I tried going into the office with the letter saying my UH is going to be canceled on July 31,2026 if this form is not filled out. They just looked at me not offering to look into this. I'm 64 soon to be 65. I have some chronic medical conditions that I take daily medicine for. I can't take my meds without Health Ins and Prescription drug plan.
