Try repeatedly to push the patient out the door even though the patient is no where near ready to leave. Your surgeon or PCP have no opinion on their patients care. They should be the decision makers. Instead the office staff blames early release on the insurance reviews which are completed by people in another state who only rely on the notes from staff. So unprofessional, & cold! Only Marywood's physician who is elderly himself makes the decision on when they are ready to go. This is NOT how our senior community should be treated.

They knew her by name and every time I was in to visit several staff members would stop in her room to check in on her and say hello even if they weren't working the floor. She loved Kim the unit manager on her floor she said she always found a way to make everyone smile and brighten their day. The care you receive feels personal and that was really important to me and my family. If I could give less than one star, I would…read more My family entrusted Four Seasons with the care of my grandmother for a short 5-day respite stay while her primary caregiver--my mom--took a much-needed break. What happened during that short time was heartbreaking. When my grandmother was admitted, she was alert, communicative, mobile with a walker, feeding herself, and enjoying time with family and friends. Five days later, we picked her up to find her unresponsive, unable to speak or walk, visibly distressed, unbathed, and disheveled--sitting alone in a wheelchair with her belongings piled in the hallway. Her shirt was halfway up, bra and stomach exposed, her pants crooked, her hair a mess, and she emitted a strong odor suggesting she hadn't been bathed the entire time. She had multiple open sores on her buttocks and heels indicating she was not mobile at all during her five day stay. She was in distress and would shriek in pain when you touched her right side. No one from the facility called us to inform us of this drastic change. Not once. When we asked what had happened, the staff had no answers. No concern. No urgency. My mom was left to transfer my grandmother into the car and received no answers to what had happened while my grandmother was in their care. Getting her into the car took 30 minutes, and no staff offered to help--only a nurse on lunch break and passersby assisted. She remained in this deteriorated state for a week at home (unable to eat, drink, speak, walk and was in severe pain) and passed away shortly after. We were told by Four Seasons staff that her condition wasn't surprising because she was a "hospice patient with dementia." This kind of dismissive, negligent attitude is unacceptable. To make matters worse, after meeting with the facility's social worker, Christina Davis, to express our concerns, we were told a formal complaint would be filed and someone from administration would follow up. No one ever contacted us again. Three months later, I filed another complaint with the administrator and still did not receive a response. What my grandmother experienced in her final days should never happen to anyone. Families deserve transparency, compassion, and dignity for their loved ones. Sadly, Four Seasons provided none of those things. I strongly encourage you to please locate another nursing home and DO NOT PLACE YOUR LOVED ONE IN THEIR CARE.

Out of all the places I've seen visiting my aunt, this one is.....pretty awful. Super short staffed. 2 workers and 1 supervisor for the entire area. During business hours they tend do have enough hands on deck but in the evenings and weekends there is a dangerous level of inattention that goes down. Tell your loved ones to stay away from this place.

I have a long long tale I will not bother to bore you with. I have been absent from Yelp forever. I love Yelping so you know it was not by choice. I was sick, incredibly, horrifically sick. The kind of sick where you get a priest in to see you just in case. Long story short I lost a body part. To paraphrase Beatrix Kiddo to Sophie Fatale in Kill Bill Vol. 1, any body part you lose *will be a part that you miss*. I was flat on my back for over three months and needed rehab for my old lady results as well as navigating my missing bit. I ended up here. Don't ever let this happen to you. You have a choice, make it wisely. In my 30 odd hours here they failed to reattach my wound vac, failed to assist me in care that I needed and allowed some random person to wander into my room and scare the bejesus out of my half unconscious self. The food was worse than any I have ever encountered to boot. Granted I was turfed here on a Friday. It was miserable. Staff eventually got to me and were extremely apologetic but too little too late. Besides the atmosphere alone here is depressing and shabby as heck. TV is like the 70s. The smell is awful, everything is extremely worn and cheap. I wouldn't go back if they paid me. Ever!

I'm writing this review for the sole purpose of warning people that this is not the place to put…read moreloved ones if they are infirm and need constant care. My dad was in here for 4 weeks and it was one issue after another until my family promised each other that we would never return my dad here. The staff smiles to your face and then neglects the patient. My dad went in with no wounds on his body- and came out with a stage 4 (the worst) bedsore that is requiring surgery, and a stage 2 bedsore, meaning they were not turning him often enough. And for them to let it get that bad and not tell us? Untenable! They were basically lying to us. It took a bout of pneumonia and a bacterial infection to send my dad back to the hospital for us to find this out. And I'll spare you the details of the bacterial infection, but the hospital told us it was because they were not keeping him clean. Couple that with they have a "two hour window" for medications- they had my dad on a roller coaster of consciousness, not what was needed to get my dad home to heal. What galls me the most is that I personally went and had a meeting with the staff and the social worker to make sure we were all on the same plan for my dad. Every single one of his care givers said that they understood and would work to get him home. Instead- he ended up in a hospital with sores on his body that I hope no one ever has to see on someone they love. We chose this home knowing it wasn't at the top of the list, but because only a handful of places take patients with trach tubes it was one of our limited choices that was nearby where my mother was. And because the staff seemed so nice, so caring. And know that it isn't cheap- it runs about $2000 more a month than other homes we looked at. I guess you really "don't get what you pay for". The Medicare website lists this as a "below average" nursing home. Wish I had seen that website before I allowed my loved one to ever cross the threshold of this place. Beware!

My grandfather was able to walk with assistance after he was discharged from U of M hospital. He was admitted to Heartland and did not see a physical therapist for 5 days. He deteriorated after that and has not gained back the strength that he had. They also stuck him in adult briefs so they wouldn't have to get him up to bathroom. My grandpa is alert and oriented so this was demeaning to him. Finally, trying to talk to a doctor is IMPOSSIBLE! I called and called and called and never got a call back. There is no communication there. Nurses don't know what physical therapy is doing and vis versa. I know there are not a lot of good nursing facilities out there but don't send your loved one here..

There were several aids that went above and beyond but they were the minority. The same goes for the nurses. I feel that there is not consistent care due to the constant rotation of staff. This lead to medication not being properly given and personal care lacking. If you are going to Optalis for short term this facility will be fine. However if you need longer term care, elderly or have dementia then the family will need to provide lots of follow up and monitoring.

I found Magnum online, these guys got a Dr to do a house visit that afternoon, thank God because he was on blood thinning medication, they set him up on a lower dose of warfarin, got a nurse to come visit him twice a week to make sure he was reacting ok to the med dosage and also got a physical therapist to come to his house twice a week! He is doing 100% better now, able to leave his house when he wants and seems a lot healthier after the medication change. The physical therapist helped him to be able to exercise on his own and strengthen himself to be able to do household chores and his hobbies, I can't thank you guys enough, he really feels independent and happy again! The service level at this facility is awful. My 85 year old mother was put in a room that seemed to…read moredouble as a storage room for old, scratched up furniture. Every day we went to the place there was no one to check who you are. You self checked in. There were times we didn't see a single person while walking through the place. Calling this place went to an automated system that routed to two nursing stations. One never answered with a message that said they would call back in 48 hours unless it was the weekend, then it would be Monday. Their social worker quit on June 10th of 2022, and they have not replaced her as of June 20th, but the phone system still sends you to her voicemail where she says she quit. Medications took up to 72 hours for them to get, including Pepto Bismal for my Mom. She went 3 days with nausea and diarrhea before they got her help even though they told us it would be done that day. Residents have no food choices. If you don't like or can't eat what they're serving you go hungry. Dinner one night was a cold turkey sandwich with cranberry juice. After 4 days we arranged for a new rehabilitation center and requested the paperwork to transfer her. No one seemed to know what to do. We were told multiple times that they were working on it, but the truth came out that they didn't know how to fill out the form online for the referral to the new facility. It took from Wednesday until Saturday at 2:00 in the morning until she was finally moved. 4 days later I received a phone call from this place asking to fill out paperwork for her. I asked if this person knew she was no longer there. She seemed surprised but said "Oh yes, we just need the paperwork for her file". If you care about your loved one, don't send them to this place.

She has access to tons of service she didn't get at home, like speech therapy, and so many more social opportunities that weren't possible when she was homebound- she plays bingo, gets her nails done, and even goes on outings like on the Diamond Jack riverboat! It's wonderful to see how much better her quality of life has gotten since moving here- I wish she had been able to come here years ago!
