It was a simple process to file the form online and the staff was quick and responsive. They dealt with Affordable Insurance/Foremost Insurance on my behalf. I've been told to expect my refund within a few days. (Unfortunately, the person with the Commissioners Office said Affordable Insurance told them they had not heard from me, which of course is untrue as I left messages that they did not return and sent a letter that was not answered.
