My father recently suffered a stroke and was there to receive rehabilitation services as well as wound care. At the moment, he is incapable of moving on his own, he can't speak and has a peg tube for feedings. The facility is extremely unsanitary, you can smell it upon entry. The staff responsible for my dad's care were either incompetent, careless and/or reckless. He was there a little over three weeks. I never saw a Nurse Practitioner and the nurses were rarely in with him. CNAs were the only staff that were easy to find. When we had questions about his care and/or services being rendered, no one could give us any information. The excuse was they weren't able to see the charts that contained the day's information provided by the staff from the previous shift. One day during a visit, I noticed my dad's breathing had become noticeably rapid. I asked my mom (who was also there) to call the nurse ( they always had to be searched for).When the CNA checked his vitals and realized it was really bad, she tried to contact the on-call nurse, and no one answered the phone! To make a long story short, my dad had to be rushed to the hospital because his blood pressure was extremely low, and he had a substantially high-grade fever. The hospital informed us that he was less than a day away from septic shock! He spent several days in the hospital, had to be rushed to surgery for an extensive wound cleanse and was given several bags of aggressive liquid antibiotics before he was even considered for discharge. If it were not for my mom and I being there with him to notice the change, he would not be here today. Thank God he was transferred to another rehabilitation facility for care after discharge from the hospital. He was put on antibiotics for six months, that should let you know how bad the infection was. Sidenote: The same day I noticed the change in his breathing, my mom had to run to the nurse's station to inform them about a patient (using one leg to move their wheelchair) who was outside alone and feet away from the busy street out front. Two staff members ran outside (one with NO shoes) to get the patient. Mind you, there is a "protocol" ( which they don't follow) to be buzzed in the doors before entry. This is designed to keep the patients safe and inside the building at all times. There was never a day I visited my dad that I had to be buzzed in, I was always able to walk right on in. THERE IS ONE STAR ONLY BECAUSE I COULD NOT SUBMIT THIS REVIEW WITHOUT IT!

His sodium levels bottomed out one night, so he became disoriented and weak - fell several times and hit his head on the tile floor. They didn't alert our family to what was going on, nor did they seek medical help. They locked him in his room and stationed someone at the door to keep him from coming out. However, no one was concerned about his health or safety. It went on like this all night. They finally called my grandmother later the next morning. He was completely unresponsive by the time they rushed him to the hospital. He was hospitalized for weeks after this and had permanent brain damage. Fortunately, space opened up at the war veterans home in Reserve while he was recovering in the hospital, so he was able to go there after. He passed away about 2 years later, and he never did return to the state he was before the trauma at Jefferson manor.

admin. dismisses all complaints regarding care and staff. many residents never see their cna's…read moreduring a full shift. cna's have been observed sleeping in a recliner in a residents room. nurse on weekends will not pass meds on a timely basis(morning med pass should be betwenn 7-9am and residents often will not receive those meds until 11am thus the noon time meds are not passed). this has been brought to the attention to the nurses supervisors and admin numerous times but they dismiss it. tratment of residents by numerous staff is neglectful and abusive.

After my sister had a stroke this year, this was the 2nd facility she was…read moretransferred to after experiencing horrible service and catching COVID 19 virus at the first facility (Colonial Nursing Home). She was sent to Nottingham because it was the facility that accepted COVID patients. My sister bounced back and forth between the hospital and facilities and my family and I are horrified by the lack of care, number 1 and number 2, the lack of professionalism. Im sorry to say that I was almost more relieved when my sister did have to return to the hospital for further care! This experience is a first for my family and has been a total NIGHTMARE!! Once we found out that my sister would be released to Nottingham, my family and I decided to call the facility to find out what to expect prior to my sisters arrival. After the horrible service we received from the first facility, , we learned to start asking more questions. I was told by a student staff member who does a great job answering the phone MOST TIMES (I won't say any names), (1) that we'd receive regular weekly video calls with our family member. (2) Each room would have a phone so we could stay in contact with our family member... along with some other broken promises. NONE OF IT HAPPENED!! My family was trying to quaterback all of this from the state of Texas unfortunately. I know that the home health care industry is generally a "lazy" industry, but COVID made it much easier for these nursing home employees to get away with this extreme foul behavior that they would never allow nor want for their own family. The process of having a member of your family needing a nursing home is ALREADY stressful enough, along with managing the medical issues that plagues your loved one. I would call to talk to my sister and would either be hung up on, no one answered the facilities phone, or they would lie to you to get you off the phone saying they would "get someone to help". I literally called this facility twice a day for 5 days -ON PURPOSE- and never got to speak to my sister. The last day of those 5 days, the nurse that answered the phone basically told me my sister "didn't want to talk to me"! THIS was the day I started really focusing my efforts on GETTING MY SISTER THE HELL OUT OF THIS PLACE... AND STATE FOR THAT MATTER!! I cant tell you what was going on in the facility, but if I have to assume that it was anything like what my family and I experienced over the past 6 mos (4 mos at Nottingham Rehab Facility) then I'm sure my guess would be that my sister received the below minimum required care in this place. The final straw for my family and I were the "alleged" sexual misbehavior towards my sister. Once we made a plan and decided to get my sister home, I called the facility THREE (3) weeks in advance to ask them to help us come up with a plan. I asked them for a medication supply, some medical equipment and any notes they could provide for us while we takeover providing care for my sister. I had to end up going to the director of nursing to get help with that (and that's ALL I'll say about that). I was told to speak with the facilities social worker and nurse practitioner (won't mention those names). The social worker was supposed to help us order necessary equipment that would assist us in caring for my sister in the meantime. The nurse practitioner agreed to call in her supply of medicines. I was initially told the equipment would be coming in a few days (this was week 1 of 3). Week 2 and 3 comes and the equipment NEVER came. I checked each week to get an updated status because I needed to gauge what type of vehicle would be required to carry out the transfer of my sister, her things and this equipment. The Friday before I was scheduled to pick my sister up, I was called by the social worker and told the equipment COULDN'T be delivered due to medicaid rules that states Medicaid ONLY sends out ordered equipment ONLY AFTER the patient is discharged. AND that one of the most needed items wouldn't be coming AT ALL. They told me that they could provide us with the wheelchair she's been using to send home with her to remedy the issue. I GUESS WE HAVE NO CHOICE AT THIS POINT.

He took over an hour explaining every detail of the Periacetabular Osteotomy (PAO) (fellowship trained with Dr. Tim Schrader, Atlanta) surgery. Dr. Faust offered several options in his expert opinion and was transparent about the pros and cons of surgery. Even though I left his office in tears, I knew could trust Dr. Faust and his team for my bilateral PAOs. I returned to running a quarter marathon, a tough mudder in Europe and a triathlon without hip pain. I am grateful Dr. Faust is able to provide this specialty surgery in Louisiana. I have no regrets. I am thankful everyday for my healthy hips.

Dr Robert Easton in 2016 revised my right hip. This was my 8th revision surgery. Dr Easton and his…read morePA and other staff were as good as it gets. Dr Easton is a very good surgeon and explained everything that was wrong with my last hip replacement. The socket kept separating from my pelvic bone and had rotated forward. Dr Easton explained that it would be a very complicated and complex operation but he was going to have five options to use if the one before that didn't work. Dr Easton got it on the first option and 6 hours later when I woke up in the recovery room I knew just by the way it felt that the surgery was successful. He is a very good listener and answers all of your questions. Praise God for Dr Easton and his staff. James Scooter Heine
