The Therapists on staff at Transitions Physical Therapy really know their stuff! I've been a patient there for many years and have been treated for three different issues: my foot, my hip and now the pelvic floor; latter is what I was just treated for. And I can't believe the difference! I feel like a new person and can hardly believe the changes! Should you have any physical therapy issues don't wait another minute before calling Transitions PT. Your better health, increased mobility, and pain relief is very achievable and just around the corner. read more

I have gone to Transitions for all my injuries and they have always exceeded expectations in getting me back to 100%. I love how they not only help you rehab your injury, but increase your awareness in your own movement. All of the staff are easy to connect and develop relationships with, which makes the rehab process much easier. I highly recommend Transitions to anyone in the Chittenden County area looking for a new PT! read more

Established in 2012.

The reason why we opened Transitions Physical Therapy is because we know that physical therapy can be done better. Our mission is to change your expectations about physical therapy, by making lives better through conversations, the exchange of knowledge, the delivery of specialist care, and the building of relationships within a community based practice.

Meet the Business Owner: Sean F.

Sean holds bachelors degrees in Health Science and Biology, a Masters degree of Physical Therapy from the University of Vermont, and a Doctorate of Physical Therapy from EIM Institute for Health Professions. Sean is currently 1 of only 150 Postural Restoration Certified therapists in the country. Sean is a member of the American Physical Therapy Association in the Sports PT and Private Practice sections as well as The Performing Arts Special Interest Group (PASIG) and the EIM PT Executives Masterminds group. Sean is also a proud member of the Performing Arts Medical Association (PAMA) and provides services for the St. Michael's Playhouse and numerous local performing artist.In addition to athletes and musicians, Sean enjoys working with patients of all ages and abilities.
