New management and they have revamped. Wonderful and encouraging employees. I've been there 3 times and it has definitely vastly improved.

My aunt came for rehab-to-home and left (to be taken to OU) with pneumonia. She was at Accel for 1 1/2 weeks with the lowest level of care I've ever seen a facility give a very elderly person, They took far too long to answer when she needed help, refused to give her appropriate food and consistently gave conflicting information about her medication, PT and vitals. Her medications were often forgotten or were doubled up. It was ridiculous how cavalier they all were about her needs and lack of wellbeing. I would never, ever recommend this facility. They will tell you what you want to hear about your loved one, but nothing changes. My aunt was literally screaming out for help during the night. Instead of being concerned, they were annoyed and closed her door. Terrible care, terrible staff - including the doctors - terrible food and attitudes. read more

I was rehabbing at Crystal Park last year after breaking my hip and a replacement and major surgery. These folks are awesome. They gave me all the encouragement and gentle nudge to get me back up on my feet and walking again. I had a stroke few weeks ago and they were going to try and send me to someplace way far away and I told him I said this is where I wanna Park and I refused to go anywhere else. Well today I learned I get to go back to Crystal get back on my feet walking again. You definitely will not be disappointed in this place. The staff the nurses are awesome. The food is so delicious and the therapist both are are just they give you the gentle nudge you need for encouragement without being overly pushy. It's like they know exactly what kind of encouragement you need to get up and moving If a loved one ever needs to this is the only place to go. I would give them 1 million stars if I could. read more

NO NO NO! Only place a family member at this facility if you do not love them or have no consideration for their care. I discharged myself after 16 hours. I pushed my nurse call button twice for post surgical pains meds. After 50 minutes and no response I gave up. I had severe pain after surgery 14 hours prior and unable to walk due to MS, I had to transfer by myself to my wheelchair and wheel 120 feet to the nurses desk. I saw two nurses/aides sitting at a desk. They CHASTISED me, saying they had other patients to take care of too. If so why were both of you at the desk? I was belittled and humiliated! What if I had fallen trying to transfer from my bed, what if my stitches had ripped open? Would I have been on the floor until a new shift change? I shudder to think what other patients who cannot get out of their beds must feel. No wonder at around midnight I heard a patient yelling "help me". This is an unsafe facility and I don't know how they still have their license. read more

This place sucks!!!! Do not put your family in there ever!! My mother waited more than an hour just to get someone to answer a simple bell!! Waited more than 24 hours for medication after she was admitted then tried to blame the place she was discharged from after they were sent two sets of discharge orders then a 3rd set, and I even had to call before they finally came in and gave her heart meds! I could go on for hours about how bad this place is!! They need an overhaul from the top down!!! I am even going to get the state involved!! I feel bad for any resident that has to stay there with no voice!! Even the one-star rating is too good!!! read more

I wrote a review about a year ago. Was really disappointed. BUT I am in a situation that I needed PT after leaving the hospital. Because of the previous pt care, I thought I'd give it another try. In less than 24 hours I have found that everything has improved tremendously. Place is very clean and the personnel has been changed. At this point, I would recommend Accel! This place is horrible. Do not leave you loved ones under their care. The resident routinely has had to wait hours for any care. The staff is (with notable exceptions) rude and very unhelpful. The resident often has to wait in her own filth for hours without any attempt by the staff to remedy the situation. It's also very dirt. The only good thing about this place is the pt staff. Everything else is trash. We will also be filing a complaint with the state of Oklahoma about the conditions of this facility. read more

My husband's father was taken to this facility for rehabilitation from the hospital. My mother in law was not allowed to be with him(even though she was with him at the hospital). He had to be transferred to the ER and the staff at Accel DID NOT notify my mother in law that they were transferring him, the ER was the one that notified her. I would not recommend this facility to anyone. read more

My girlfriends comment has been blocked from anyone else seeing it. Here is what she wrote If you care about your loved ones stay the hell away from crystal park. My grandma was placed there around a month ago after suffering from multiple strokes and was supposed to be receiving physical therapy. She cannot swallow food that isn't puréed and is unable to walk on her own. The first thing that bothered me when we placed her was that in the entirety of her stay they never once cleaned her laundry. They placed her urine covered clothing in bags and shoved them in her closet. The nurses and aides rarely checked up on her. She constantly had worsening bed sores and was never rotated when we checked. One afternoon we came in to her refrigerator being unplugged and all of the food/beverages we had brought for her were left spoiled inside. We then received a text from her phone to my mom saying that she was hungry and asked if we would bring her fruit and fried rice. She one, was unable to text at this point, and two, she knows she can't eat that. The text was incredibly suspicious and not at all how she would text. We asked her later and she said it wasn't her. The next time we came to visit both aides were sitting on their phones when we walked in to my 100lb grandma laying on the hardwood floor cold and alone. She had fallen and no one checked on her for I don't know how long. They're lucky she didn't break anything. During the end of her stay my mom received a voicemail from the business office, specifically a woman named Adrienne. saying that they had reported her to adult protective services for "abandonment" because they had been trying to reach her and she missed a call while she was working at her Dialysis job ACTUALLY taking care of patients. My mom and I were there every single day checking on my grandma asking for updates and nobody would tell us anything. My mom called her back and she was incredibly rude yelling at my mom on the phone threatening to drop my grandma (who cannot WALK) off at a homeless shelter. Before my mom finally straightened her out. This place is just awful. I watched my grandma wither away in this place. She's been at her new home for two days and has already shown drastic improvement uin speech and comprehension. Don't send your loved ones here please. read more

If you want your loved ones "misplaced", this is the facility for you. I went to the facility awaiting my mothers transfer from the hospital for hip replacement surgery. She was to start therapy there. I arrived and asked for my mothers room. They told me she hasn't arrived yet and I could wait in her room. When I went to her room, she was there in a wheelchair next to her bed, in the dark, and not moved to her bed right beside the wheelchair. The nurse was as surprised as I was that she did actually arrive already. read more

I don't recommend if you value your loved ones do not send them here! Not particularly happy with this facility, there's a couple of nurses that actually do their jobs other than that the rest of the staff is a joke. They have left my father who's recovering after being on life support for a month and a half in his own urine and fecal matter for extended periods of time. The person in charge of the facility only cares about receiving his bonus check every month so he goes in and, takes off a lot of things to make sure the numbers look right So they're always under stocked or running out of items that they need, so be prepared that you'll have to buy things for a family member while their in there. We've already had a tech last night tell my father that she's not going to clean my father up and that he needs to take care of it on his own mind you the only part of his body that he can move is his right arm. She tried even today to continuously make sure that we weren't reporting her to cover her rear. Another person again today just like over half the staff decided to make my father feel like he's a burden saying that he's pushing his call like too much because he's needing to get his diaper changed. My father already has a severe diaper rash and it's peeling and bleeding in certain areas, they don't want to do their jobs they'd rather sit back and be lazy. Tomorrow morning I'm going to be speaking with the person in charge of the facility, I will also be contacting a lawyer tomorrow and demanding that my father is transferred to a different facility whatever you do do not take your loved ones to this place. I've had to keep going back and forth to the facility to do these people's jobs for them, because they cannot be bothered to do it themselves I'm literally having to babysit the nurses and the techs to ensure that they're doing their jobs. And my father is now scared of one of the people working there this is not a safe environment. One more thing for me to add to let the person know. Something I forgot to mention. They tried giving my dad insulin AND HE'S NOT EVEN DIABETIC! I had to say something to the nurse. My first concern was my dad was a diabetic. He isn't thankfully. UPDATE: I talked to the regional manager shortly after. Nothing has gotten fixed three times they have tried to give my father insulin. For whatever reason and, last night the nurse walked in three or four different times knowing that my father needed to be changed and did nothing and, continued to let him sit in his own wet diaper. I had to call them up complaining to them and it still took 20 minutes before they decided to get up and go change him. They were sitting at the desk watching something on their phones and eating apparently what was more important then doing their jobs. My dad has yet to have a bath since he's been there. Last night I got so fed up I called adult protective services on them so now there is an investigation being launched. I will never recommend this place I value my family too much I shouldn't have to babysit staff just to make sure they do their jobs. read more

The physical therapy was good. The continuity of care is not there. It's part of this new COVID reality. They didn't do much with wound care. My mom's hip got infected. Her need for pain (Tylenol) medicine was not adhered to. It was not a good experience. We ended up checking my mom out and moving her to another facility. read more

This place is an absolute joke! They get $200 a day, accept patients, and then expect FAMILY members with no nursing experience to sit up there all day and night. Why? Because they don't have the staff to care for anyone! Wouldn't suggest this place to my worst enemy. Unless you are loaded with money and want to pay out of pocket for a sitter on top of what the worthless place charges! 24/7 care is false advertising for this place!! read more

I'm a Christian woman, so I'm not going to lie to you about our experience at this place. My mother was there for 2 and a half days. She arrived on 07/10/23. When she arrived, it appeared to be a pleasant place, but almost immediately, we noticed it was not. Mom had a hip replacement, and she was in pain when she moved, but she could move slowly to her bed on her own with assistance. So, after a couple hours being there, I asked them to transfer her from the wheelchair to the bed, I wanted to see how they were going to be with her, but before I could say "she has to move slowly", they literally pick her up (roughly) and put her on the bed. My mother SCREAMED OUT IN PAIN!!! They said "oh, so we startled her". No!!! You hurt her!!! From there, it went down hill. She had family with her 24/7 - we wont leave her there alone, but I don't think that made a difference. We pushed her button for help, they would come when the grand daughters or myself was there, but when we left and my 88 year old dad was there, they wouldn't come. She was crying, and left in pain for at least 2 hours until she would call me and I would come up there. I had to go look for the nurse to get her pain meds. It was horrible. After the second day, I told them she's leaving here. I took my mom out of that place and brought her home yesterday (07/13/23). Her spirit is shining bright again, she's not crying, or getting depressed. PLEASE... if you care for your loved one, please don't take them here. I wish I was rich.... I would build a skilled nursing home and make sure everyone is cared for by compassionate people, who love what they do, not just there because they need a paycheck. May God protect those there that are alone with no family. I can't even imagine what they go through. God, protect them. read more

They do not give you your medication daily, bath you on a regular basis, attend to you in a timely manner, care for you in a caring way. They are understaffed, they are mean and nasty, they yell at you. They have a few nurses that are kind, the physcial therapy staff is great but it does not out way the ill-treatment and neglect overall. read more

I am a patient here. It is terrible. Our medications are passed out to us at random times and not at scheduled times. They are understaffed. There are a handful of staff who seem to genuinely care for the wellbeing of their charges, and they are constantly stressed trying to pick up the slack of their coworkers. It has been more than a week since my room was cleaned. This place is Not at all what it seemed! The dementia patients are yelled at and treated with scorn. Very frustrated. I cannot say I would ever recommend accel to anyone I cared about. my little sister who is mentally chalenged has been there for 7 days. today is the 4th attempt to contact her nurse. no return call. i'm grateful it's not an emergency. as her sole caregiver i feel helpless. her benefits are limited so there is no alternative or i would gladly take it. I'm not at all comfortable with her care at accel. A warning review is all I can do at this point. I'm sure my issues will be taken care of eventually with some confrontation, which i'm not at all comfortable with. The facility itself is nice and clean. Looks are deceiving. read more

I should have realized that the reviews on SeniorAdvisor were a red flag for placing a loved one at this facility. Here is my experience: My mom was transferred here from the hospital in the hopes that physical therapy would improve her condition. Upon arrival, she lay in her bed for an hour and a half before someone came into the room to check on her. None of the day staff gave me any type of orientation or contacted me about a plan for her while I was in her room that day. After I sought help from some of the aides, it was clear they did not have the experience to handle a patient who was not taking food/medicine by mouth. As an aside, the facility appears to be woefully understaffed and to employ a fair number of newer graduates. I requested an IV be put in, so my mom could take medication and stay hydrated. After more than 2 days of receiving almost no food or water, staff finally put in an IV for fluid intake only a few hours before I had her moved to a different facility. Nurse "K" was the only kind and caring person I dealt with. On day 2, she put me in touch with an outside hospice management team as an option. During the hospice team meeting, the director of nursing services very coldly mentioned that my mother was not making any progress there after 2 days, implying that she did not belong there. However, when the time came to transfer my mom to a different facility, she flatly refused to arrange the transfer. To my relief, the receiving organization very graciously offered to arrange the transportation. Overall, I was astounded at the nursing services director's total lack of professionalism, not only in her interaction with me, but also in how this facility deals with a patient who doesn't necessarily fit into their "norm". read more

This place is awful! Mom was sent here from the hospital post surgery for rehab. She was here 2 1/2 weeks and I never got a call about her care or progress one time, despite asking numerous times. Mom is half blind and the staff here did nothing more than necessary to help her, with the exception of a couple of people. We had to ask for menus daily and the meals were laughable - no diabetic options and alot of the evening meals were not complete meals. She fell on 2 different occasions and I never received a phone call to let me know, even though that is supposedly standard protocol. After raising a big fuss about it we were told someone would call us. That never happened and mom was discharged with one day's notice. While the location was convenient, everything else sucked. So after my 24 Dec and 25 Dec review remarks, on 25 Dec Mr Kubala's niece called you because she couldn't get an answer on Mr Kubala's in-room phone. In interview with your staff nurse and attempt to communicate with Mr Kubala with her assistance it was readily apparent his condition had significantly degraded and family had not been informed. He had not been seen by your staff physician though his condition upon admittance and his rapid degraded state clearly pointed to need for medical intervention. His niece had to insist your staff nurse call 911 and have Mr Kubala immediately returned to Mercy Hospital after about a 48 hour stay with you. -- I received your stock review response 1/02/2020, 9 full days after asking you to contact me - 8 days after posting my first review. -- Mr Kubala passed away at 6:36 am 1/02/2020. -- not much left to talk about except the missing wristwatch and two bags of missing clothing/effects that cannot be located though Mr Kubala's niece says your staff did conduct a search for them when she visited your facility in an attempt to recover them. In addition to an admission medical review by your physician, I would suggest your facility record a complete inventory of possessions immediately upon admission of any patient. Apart from what I've stated in this review I don't believe there is any further reason for contact. I know you have received some glowing reviews. That's great, I'm glad for the families whose needs you've met. We simply were not one of those families. read more
