(Many others report the same experience on Reddit.) Smoothie King customer service does not respond. I won't give my business to a company that mistreats their regular customers in this way. (UPDATE: After three months Smoothie King *finally* updated their system so that you can do a password reset. This is what happens when a company has their app development done by the CEO's half-bright cousin.)
