These were not minor clerical issues. Some of the information I questioned was significant enough that, if left uncorrected, it could follow me in my medical history and potentially affect future treatment decisions, particularly regarding pain management and pain meds I raised these concerns in July and submitted the required amendment forms. Unfortunately, the process was far from straightforward. I had to repeatedly advocate for myself, provide supporting documentation, and push for corrections that I felt should have been addressed more readily. Although some issues were acknowledged and corrections were agreed upon, I later found disputed information appearing in subsequent notes. More than 60 days after submitting my request, I still had not received a complete resolution. My September 1 and September 25 emails went unanswered. I haven't heard from them since August. The physicians were kind, but the difficulty getting serious documentation concerns addressed has significantly affected my confidence in this practice. It has also left me questioning my comfort with a physician assistant independently ordering procedures, being that he took it upon himself to change what my orthopedic surgeon sent me there for and on top of that sending me for a procedure that couldn't have even been completed because he overlooked the fact I had a spinal fusion, even though it was discussed multiple times and in my files. Patients should not have to work this hard to ensure their medical records are accurate, especially when those records could influence future care. They should also be able to trust that ordered procedures are appropriate and can actually be performed, and that prescriptions or instructions from their physicians are not overlooked or disregarded. I encourage patients to carefully review their medical records and billing statements.
