transitions which is in building A. This is for residents with mild to moderate cognitive decline who need the safety of a locked facility so that they do not wander outside and possibly get hit by a car or wander off somewhere and not find their way home. The front entrance has a double layer of protection with 2 doors requiring a code to get out, so it is a bit more challenging for the residents to wander out and get into trouble. The 3rd tier of care is memory care for people with moderate to significant dementia and higher levels of physical care needs. There are 4 caregivers per day time shift in this building as there are quite a few residents in wheel chairs. This is the largest of the 3 buildings, with C and D combined into one large building with 3 different tv rooms. During the day, each building has a Wellness Director, a Med Tech, 1-4 caregivers and usually an Activities Facilitator from 10:00-4:00 pm. The buildings have high ceilings and lots of natural light. The furnishings and décor are light and modern looking. Aside from some scuffs on the walls from dining chairs, most things are in decent shape. The housekeepers are friendly and approachable and keep things clean and sanitary. The buildings and residents smell good much of the time. Residents on average receive 2 showers per week. Each room contains 2 call buttons with pull cords. One near the bed and one in the bathroom. Some residents prefer to keep a call button on a lanyard around their neck. When a resident pulls their call button cord an alert with a chime goes off on the care giver's cellphone indicating which room help is needed in. The response time varies depending on whether the care givers are available or busy giving another resident a shower, etc. The Med Techs if not busy can answer the calls as well. There is an activities calendar on the wall of each building. It is different for each of the buildings due to cognitive and physical abilities. The day usually starts with morning exercise and ends with bingo. There is a company van that takes residents to doctor's appointments. There is usually a scenic drive on alternating Mondays for the different buildings. When the weather is warm, there are outings to different nature areas. There is a full time nurse on staff who is there during business hours Monday-Friday and sometimes at varying hours on the weekend. First responders are called each time a resident falls or has any unusual type of health issue. Providence St. Vincent Hospital is in close proximity and is a clean, modern facility with nice rooms and decent food. There are 3 meals a day served at Farmington Square. There are 2 official snack times in buildings A and C/D where an employee will put glasses of water and snack items on a cart and offer them to residents in common areas and in rooms. If residents are hungry at other times they can ask any staff member to get them something from the kitchen. The kitchen is off limits to residents due to the fact that there are knives, etc. Some residents have small refrigerators in their rooms to store special treats. The meals are decent. They are not gourmet, but are hearty and basically healthy. They can be modified for diabetics and food allergies. I give the facilities 4 stars because overall I think it is clean and safe and that the residents are well cared for. The only downside is that the facility is located on a small piece of property which does not have much in the way of outdoor walking spaces. Building C/D has a nice outdoor space where there are some short walking paths and where luaus and other events can be held during nice weather. Buildings A and B each have a nice patio where residents can sit and enjoy the sun and pretty flowers during the warm months. For residents who get restless, like to walk for exercise, or need to drop a little weight for health reasons, this can be a bit of a challenge. The facility's current website is okay, but a bit confusing in that it really does not differentiate between "Transitions" and "Memory Care." They are in fact 2 different levels of care in 2 different buildings. Pictures are genuine, but missing some shots of the nice living rooms.

The food is decent but not great. They serve the same things over and over on rotation. You can…read morerequest a salad or sandwich instead if you remember to ask ahead of time. The older caregivers are usually more attentive, they don't play on their phones all the time like the younger ones with all the tattoos. The activities are ok, mostly the same thing over and over though. It would be nice if they had a library and a computer that everybody could use and teach us how to do more things on it. There is a magazine rack but mostly just the New Yorker, they need more variety. It's hard sometimes to find the medicine person when you need them. You have to search all around and they are gone a lot. It's hard when you can't find somebody to help you. They should take us out more so we can walk or swim or go to the library. It gets boring doing the same thing all the time.
